Objective: To investigate in the literature the relation of socioeconomic factors in the incidence of the disease and other outcomes related to leprosy. Method: Integrative review conducted in Lilacs, Medline, Scopus databases and SciELO online library with studies from 2000 to 2016. Results: 32 studies were included. Only studies that analyzed statistical associations of socioeconomic factors and outcomes related to leprosy were selected. Conclusion: Leprosy is greatly affected by the social context in which the patient is inserted, the chances of exposure to illness are the result of a set of not only individual aspects, but also of contexts or collective conditions. It is imperative for Nursing, as an essential part of the multiprofessional team entrusted with the care and surveillance of the disease, to recognize these factors to predict unfavorable outcomes and to develop new practices capable of reducing inequities.
RESUMO Objetivo: Investigar os fatores de risco associados ao adoecimento por hanseníase em contatos de casos da doença. Métodos: Realizou-se uma revisão sistemática e metanálise com busca nas bases de dados: Medical Literature Analysis and Retrieval System Online (MEDLINE), Embase, Cochrane Library, Cumulative Index to Nursing and Allied Health Literature (CINAHL), Literatura Latino-Americana e do Caribe em Ciências da Saúde (LILACS), Scopus e Web of Science até setembro de 2019. A seleção, a análise e a avaliação da qualidade dos estudos foram realizadas por quatro revisores. Utilizou-se modelo de efeitos aleatórios para calcular o risco relativo agrupado (RR) e intervalos de confiança de 95% (IC95%) quando na presença de heterogeneidade superior a 50%. Resultados: A busca resultou em 2.148 referências e foram incluídos 24 estudos. Estes, em sua maioria, foram realizados no Brasil e na Índia, com delineamento coorte, e incluíram contatos domiciliares, peridomiciliares e sociais. Mostraram-se associados ao adoecimento por hanseníase em contatos: o analfabetismo (RR = 1,48; IC95% 1,22 - 1,79), a convivência intradomiciliar (RR = 2,41; IC95% 1,87 - 3,10) com caso de hanseníase apresentando alta carga bacilar (RR = 2,40; IC95% 1,69 - 3,41), a soropositividade ao antígeno PGL-1(glicolipídeo fenólico-1) do Mycobacterium leprae (RR = 3,54; IC95% 2,21 - 5,67), presença do bacilo na corrente sanguínea (RR = 10,61; IC95% 4,74 - 23,77) e reação de Mitsuda negativa (RR = 2,68; IC95% 1,76 - 4,07). A imunização com bacilo Calmette-Guérin (BCG) teve efeito protetor contra o adoecimento (RR = 0,52; IC95% 0,34 - 0,78). Conclusão: O adoecimento por hanseníase em contatos perpassa pela determinação social, pela susceptibilidade individual e por fragilidades no acesso às ações de controle da doença; contudo, fatores de risco modificáveis são os principais determinantes do adoecimento nessa população.
Introdução: a hanseníase é uma doença infecciosa cujo diagnóstico, quando realizado tardiamente, resulta em dano neural e incapacidades físicas que restringem a participação social do indivíduo e contribuem para o estigma e discriminação. Além disso, fatores de risco ambientais, socioeconômicos e individuais podem contribuir para a vulnerabilidade ao adoecimento e evolução para incapacidades físicas. Objetivo: analisar a influência da escolaridade na ocorrência de incapacidades físicas no diagnóstico da hanseníase no Vale do Jequitinhonha, Minas Gerais. Material e Métodos: trata-se de um estudo transversal realizado na população de casos de hanseníase diagnosticados no Vale do Jequitinhonha, no período de 2001 a 2017, a partir de dados obtidos no Sistema de Informação de Agravos de Notificação (SINAN). A variável dependente foi o grau de incapacidade física no diagnóstico da hanseníase e a variável explicativa o nível de escolaridade dos casos. A análise estatística compreendeu a realização dos testes qui-quadrado, coeficiente de correlação de Spearman e odds ratio. Resultados: foram notificados 1.940 casos de hanseníase no Vale do Jequitinhonha com uma taxa média de detecção de 47 casos/100.000 habitantes. Houve uma correlação inversamente proporcional entre as variáveis escolaridade e grau de incapacidade com significância estatística (-0,17; p<0,0001). Indivíduos sem escolaridade tiveram 82% mais chance de apresentarem incapacidades no diagnóstico de hanseníase quando comparados àqueles com nível fundamental (OR = 1,82; p = <0,0001), a chance de incapacidades foi ainda maior na comparação com a escolaridade de nível médio (OR = 4,03; p<0,0001). Conclusão: o aumento no nível de escolaridade dos casos reduziu a chance de ser diagnosticado com alguma incapacidade instalada.
RESUMOObjetivo: comparar a presença e extensão dos atributos da atenção primária no geral e no desempenho da hanseníase. Métodos: estudo transversal e avaliativo, realizado em um município de Minas Gerais, com 28 profissionais da Estratégia Saúde da Família, sendo composto por médicos e enfermeiros. Para a coleta de dados foram aplicados o Primary Care Assessment Tool e o Instrumento de Avaliação de Desempenho da Atenção Primária nas Ações de Controle da Hanseníase. Utilizou-se os testes estatísticos Wilcoxon e t de Student com significância p<0,05. Resultados: A atenção primária, no global, apresentou alta orientação no escore geral e derivado. Nas ações de hanseníase, apenas o escore essencial alcançou alta orientação. Encontrou-se diferença significativa no escore essencial, atributos acesso e integralidade dos serviços disponíveis, e no escore derivado, atributo orientação comunitária. Conclusão: o desempenho da atenção primária no geral é melhor, o que demonstra fragilidades na integração das ações de controle da hanseníase e necessidade de fortalecimento das estratégias de enfrentamento da endemia no que concerne à Estratégia de Saúde da Família. ABSTRACTObjective: to compare the presence and extent of primary care attributes in general and in leprosy performance. Methods: crosssectional and evaluative study, carried out in a city of Minas Gerais, with 28 professionals from the Family Health Strategy, consisting of doctors and nurses. The Primary Care Assessment Tool and the Primary Care Performance Assessment Instrument for Leprosy Control Actions were applied for data collection. Wilcoxon statistical test and Student's t test were used with p <0.05. Results: Primary care, overall, showed high orientation in the general and derived score. In leprosy actions, only the essential score reached high orientation. There was significant difference in the essential score, access and integrality attributes of the available services, and in the derived score, attribute community orientation. Conclusion: the primary care general performance is better, which demonstrates weaknesses in the integration of leprosy control actions and the need to strengthen the endemic coping strategies in relation to the Family Health Strategy.
