Carotid-jugular fistulae are rare, but habitually cause morbidity and mortality. They are often linked with penetrating trauma, primarily caused by gunshots. This report describes the case of a patient who was the victim of a gunshot wound to the left cervical area, provoking a carotid-jugular arteriovenous fistula and occlusion of the common carotid artery. The fistula was corrected by ligature of the internal jugular vein and arteriorrhaphy of the left common carotid artery with a bovine pericardium patch.
ResumoA oclusão de ramo ilíaco de endoprótese bifurcada de aorta surge como complicação decorrente da correção endovascular do aneurisma de aorta abdominal e várias abordagens terapêuticas têm sido empregadas para o tratamento dessa complicação em casos de isquemia de membro inferior. Apresentamos dois casos de tratamento totalmente percutâneo da oclusão de ramo ilíaco de endoprótese de aorta abdominal com dispositivo de trombectomia rotativa e aspirativa seguida de angioplastia com stent primário, sem complicações operatórias.Palavras-chave: oclusão de enxerto vascular; trombectomia; aneurisma. AbstractIliac limb occlusion of bifurcated aortic stent graft appears as complication of endovascular repair of abdominal aortic aneurysm and several therapeutic approaches have been used to treat this problem in cases of lower limb ischemia. In two cases a totally endovascular approach was used to treat the limb occlusion by means of rotational and aspiration thrombectomy, followed by stenting. In both cases there were no postoperative complications.Keywords: graft occlusion, vascular; thrombectomy; aneurysm. IntroduçãoA correção endovascular do aneurisma de aorta abdominal (CEAAA) foi desenvolvida para um tratamento menos invasivo e com o objetivo de diminuir a morbi/mortalidade do tratamento cirúrgico em pacientes portadores de aneurisma de aorta. Ela tem se mostrado responsável pela redução significativa da taxa de mortalidade operatória, do volume de transfusão sanguínea e do tempo cirúrgico e de internação em unidade de terapia intensiva. Entretanto, requer maior núme-ro de intervenções secundárias para manutenção da exclusão aneurismática e/ou tratamento das complicações [1][2][3][4][5][6] . A intervenção secundária é definida pela Society for Vascular Surgery (SVS) e pela American Association for Vascular Surgery (AAVS) como qualquer procedimento cirúrgico ou endovascular realizado após a correção endovascular inicial 7 . O registro Eurostar 8 classifica as intervenções secundá-rias de acordo com a sua magnitude: 1) necessidade de laparotomia; 2) procedimentos extra-anatômicos; 3) intervenções transfemorais percutâneas ou abertas; todos em busca da padronização para melhor conhecimento e terapia das complicações da CEAAA.
Aortoiliac aneurysms are particularly challenging to treat using a totally endovascular approach, because embolization of the internal iliac arteries can cause major complications. Several conventional and endovascular surgical techniques have been described that offer preservation of at least one internal iliac branch and options for completely endovascular treatment include branched stentgrafts and the parallel grafts technique. Here, the authors report the first case of endovascular treatment with preservation of an internal iliac branch using a Brazilian iliac branch device.Keywords: abdominal aortic aneurysm; endovascular procedures; iliac artery. ResumoO aneurisma aortoilíaco tem representado desafio terapêutico principalmente em relação ao tratamento endovascular, visto que a embolização das artérias ilíacas internas pode levar a graves complicações. Inúmeras técnicas cirúrgicas convencionais e endovasculares têm sido descritas para a preservação de ao menos um ramo ilíaco interno. Dentre as opções de tratamento totalmente endovascular, podemos citar as endopróteses ramificadas e a técnica de próteses paralelas. ResumoO schwanoma representa etiologia rara de tumor benigno de laringe, tendo como principal tratamento a ressecçâo cirúrgica. O objetivo deste trabalho é relatar um caso raro de paciente jovem submetido à ressecçâo cirúrgica de schwanomalaríngeo,evoluindo, no transperatório, para complicaçâo hemorrágica grave enecessitando de embolizaçâo de urgencia.Palavras-chave: embolizaçâo terapéutica; schwanoma; laringe. The objective of this paper is to report on a rare case of a young female patient who suffered severe intraoperative hemorrhaging during surgical resection of a laryngeal Schwannoma and needed emergency embolization.
Erectile dysfunction (ED) is a common multifactorial disorder. The authors report a case of perineal trauma, evolving with ED for bulbar penile artery fistula to the spongy body, promoting an arteriovenous shunt and culminating with erectile deficit, penile congestion and pain. Patient underwent an endovascular embolization and is now at the 6 th month of ambulatorial follow-up. He returns to sexual activities, without complaints of penile rigidity and pain.
Pulmonary embolism is a major cause of death after traumas and, in the majority of trauma cases, the anticoagulation drugs usually employed for prevention and treatment of pulmonary embolisms are contraindicated. We report on a case in which a prophylactic and temporary inferior vena cava filter (VCF) was implanted in a multiple trauma patient at high risk of pulmonary embolism.
The pelvic congestion syndrome is defined by anatomical criteria, hemodynamic and clinical result of hypertension and venocapillary stasis in the pelvic area and can be extended to the lower limbs. The presence of varicose veins of the lower limbs may or may not correlate reflux of the saphenous system and, in cases of absence of saphenous reflux, the reflux pelvic presents itself as an important etiological factor responsible for recurrence of venous disease in lower limbs. The authors report a small series of three cases of young patients suffering from pelvic congestion syndrome associated with varicose veins of the lower limbs treated by pelvic embolization and surgical treatment of varicose veins of the lower limbs during the surgical procedure with good initial results and no embolic complications.A síndrome de congestão pélvica é definida por critérios anatômicos, hemodinâmicos e clínicos, decorrentes da hipertensão e estase venocapilar no território pélvico, podendo ser estendido aos membros inferiores. A presença de varizes dos membros inferiores pode estar ou não relacionada ao refluxo do sistema safeno e, nos casos de ausência do refluxo safeno, o refluxo pélvico apresenta-se como importante fator etiológico responsável pelo aparecimento e recidiva da doença venosa nos membros inferiores. Os autores relatam uma pequena série de três casos de pacientes jovens portadoras de síndrome de congestão pélvica associada à varizes dos membros inferiores tratadas por embolização pélvica e tratamento cirúrgico das varizes de membros inferiores, no mesmo tempo cirúrgico, com bons resultados iniciais e ausência de complicações embólicas.Palavras-chave: dor pélvica; embolização terapêutica; varizes.
