Filho;. ANEMIA GRAVE RECORRENTE COMO ÚNICO ACHADO CLÍNICO DE RETOCOLITE ULCERATIVA EM PACIENTE ADULTO JOVEM. ABCDExpress. 2017;1(2):1161.Instituição: HOSPITAL UNIVERSITARIO PROFESSOR EDGARD SANTOS -UFBA APRESENTAÇÃO DO CASO: G.C.S., 17 anos, masculino, relato de anemia grave recorrente desde os 9 anos de idade, com necessidade transfusional de repetição e uso crônico de reposição oral de ferro, sem diagnóstico esclarecido. Paciente apresentava ainda episódios febris intermitentes, autolimitados, sem comprometimento do estado geral ou sintomas associados. Nega exteriorização de sangramentos, dor abdominal ou quaisquer crises álgicas. Refere ritmo intestinal sem alterações, com dejeções presentes, um episódio diário, fezes formadas, sem sangue ou muco e ausência de tenesmo. Relata que, aos 12 anos, apresentou lesões nodulares cutâneas, indolores, autolimitadas, em extremidade distal de ambos os membros inferiores, com resolução cicatricial em lesões planas e hipercrômicas. Exames laboratoriais evidenciaram anemia ferropriva, com necessidade de reposição parenteral de ferro, e elevação isolada de gama-GT (5x o valor normal). Eletroforese de hemoglobinas sem alterações. Autoanticorpos antinuclear e anticélula parietal não reagentes. Colangioressonância demonstrou padrão sugestivo de colangite esclerosante primária (CEP). Colonoscopia apresentava edema, hiperemia, friabilidade e ulcerações em reto, e cólon esquerdo, contínuo por toda extensão avaliada, compatíveis com Retocolite Ulcerativa (RCU). DISCUSSÃO: Anemia representa a mais comum manifestação extraintestinal das doenças inflamatórias intestinais (DII). Meta-análise recente revelou 21% de prevalência (IC 95%) dessa deficiência em pacientes RCU. Sexo feminino e atividade de doença são os maiores determinantes de anemia existente ao diagnóstico de DII. Essa deficiência de etiologia multifatorial tem como principais causas a deficiência de ferro e/ou vitamina B12, associadas à anemia de doenças crônicas. Anemia persistente ou recorrente por 3 ou mais anos tem correlação independente com hospitalizações, visitas ao especialista e cirurgias em DII, sendo um significativo fator preditor de maior morbimortalidade em portadores de RCU. No contexto clínico, esta comorbidade tem impacto negativo na qualidade de vida, interferindo em bem-estar, função cognitiva, e interação social nestes pacientes. COMENTÁRIOS FINAIS: Deve-se ressaltar a inpotância das DII nos diagnósticos diferenciais de anemias, visto que esta comorbidade pode preceder o desenvolvimento de sintomas intestinais de DII, além de ser importante sinal em revelar a presença de DII não-diagnosticada, particularidade observada no contexto relatado. TRATAMENTO DE HEPATITE C. ABCDExpress. 2017;1(2):955.Instituição: HUPES CASO CLÍNICO: KFH, sexo masculino, 57 anos, natural e procedente de Salvador-BA, diagnosticado com hepatite C em 2002, realizou primeiro tratamento sem resposta virológica sustentada com Interferon peguilado e ribavirina. Realizou novo tratamento em 2017 com Sofosbuvir, Daclatasvir e Ribavirina por 12 semanas com carga viral indetectável 3 meses após o tratamento. Em abril de 2017, fase pós-tratamento, o mesmo evoluiu com ascite e, em investigação complementar, foi detectado um nódulo hepático de 12 cm com "wash-out" na tomografia de abdome, diagnosticando CHC. Foi realizada quimioembolização do tumor, no entanto, paciente cursou com trombose de veia porta e hemorragia digestiva alta no pós procedimento, evoluindo com ACLF tipo 3 e óbito em Junho de 2017. DISCUSSÃO: O tratamento com as novas drogas tem tido altas taxas de cura, principalmente entre os pacientes não cirróticos. No entanto, existem relatos na literatura do aumento do risco de CHC após o tratamento do vírus C com os antivirais de ação direta (DAA), livres de interferon. Sabe-se que a rápida negativação do vírus no organismo pode estar relacionada a alterações da resposta imune do hospedeiro e aumento da expressão de proto oncogenes podendo estar envolvido no mecanismo das recidivas tumorais precoces e incidência de tumores mais agressivos. No entanto, esse achado está sujeito a alguns vieses, como o aprimoramento da tecnologia dos exames de imagem, estágios de cirrose mais avançados dos pacientes que vem sendo tratados com as novas drogas e pacientes mais idosos submetidos ao tratamento. CONCLUSÃO: Os dados da literatura ainda são controversos e mais estudos se fazem necessário sobre os possíveis efeitos colaterais e complicações pelo uso dos DAAs no tratamento de Hepatite C. ALCOÓLICA EM MULHER MAGRA. ABCDExpress. 2017;1(2):937.Instituição: HUPES CASO CLÍNICO: JGS, sexo feminino, 48 anos, sem história de etilismo, sorologias virais negativas, auto anticorpos negativos, história de uso esporádicos de chás (capim-santo), eutrófica, exames laboratoriais evidenciando LDL 190 , elevação de ALT e GGT , escore de HOMA 3,2. Endoscopia digestiva Alta com varizes esofágicas de fino calibre. Ultrassonografia com sinais de doença hepática crônica parenquimatosa e ascite. Biópsia hepática evidenciando esteatohepatite estádio 2, sem fibrose. DISCUSSÃO: O diagnóstico de esteatohepatite não alcoólica é crescente e vem se tornando a principal causa de cirrose, principalmente em países ocidentais. Os fatores de risco conhecidos para a doença primária são a obesidade central, diabetes mellitus, dislipidemia e síndrome metabólica. Sabe se que a doença pode ser também secundária a exposição medicamentosa e a toxinas ambientais. A condição tem alta morbimortalidade tanto pelas consequências relacionadas a hepatopatia, quanto pelo maior risco cardiovascular desses pacientes. CONCLUSÃO: A importância de estratégias de redução de peso para prevenção e controle da Esteatohepatite não alcoolica é bem estabelecida na literatura, uma vez que existe uma forte associação entre o IMC e o risco de desenvolvimento da doença. Existem estudos mostrando que a esteatohepatite pode também ocorrer em pacientes magros associada a outros fatores de risco.
