Serum sclerostin was raised in alcoholic patients, and it correlated with decreased markers of bone synthesis and increased markers of bone breakdown. The elevation in sclerostin levels was clearly related with liver function, but not with ethanol intake, nutritional status or concomitant HCV infection.

The trabecular bone mass (TBM) of the proximal epiphyses of right tibiae belonging to 273 prehispanic inhabitants of Gran Canaria (60. 81% males and 35.53% females) were assessed by histomorphometrical analysis of undecalcified samples and compared with that of 41 samples from El Hierro (52.63% males and 47.37% females). Among the prehispanic population of Gran Canaria 19.05% showed TBM values below 13.5% and 30.40% below 15%, although individual variability was high (range 6.71-35.4%). In sharp contrast with these results, only one case (2.44%) from El Hierro showed a TBM value below 15%, whereas mean TBM (23.50;+/- 5.60%) was significantly higher than that of the population of Gran Canaria (17.88 +/- 5.20%). The high prevalence of osteoporosis on Gran Canaria may reflect protein-calorie malnutrition. Prehistorically Gran Canaria exhibited a relatively high population density (30-40/km(2)) and a strong reliance on agriculture, in contrast with a lower population density (4/km(2)) on the island El Hierro, where the population consumed mainly marine products.

Serum IFN-γ values were increased in alcoholics, who also showed raised IL-13 and IL-10, but lower IL-4 levels. Given the immunomodulatory roles of IL-10 and IL-13, this increase may be interpreted as a compensatory rise of anti-inflammatory cytokines. We failed to find any relation with mortality.

This study was performed in order to analyse the prevalence, clinical characteristics and mortality of heavy drinkers among hospitalized patients during a 2-year period. Chronic excessive alcohol consumption (daily intake >80 g of ethanol for males and >40 g for females) was found in 278 of 2913 hospital admissions and was strongly associated with the male sex (90.69%). Heavy drinkers were significantly younger than other admissions (15 and 10 years for men and women, respectively), but showed similar mortality rates to other admissions, despite a much earlier age at death (19.5 years for men and 22 years for women). There was a trend towards higher mortality rates among severe alcoholic women than severe alcoholic men and non-alcoholic women. Liver cirrhosis was the entity most frequently observed in the heavy drinkers, and was significantly more prevalent in alcoholic women.
