IntroduçãoA institucionalização da avaliação em saúde, como resultado da intenção de conferir racionalidade às intervenções setoriais 1,2,3,4 , vem ganhando força em diferentes áreas do setor públi-co em saúde. No Brasil, e mais especificamente no campo das políticas de saúde, a avaliação parece tomar um lugar central nas organizações 5 , apesar de essa atividade ainda se apresentar de forma incipiente, pouco incorporada às práticas, possuindo quase sempre um caráter prescritivo e burocrático 6,7 .Fatores como as mudanças nos procedimentos legais e administrativos na gestão do Sistema Único de Saúde (SUS); a maior complexidade do perfil epidemiológico do país; e a exigência cada vez maior do controle dos gastos em saúde podem ser apontados como fatores envolvidos no interesse crescente em avaliação de políticas, programas e serviços de saúde no Brasil 8 .No entanto, extrapolando a necessidade de avaliar a eficiência dos serviços e do sistema de saúde, outros fatores contribuíram para que a avaliação em saúde se firmasse como um novo campo de conhecimento e de práticas, destacando-se: a incorporação contínua de novas tecnologias; a ampliação da oferta e da complexidade organizacional dos serviços de saúde; e a crescente necessidade de informação sobre a eficácia, a efetividade e o funcionamento do sistema 9,10 .ARTIGO ARTICLE
Low-level laser therapy proved effective for the prevention of severe oral mucositis and intense oral pain in patients submitted to hematopoietic stem cell transplantation.
Objective: The aim of the present study was to determine associations between the late diagnosis of oral cancer and demographic/clinical factors. A quantitative cross-sectional study was carried out in 2007. Material and Method: Oral tumors were recorded based on the TNM staging system, with T1 and T2 considered early diagnosis and T3 and T4 considered late diagnosis. Results: Patient age ranged from 30 to 105 years (mean: 64.7). Just over half of the patients (54.9%) resided in urban areas and approximately 1/4 (25.7%) had an agricultural occupation. The majority had smoking habits (89.9%) and consumed alcohol (62.5%). The most frequent tumor site was the tongue (42.4%), followed by the hard palate (19.2%) and lip (12.2%). The majority (70.1%) were in advanced stages (III and IV). A greater percentage of smokers were in advanced stages than non-smokers. The percentage of cases in the early stages was lowest when the tumor was located in the tongue (14.1%) and highest when located in the buccal mucosa (80.0%). Conclusion: The main causes of the late diagnosis of oral cancer are insufficient training on the part of physicians and dentists in the field of pathology, delays on the part of patients in seeking medical assistance and deficient access to quality medical care, reflecting the absence of preventive public health programs and an effective healthcare system.
No Brasil, o câncer é a principal causa de morte por doença em crianças e adolescentes. O Instituto Nacional do Câncer (INCA) estima a ocorrência de 8.460 novos casos de câncer em crianças e adolescentes no ano de 2020.O objetivo desse estudo foi identificar as manifestações bucais mais frequentes nos pacientes pediátricos em tratamento antineoplásico. Essa pesquisa incluiu 137 crianças e adolescentes de 0 a 19 anos de idade. Os dados incluíram um questionário semiestruturado para verificar as manifestações bucais mais frequentes decorrentes da terapia antineoplásica. Os testes estatísticos utilizados foram o teste Qui-quadrado de Pearson e para avaliar a força da associação foi obtido o Odds Ratio (OR) ou Razão das Chances (RC) com respectivo intervalo de confiança. Os resultados encontrados demonstram que a idade média dos pacientes pesquisados foi de 6,8 anos; o sexo masculino (57,7%) foi predominante na amostra; a neoplasia maligna mais incidente foi à leucemia (67,9%); 70,1% dos pacientes apresentaram pelo menos uma manifestação oral, sendo a mucositede maior prevalência (56,2%), seguida da xerostomia (46,7%), gengivite (41,6%), disfagia (35,8%), disgeusia (35,8%), candidíase (34,3%) e herpes (21,2%). Encontraram-se 54,7% dos pacientes com saúde bucal favorável e 40,9% deles com saúde bucal desfavorável. Concluímos que o paciente infantil deve ser avaliado por um odontopediatra previamente ao início da quimioterapia para minimizar as complicações bucais durante o tratamento antineoplásico.
Children with cancer commonly present oral health impairments due to lack of orientation about oral hygiene, which should be directed to caregivers since they are essential in this process. We developed and validated a guideline directed to caregivers for oral hygiene of children with cancer. This exploratory methodological study developed an educational guideline in three stages: analysis of oral health of children attended at the oncology service; literature review and development of the guideline for oral hygiene; semantic, appearance, and content validation by dentists, education professionals, and the target population. We used the Educational Content Validation Instrument in Health, and agreements of ≥ 80 % among evaluators were considered to maintain or modify the assessed items. Professionals and caregivers were mostly female; the latter were predominantly mothers with low educational level from inland areas of Pernambuco state (Brazil). Most professionals had more than ten years of experience in pediatric dentistry. Agreement was > 80 % in all items. The content of the guideline for oral hygiene was valid and relevant to be used in children with cancer.
