PurposeThe aim of this study was to evaluate the impact of exposure to biomass smoke vs cigarette smoke on serum inflammatory markers and pulmonary function parameters in patients with chronic respiratory failure (CRF).Patients and methodsA total of 106 patients with CRF divided into age and gender-matched groups of cigarette-smoke exposure (n=55, mean [SD] age: 71.0 [12.0] years, 92.7% were females) and biomass smoke exposure (n=51, mean [SD] age: 73.0 [11.0] years, 94.1% were females) were included in this retrospective study. Data on patient demographics (age and gender), inflammatory markers, including neutrophil-to-lymphocyte ratio, C-reactive protein, platelet/mean platelet volume ratio, arterial blood gas analysis, and pulmonary function test findings, including forced expiratory volume in 1 second (FEV1), forced vital capacity (FVC), and FEV1/FVC were obtained from medical records.ResultsCarbon dioxide partial pressure levels were significantly higher in the biomass smoke exposure than in the cigarette smoke exposure group (mean [SD] 51.0 [8.0] vs 47.0 [8.0] mmHg, p=0.026, respectively). Spirometry revealed similarly low levels for FEV1 (%) (38.0 [16.0] vs 40.0 [12.0]%) and FVC (%) (45.0 [19.0] vs 39.0 [19.0]%) in cigarette-smoke and biomass smoke exposure groups, whereas biomass smoke exposure was associated with significantly higher FEV1/FVC (75.0 [14.0] vs 58.0 [12.0]%, p=0.001), lower FVC (mL) (mean [SD] 744.0 [410.0] vs 1,063.0 [592.0] mL, p=0.035) and lower percentage of patients with FEV1/FVC <70% (36.8% vs 82.0%, p<0.001) than cigarette smoke exposure.ConclusionOur findings indicate similarly increased inflammatory markers and abnormally low pulmonary function test findings in both biomass smoke exposure and cigarette smoke exposure groups, emphasizing the adverse effects of biomass smoke exposure on lungs to be as significant as cigarette smoke exposure. Association of biomass smoke exposure with higher likelihood of FEV1/FVC ratio of >70% and more prominent loss of vital capacity than cigarette smoke exposure seems to indicate the likelihood of at least 18 years of biomass exposure to be sufficiently high to be responsible for both obstructive and restrictive pulmonary diseases.
We aimed to establish that a bronchoscopic view can be as reliable as microbiology, and support an empirical tracheobronchial fungal infection (TBFI) treatment decision. We retrospectively studied 95 respiratory failure patients with suspected TBFI admitted to the intensive-care unit (ICU) in 2008 with sticky secretions, hyperaemic mucosa, and whitish plaques on bronchoscopic view. Patients not suspected of having TBFI were chosen as a control group (n = 151). Broncheoalveolar lavage (BAL) fluid was cultured, and biopsy samples were taken from the lesions. Biopsy samples positive for fungi were defined as 'proven', only BAL-positive (+ fungi) cases were 'probable TBFI', and BAL-negative (- fungi) cases were 'possible TBFI'. BAL (+ fungi) and BAL (- fungi) in the control group were defined as 'colonization' and 'no TBFI', respectively. The sensitivity, specificity and positive and negative predictive values of BAL (+ fungi) were 85.1% (63/74), 81.4% (140/172), 66.3% (63/95), and 92.7% (140/151), respectively. Biopsies were performed in 78 of 95 patients, and 28 were proven TBFI with fungal elements, and 100% were BAL (+ fungi). Probable TBFI was seen in 30 of 95 patients with BAL (+ fungi), and possible TBFI (BAL(- fungi)) in 25 of 95. Among the 95 patients, microbiology revealed fungi (90.5% Candida species; 9.5% Aspergillus) in 63 (66.3%). In the controls, the colonization and no TBFI rates were 11 of 151 and 140 of 151, respectively. Observing sticky secretions, hyperaemic mucosa and whitish plaques by bronchoscopy is faster than and may be as reliable as microbiology for diagnosing TBFI. These findings are relevant for empirical antifungal therapy in suspected TBFI patients in the ICU.
