Filho;. ANEMIA GRAVE RECORRENTE COMO ÚNICO ACHADO CLÍNICO DE RETOCOLITE ULCERATIVA EM PACIENTE ADULTO JOVEM. ABCDExpress. 2017;1(2):1161.Instituição: HOSPITAL UNIVERSITARIO PROFESSOR EDGARD SANTOS -UFBA APRESENTAÇÃO DO CASO: G.C.S., 17 anos, masculino, relato de anemia grave recorrente desde os 9 anos de idade, com necessidade transfusional de repetição e uso crônico de reposição oral de ferro, sem diagnóstico esclarecido. Paciente apresentava ainda episódios febris intermitentes, autolimitados, sem comprometimento do estado geral ou sintomas associados. Nega exteriorização de sangramentos, dor abdominal ou quaisquer crises álgicas. Refere ritmo intestinal sem alterações, com dejeções presentes, um episódio diário, fezes formadas, sem sangue ou muco e ausência de tenesmo. Relata que, aos 12 anos, apresentou lesões nodulares cutâneas, indolores, autolimitadas, em extremidade distal de ambos os membros inferiores, com resolução cicatricial em lesões planas e hipercrômicas. Exames laboratoriais evidenciaram anemia ferropriva, com necessidade de reposição parenteral de ferro, e elevação isolada de gama-GT (5x o valor normal). Eletroforese de hemoglobinas sem alterações. Autoanticorpos antinuclear e anticélula parietal não reagentes. Colangioressonância demonstrou padrão sugestivo de colangite esclerosante primária (CEP). Colonoscopia apresentava edema, hiperemia, friabilidade e ulcerações em reto, e cólon esquerdo, contínuo por toda extensão avaliada, compatíveis com Retocolite Ulcerativa (RCU). DISCUSSÃO: Anemia representa a mais comum manifestação extraintestinal das doenças inflamatórias intestinais (DII). Meta-análise recente revelou 21% de prevalência (IC 95%) dessa deficiência em pacientes RCU. Sexo feminino e atividade de doença são os maiores determinantes de anemia existente ao diagnóstico de DII. Essa deficiência de etiologia multifatorial tem como principais causas a deficiência de ferro e/ou vitamina B12, associadas à anemia de doenças crônicas. Anemia persistente ou recorrente por 3 ou mais anos tem correlação independente com hospitalizações, visitas ao especialista e cirurgias em DII, sendo um significativo fator preditor de maior morbimortalidade em portadores de RCU. No contexto clínico, esta comorbidade tem impacto negativo na qualidade de vida, interferindo em bem-estar, função cognitiva, e interação social nestes pacientes. COMENTÁRIOS FINAIS: Deve-se ressaltar a inpotância das DII nos diagnósticos diferenciais de anemias, visto que esta comorbidade pode preceder o desenvolvimento de sintomas intestinais de DII, além de ser importante sinal em revelar a presença de DII não-diagnosticada, particularidade observada no contexto relatado.
