Esophageal squamous papilloma (ESP) is a rare benign tumor. ESP is generally detected incidentally during esophagogastroduodenoscopy, which is usually performed to investigate dyspepsia. We present three cases of this rare endoscopic finding. While two of our patients were asymptomatic, one had dysphagia, and the lesions were excised. Endoscopists should be able to make the differential diagnosis of papilloma detected in the esophagus and have knowledge about these lesions as they may carry malignant potential. Excision of papillomas is recommended for definitive diagnosis and treatment.
Meme kanseri kadınlarda en sık görülen kanser olup, morfolojik, klinik, hormon reseptör düzeyi, tedaviye yanıtlarına göre farklı özellikleri olan tümörlerdir. Günümüzde bu tümörlere yapılan medikal tedavilerde ER, PGR, HER2 düzeyleri önemli ölçüde yol gösterici ve sağkalımı etkileyici prognostik faktörlerdir.Gereç ve Yöntem: Kliniğimizde meme kanseri nedeniyle ameliyat edilen hastalarda yapılan tarama sonucu her üç prognostik ve prediktif proteini (ER, PGR, HER2) negatif olan 47 olgu değerlendirilmeye alındı. Ortanca 36 aylık takip ortaya konuldu ve yinelemeler değerlendirildi.Bulgular: Kırk yedi hastanın 23'ü (%49) 50 yaş altı, 24'ü (%51) 50 yaş üstüdür. Olguların çoğu invazif duktal karsinom (%78) olup ikinci sıklıkla metaplastik karsinom ve miyoepitelyal diferansiyon gösteren invaziv duktal karsinomdur. Olguların %36'sı nükleer grade II, %64'ü nükleer grade III idi, %36'sında anjiyolenfatik invazyon vardı. Olguların ortalama tümör çapı 3,6 cm olup; %14'ü T1 (0-2 cm), %86'sı T2 (2-5 cm), %10'u T3 (>5 cm) idi. Olguların %41,2'sinde lenf bezi metastazı saptanmadı, %58'sinde lenf bezi metastazı vardı. Olguların %68'ine modifiye radikal mastektomi, %32'sine meme koruyucu cerrahi yapıldı. Olguların %92'sine radyoterapi ve kemoterapi yapılmıştır. Otuz altı aylık takipte 17 (%36) olguda yineleme meydana geldi. Yinelemeleri 7'si lokal (%41), 10'u uzak yineleme (%59) idi.Sonuç: Triple negatif tümörler kötü prognozlu yüksek histolojik gradeye sahip, invazyon riski yüksek, sağkalımı kötü olan tümörlerdir. ER, PGR, HER2'yi hedefleyen günümüz tedavilerinden yararlanamayan bu grup hastaların yeni yaklaşımlara ihtiyaçları vardır.