Amaç: Daha iyi bir yoğun bakım işleyişi sağlanabilmesi, mortalite ve morbiditenin azaltılabilmesi için yoğun bakım performans değerlendirilmesi-nin yapılıp aksayan noktaların saptanması önem taşır. Bu amaçla benzer büyüklükte, benzer hasta profili ve sevk kapasitesindeki yoğun bakım üni-telerinin karşılaştırılmaları bakım kalitesi açısından yol gösterici olacaktır. Literatürde yoğun bakımda kantitatif performans kalite ölçüm kriterlerini kendi ünitemizde saptamak, elde edilen veriler ile aksayan noktaları saptamak ve yoğun bakım kalitemizi yükseltmek adına çözümler üretmek ve gelecekte ünitemizin zaman içerisindeki gelişimini takip etmeyi amaçladık.Gereç ve Yöntemler: Kapalı özellikte, on yataklı, üç hasta başına bir hemşirenin görev yaptığı üçüncü basamak bir üniversite hastanesi genel yoğun bakım ünitesinde; 01 Ocak-31 Aralık 2016 tarihleri arasında takip edilen 347 hastanın demografik özellikleri ve klinik takip verileri retro-spektif olarak tarandı.Bulgular: 2016 yılı yatak kapasitesi kullanım oranı %95.81 olup bir yıl için-de takip edilen hasta sayısı 347 (211 erkek, 136 kadın), hastaların yaş ortalaması 65 (19-96 yaş),ortalama yatış süresi 9 gündü (1-182 gün). Yoğun bakıma kabul edilen hastaların %32.5'i acil servisten, %31'i medikal servislerden, %26.2'si postoperatif, %6.1'i cerrahi servislerden ve %4.2'si ise dış merkezden sevk edilen hastalardı. Bu hastaların % 45,2'si mortal seyretti, taburcu edilen hastaların %27.9'u cerrahi servilere, %27.8'i ise medikal servilere taburcu edildi. APACHE II ortalamaları 23(2-45) olup APACHE II' ye göre beklenen mortalite oranı %49, gerçekleşen kaba mortalite %45,2 (157/347) ve standardize mortalite oranı(SMR= gözlenen mortalite/ beklenen mortalite) ise 0,92 saptandı. Hematolojik tanısı olan hastalarda mortalite %58,1; postresüsite hastalarda ise %64,3 gibi yüksek sevilerdeydi. Bu süreçte 25 hastaya perkütan trakeotomi, 9 hastaya trakeostomi ve 13 hastaya PEG açıldı. İnvaziv santral venöz kateter (SVK) kullanım oranı %82, SVK ilişkili kan dolaşım enfeksiyon hızı %12.7 (32 hasta), invaziv mekanik ventilatör kullanım oranı %71ve ventilatör ilişkili pnömoni oranı %12.7 (32 hasta) saptandı. Taburculuk sonrası 48 saat içinde tekrar yoğun bakıma yatış oranı %2,6 ( 9 hasta), postoperatif 48 saat içinde mortalite oranı %2,2 (2/90), taburculuk sonrası 4 aylık takipte 22 hasta (%5.1) tekrar yatırılmış ve 51 hasta (%13.5) ise mortal seyretmiştir. Acil servisten alınan, respiratuar sistem hastalıkları bulunan ve erkek cinsiyette hastalarda uzun vadede tekrar yatış riski iki kat, 60-70 yaş grubunda ise dört kat daha fazla saptandı. Bu hastaların %56' sı tekrar taburcu edilebildi.Tartışma ve Sonuç: Ölçmediğimiz şeyleri değerlendirmemiz mümkün değildir, bununla birlikte mühim olan ölçmek değil ölçtüğümüzle ne yaptığı-mızdır.
