A high rate of complications is associated with open reduction and internal fixation of Sanders type 4 fractures of the calcaneum. We assessed the long-term outcome of 83 Sanders type 4 comminuted intra-articular fractures of the calcaneum in 64 patients who underwent non-operative treatment between 1999 and 2005. Each fracture was treated by closed reduction and immobilisation in a long leg cast. Patients were reviewed every three months in the first year, and every six months thereafter. At each visit, the involved ankles were assessed by the American Orthopaedic Foot and Ankle Society (AOFAS) criteria. The degree of fracture healing and the presence of osteoarthritis were assessed. At a mean follow-up of 51 months (24 to 70) the mean AOFAS score was 72 (52 to 92). Osteoarthritis was scored radiologically using Graves' classification and was evident in the subtalar joints of 75 ankles (90%) on x-ray and in all ankles on CT scans, of which 20 were grade 0 or 1, 39 grade 2, and 24 grade 3. A non-operative approach to treating these fractures may be simpler, less expensive, easier to administer with fewer complications, and may be better tolerated than surgery, by many patients.
Bu çalışmada, amatör olarak spor yapan ve ön çapraz bağ (ÖÇB) rekonstrüksiyonu yapılan bireylerde, operasyonun fonksiyonel sonuçları ve sportif aktiviteleriyle olan ilişkisinin araştırılması amaçlandı.
Gereç ve Yöntemler:Ön çapraz rüptürü nedeniyle dört katlı hamstring otogrefti ile artroskopik rekonstrüksiyon uygulanan hastaların fonksiyonel sonuçlarını Lysolm ve Tegner aktivite skorlama sistemi ile retrospektif olarak analiz ettik. Kinezyofobi değerlendirilmesi Tampa ölçeği kullanarak yapıldı.Bulgular: Çalışmaya dahil olan 61 kişinin tamamı erkekti (yaş dağılım: 17 -48 yıl, ortalama: 30,6±6,9). Takip süresi en kısa 27 ay iken en uzunu 74 ay olmak üzere ortalama 47.4±20.5 ay idi. Ameliyat öncesi 59,1±27,4 (dağılım: 5 -90) olan Lysholm skoru son kontrolde 95,9±12,2 idi (dağılım 69 -100). Tegner aktivite skorları ortalaması travma öncesi 6,4±2,8 (dağılım: 3-10), ameliyat sonrası 5,2±3,5 (dağılım: 3-10) idi. Tampa kinezyofobi ölçeği ortalaması 41,2±9,7 (29-49) olarak bulundu. Sonuç: Cerrahi sonucunda, hastaların çoğunda fonksiyonel sonuçlar tatmin edici olarak tespit edilirken, yaklaşık yarısında ise aktivite skorları istenilen düzeye ulaşmıştı. Tampa kinezyofobi ölçeği yüksek bulunması, ÖÇB cerrahisinde ne kadar titiz davranılsa da ameliyat öncesinde ve sonrasında alacağı profesyonel fizyoterapi ve psikolojik danışmanlığın da önemli derecede etkili olabileceğini gösterdi.
Bu çalışmanın amacı AO tip C distal humerus kırığı tanısıyla cerrahi tedavi uygulanan erişkin hastanın orta dönem sonuçlarını değerlendirmektir. Hastalar ve Yöntem: Geriye dönük çalışmamızda, AO tip C distal humerus kırığı tanılı 20 hastaya (12 kadın, 8 erkek, ortalama yaş 44.3) posterior yaklaşım ile olekranon chevron tipi osteotomi ve Paralel Plak Vidayla tespit uygulandı. Olguların ortalama izlem süresi 74 aydı (56-94 ay). Olguların son kontrollerinde hareket aralığı ölçümü, Mayo Elbow Performance Score (MEPS) ve the Disabilities of the Arm Shoulder and Hand (DASH) score ile fonksiyonel değerlendirme yapıldı. Bulgular: MEPS ile değerlendirmede kategorik olarak; 6 olgu çok iyi, 10 olgu iyi, 2 olgu orta, 2 olgu kötü sonuç olarak belirlendi. Hastaların hareket aralığına bakıldığında ortalama fleksiyon 125.3°, ortalama ekstansiyon kaybı ise 13.8° idi. Hastaların %75'inde hareket açıklığı, fonksiyonel hareket açıklığı olarak kabul edilen 30°-130° veya üzerindeydi. DASH anketine göre ortalama skor 7.2 olarak belirlendi. Radyolojik olarak orta dönemde hiçbir hastada dirsekte artroz bulgusu ve implant gevşemesi tespit edilmedi. Sonuç: AO tip C distal humerus kırıklarında olekranon osteotomisi ve paralel plak vida ile tespitin kabul edilebilir bir cerrahi teknik olduğu söylenebilir.
