La région Centre-Val de Loire est particulièrement impactée par la pénurie de professionnels de santé. La démographie des chirurgiens-dentistes n’échappe pas à cette conjoncture au point que le départ à la retraite d’un praticien est devenu un véritable enjeu de santé publique. Pour cela, des passerelles ont été créées entre les facultés d’odontologie de Nantes, de Clermont-Ferrand et la faculté de médecine de Tours, pour accueillir des étudiants de troisième cycle court en région Centre-Val de Loire, faire le lien avec les praticiens libéraux et permettre aux étudiants de se confronter aux problématiques de santé de ce territoire.
Introduction : La mauvaise santé orale des patients schizophrènes est un problème de santé publique qui concernerait 600 000 personnes en France. Le but de cet article est de présenter les différentes étapes de la construction d’un programme d’éducation en santé orale, spécifique aux patients schizophrènes et tenant compte de leur savoir expérientiel, ainsi que les résultats de l’étude pilote ce programme. Patients et méthode : La méthode des groupes de discussion ( focus groups ) a été appliquée à un groupe de professionnels de santé et d’usagers afin de faire émerger un corpus exploratoire pour la construction d’un programme d’éducation en santé orale. Un groupe expert composé pour partie de patients schizophrènes a validé les thématiques et les outils de ce programme. Une étude pilote de faisabilité a ensuite été réalisée auprès d’un groupe témoin composé de sept patients schizophrènes. Résultats : Au total, 26 personnes ont participé à cette étude. Les thématiques principales retenues par le groupe expert visent à favoriser l’action consistant à prendre soin de sa santé, à améliorer la pénétrabilité du système de soins et à promouvoir une approche sanitaire globale. L’étude a montré la capacité des acteurs à coconstruire un programme et à faire évoluer les représentations que les patients schizophrènes ont de ce problème de santé. La plupart des outils éducatifs ont été jugés pertinents. Conclusion : Un programme d’éducation en santé orale a été construit dans le cadre d’un partenariat patients-soignants et a montré sa faisabilité. Une étude randomisée multicentrique est en cours pour évaluer l’efficacité de ce programme avec un haut niveau de preuve.
Introduction: Opinions differ regarding the combined use of local anaesthesia (LA) and general anaesthesia (GA) in oral surgery procedures. The aim of this study was to evaluate practices in France concerning intraoperative LA for oral surgery performed under GA. Practitioners and method: We conducted a prospective survey of 250 oral surgery practitioners (CNIL-2045135v0 e) and carried out a literature review with the MEDLINE search engine (PubMed) covering the period from January 2000 to September 2017. Results: Among the 77 practitioners who participated, 88.3% were dental practitioners, the majority of whom were in the 25–34-yr age group. More than half (59%) infiltrated the surgical site; 46% pre-operatively, 24% intraoperatively and 11% post-operatively. Discussion: LA under GA appears to have advantages for post-operative pain management, dissection of the first mucosal plane and bleeding management pre- and post-operatively. The contraindications remain the same as for patients in a vigilant state. In children, it should be used in moderation to limit the risk of self-inflicted lip or mouth trauma during recovery. Conclusion: The indications of LA under GA are operator-dependent and the analysis of the literature did not allow us to determine the interest or not of LA administered intraoperatively during oral surgery performed under GA.
Contexte : La démographie des chirurgiens-dentistes en région Centre-Val de Loire est très inférieure à la moyenne nationale et est marquée par de nombreuses inégalités territoriales de santé. Le but de cette étude est de préciser les habitudes de consommation de soins dentaires dans cette région sur la base d’un acte, le détartrage (HBJD001) qui permet une élimination mécanique du tartre afin de prévenir l’apparition de maladies parodontales. Méthode : Il s’agit d’une étude rétrospective réalisée à partir de données issues de plusieurs bases du Système national d’information inter régimes de l’assurance maladie de l’année 2016, effectuée par l’Institut statistique des professionnels de santé libéraux. Résultats : On observe un sous-recours aux détartrages par rapport à la France métropolitaine avec certains territoires qui cumulent des difficultés d’offre de soins et un recours aux détartrages inférieur à la moyenne régionale. Bien que l’offre de soins soit inégalement répartie, elle influe peu sur le volume d’actes consommés par patient, notamment sur le recours aux détartrages. À l’inverse, les indicateurs de richesse ou de pauvreté impactent la consommation de l’acte HBJD001. Conclusion : Cette étude suggère que la consommation de détartrages est indépendante de l’offre de soins, mais liée au gradient social de la population. Elle pointe également le déficit de la consommation de cet acte en regard de celle observée en France métropolitaine. Le rôle potentiel de l’acte HBJD001 comme marqueur d’un parcours de soins préventifs ainsi que la nécessité d’un ajustement du zonage déterminé par l’ARS sont mis en avant.
