Introdução: O nó verdadeiro de cordão é um evento raro, presente em 0,4 a 1,5% das gestações. Pode surgir em decorrência das mudanças da estática fetal durante a gravidez, sendo mais frequente em multíparas e em casos de cordões umbilicais longos. Relato de caso: Paciente de 34 anos, preta, casada, do lar, Gesta 2 Para 1 Aborto 0, com história anterior de gestação também gemelar há dois anos, de desfecho favorável. Foi encaminhada para o pré-natal de alto risco na gestação atual, com 18 semanas, com confirmação por ultrassonografia de primeiro trimestre de nova gestação gemelar, dicoriônica e diamniótica. Evoluiu assintomática e sem intercorrências no pré-natal, com nove consultas realizadas. Em sua última consulta, apresentava 37 semanas e seis dias e foi identificada ao exame físico atividade cardíaca presente em ambos os fetos, com frequência normal e movimentação fetal de ambos, quando foi encaminhada para avaliação na maternidade, com 38/39 semanas. Deu entrada na maternidade com 38 semanas e três dias, assintomática, mas ao exame físico não foi identificada atividade cardíaca de um dos fetos. Realizou-se ultrassonografia com Doppler à internação, com identificação de feto1 com atividade cardíaca presente e feto2 sem atividade cardíaca e visualização de interrupção do fluxo sanguíneo no cordão umbilical para o feto2. A paciente foi encaminhada para cesárea de urgência com nascimento de feto1 vivo, sem malformações aparentes e extração de feto2 natimorto, sem malformações aparentes. Confirmou-se à dequitação placentária que o cordão umbilical do feto2 apresentava nó verdadeiro de cordão, justificando seu óbito. Conclusão: O nó verdadeiro de cordão umbilical normalmente se apresenta de forma frouxa, com pouca ou nenhuma repercussão hemodinâmica para o feto. No entanto, sua apresentação de forma justa está relacionada ao estreitamento da luz vascular dos vasos umbilicais, com redução do fluxo sanguíneo ao feto, asfixia e risco até dez vezes maior de óbito fetal. Além disso, a gemelaridade por si só caracteriza uma gestação como de alto risco para o bem-estar materno e fetal ao potencializar o risco de complicações como as que podem ocorrer em uma gravidez única. Com base no caso acima, pode-se compreender que o diagnóstico precoce do nó verdadeiro de cordão é extremamente difícil, visto que o mecanismo etiológico desse evento é a movimentação fetal, que costuma ocorrer durante toda a gestação. Assim, se o estreitamento da luz do cordão ocorre de forma tardia, como no caso descrito, a monitoração do pré-natal por meio da ultrassonografia, bem como por exame físico, pode não identificar esse evento e resultar em desfecho desfavorável para o feto, de modo completamente imprevisível.
