Introduction: Hemiparesis post-stroke usually results in locomotor limitations. As conventional rehabilitation is monotonous, the Serious Games (SG) represents an excellent treatment strategy, allowing to perform physical training in an interesting and enjoyable way. Objective: To evaluate the effects of an exercise program using the SG developed for hemiparetic stroke patients’ locomotor rehabilitation. Method: Non-Randomized Controlled Clinical Trial. Twenty-four hemiparetic stroke patients with subacute or chronic stroke (twelve men), mean age of 57.8 ± 10.4 years (injury time of 16.8 ± 19.6 months) participated in the study. The experimental group (n = 16) participated in an exercise program with the SG for lower limb rehabilitation. The control group (n = 8) received conventional treatment (kinesiotherapy). The intervention consisted of sessions twice a week for ten weeks. The following parameters were assessed: muscle strength (dynamometry), spasticity (Modified Ashworth Scale), functional mobility (Timed Up and Go Test - TUGT), and the gait speed (GS). Results: Both groups showed improvements, but the experimental group was better in all the studied variables, the muscular strength of the lower limb paresis and of the quadriceps femoris (p = 0.002; d = 0.7); and for the hamstrings (p < 0.001; d = 1.3), TUGT (p < 0.001; d = 0.4), and GS (p = 0.001; d = 0.4). Conclusion: The exercise program with the SG was useful for the patients treated in this study. The results showed a superiority of the SG regarding the conventional treatment in all the controlled variables. This was probably because of the greater repeatability of the exercises and the increased attention and motivation.
Como disfunções do equilíbrio são freqüentes nos pacientes hemiparéticos por acidente vascular encefálico (AVE), o treinamento do equilíbrio é meta fundamental em seu tratamento. O objetivo deste estudo preliminar foi verificar os efeitos da biorretroalimentação, por treinamento em plataforma instável computadorizada, em seis pacientes hemiparéticos por AVE (três homens e três mulheres, com idade média de 56,2 anos). Os pacientes foram avaliados, antes e após o tratamento, quanto a mobilidade funcional (pelo teste de levantar e caminhar cronometrado, TUGT na sigla em inglês), alcance funcional, qualidade de vida relacionada à saúde (pelo Perfil de Saúde de Nottingham) e equilíbrio sobre prancha instável. O treinamento foi feito na mesma prancha, em 23 sessões de cerca de 30 minutos cada, durante oito semanas. Os resultados indicam melhora do equilíbrio, em média, de 119,1% com pés separados e de 79,6% com pés juntos (p<0,001); melhora média do alcance funcional de 15% (p<0,001); melhora da mobilidade de 25,6% (p<0,001); e discreta melhora nos escores da auto-avaliação de qualidade de vida. O programa de treinamento resultou pois em melhoras significativas na amostra estudada, sugerindo que a biorretroalimentação do equilíbrio pode ser uma ferramenta valiosa na reabilitação de pacientes hemiparéticos em decorrência de AVE.
-In the recent decades the bioimpedance has been used in many areas of medicine. The technological advance of electronics has enabled the increase of bioimpedance applications. Measuring biological tissues is a hard work, which leads to the need of continuous research based on better methods and equipments for diagnosis. Many electronics manufacturers have helped the biomedical area through the development of new electronic components and equipment. The objective of this work is to adapt the impedance meter AD5933 for bioimpedance measurements. Two Analog Front-End (AFE) circuits were built with different Voltage Controlled Current Source (VCCS) topologies, which are a load-in-the-loop and a mirrored modified Howland current sources. Circuits were implemented and both two and four electrode measurements in the frequency range of 5 to 100 kHz were performed. Results showed that both front-end circuits are suitable to be used with the AD5933 at lower frequencies. Also, it might be a lowcost alternative for electrical bioimpedance applications.
Objetivo: Avaliar os efeitos de um programa de reabilitação baseado na combinação da Terapia do Espelho (TE) e Estimulação Elétrica Neuromuscular (EENM) para o membro superior de pacientes hemiparéticos por Acidente Vascular Cerebral (AVC). Métodos: Trata-se de um estudo pré-experimental envolvendo cinco pacientes com idade média de 53,8 (±7,6) anos, de ambos os sexos. O programa de tratamento consistiu em 20 sessões, com frequência de duas vezes semanais durante 10 semanas. Foram avaliados quanto a força e destreza manual, recuperação motora, espasticidade, independência funcional e percepção da qualidade de vida. Para análise dos dados foi utilizada estatística descritiva (médias e desvios padrões), e o Teste t de Student para comparar medidas de pré e pós-testes das variáveis controladas no estudo. Resultados: O tratamento baseado na TE associada à EENM trouxe benefícios significativos nas variáveis força e destreza manual, recuperação motora e redução da espasticidade, porém, não houve diferença significativa na percepção de qualidade de vida e independência funcional. Conclusão: Esta combinação parece potencialmente interessante para reabilitação do membro superior de pacientes hemiparéticos por AVC, especialmente para aqueles na fase crônica.
