Цель исследования. Определить структуру и клинические особенности маточного и цервикального фактора бесплодия у женщин раннего и активного репродуктивного возраста.Материал и методы исследования. Проведено проспективное амбулаторное и стационарное исследование 222 пациенток в отделении гинекологической эндокринологии ГУ «НИИ АГ и П» в период с 01.03.2015 по 01.03.2018 гг.Заключение. Выявлена высокая частота сочетанных форм маточно-цервикального фактора бесплодия по отношению к изолированным формам (73,6 % против 27,3 %, р < 0,01) у инфертильных женщин в раннем и активном репродуктивном возрасте. В структуре маточного фактора определена высокая частота миомы матки у 34 % пациенток, полипов эндометрия -26,4 %, патологии эндометрия -22,6 % и врожденных аномалий матки -в 11,3 % случаев. В сочетанной форме маточно-цервикального фактора бесплодия диагностированы заболевания со следующей частотой встречаемости: хронический метроэндометрит -77,6 %, гипоплазия матки -46,7 %, эрозия шейки матки -50,5 %, врожденные аномалия матки и шейки матки -в 32,7 %, полип эндометрия в -31,8 % и патология эндометрияв 22,4 % случаев, что способствует увеличению частоты бесплодия.Ключевые слова: хронический эндометрит, врожденные аномалии матки и шейки матки, внутриматочные синехии, полип цервикального канала, миома матки.
Кафедра акушерства и гинекологии, институт последипломного образования в сфере здравоохранения республики Таджикистан, Душанбе, респуб лика Таджикистан 2 отделение эндокринологической гинекологии, Таджикский научноисследовательский институт акушерства, гинекологии и перинатологии, Душан бе, республика Таджикистан Цель: изучить структуру врождённых пороков репродуктивных органов у девушек-подростков и женщин раннего и активного репродуктивного возраста по данным их обращения в Таджикский НИИ акушерства, гинекологии и перинатологии. Материал и методы: обследованы 182 девушки-подростка и женщины репродуктивного возраста (от 14 до 26 лет) с изолированными и сочетанными врождёнными пороками репродуктивных органов и аномалиями мочевыделительной системы. Всем пациенткам проведены: сбор анамнеза (матерей и родственников), общеклинический (общий и гинекологический) осмотр, исследование органов брюшной полости, прямой кишки, зондирование влагалища, эндоскопическое (гистероскопия, диагностическая и лечебная лапароскопия) исследование, оценка состояния молочных желёз по шкале Tanner, гормональный скрининг, консультация генетика и уролога, определение полового хроматина, эхографические методы исследования матки и яичников, магнитно-резонансная томография органов малого таза. Результаты: у 29 (15,9%) пациенток отмечались различные разновидности аномалий развития влагалища с нарушением оттока менструальной крови; врождённая аномалия тела и шейки матки имела место у 86 (47,3%) пациенток (седловидная матка-в 43 (23,6%), двурогая матка-в 23 (12,6%), однорогая матка-в 8 (4,4%), полное удвоение матки и шейки матки-в 7 (3,8%) и рудиментарная функционирующая матка-в 5 (2,7%) случаях). Синдром Mayer-Rokitansky-Küster-Hauser (MRKH) был выявлен у 22 (12,1%) пациенток, при этом во всех случаях был диагностирован женский половой хроматин (46, XX), 5 (2,7%) больных страдали женским ложным гермафродитизмом и 4 (2,2%)-мужским ложным гермафродитизмом. У 36 (19,8%) пациенток пороки развития гениталий сочетались с аномалиями мочевыделительной системы. Заключение: выявлены различные формы врождённых пороков развития репродуктивных органов, клинически проявившиеся с началом менархе, либо-половой жизни. В структуре этих пороков аномалия развития матки и шейки матки преобладала над другими врождёнными пороками и составила 47,3% случаев.
