RESUMO:Este estudo objetivou caracterizar os casos novos de hanseníase; analisar os aspectos socioeconômicos, demográficos, clínico-epidemiológicos e de acesso ao diagnóstico e tratamento relacionados à ocorrência de hanseníase com incapacidades físicas; e a evolução das incapacidades durante o tratamento. There was a statistical relationship between the development of physical deformities and those affected by leprosy with lower levels of schooling, higher neural involvement, and greater difficulties in accessing the health unit, in addition to the maintenance of the degree of disability. There is a reduced operational capacity of the Family Health Strategy (FHS) regarding leprosy control actions and the need to implement the policy according to the Unified Health System (SUS, from its acronym in Portuguese).
A hanseníase ainda é considerada um problema de saúde pública no Brasil. O objetivo deste estudo é analisar a situação epidemiológica da hanseníase na microrregião de Almenara/MG segundo sua distribuição espacial e suas relações com as condições socioeconômicas da população. Trata-se de um estudo ecológico. Os resultados colocam a microrregião como uma área hiperendêmica e indicam que os serviços de saúde não estão sendo capazes de detectar todos os casos existentes, contribuindo para manter a cadeia de transmissão da doença. A análise espacial identificou dois clusters, sendo um com coeficientes de detecção acima e outro abaixo do esperado. Acredita-se que os resultados obtidos servirão para balizar a elaboração e implementação de medidas de prevenção e controle da hanseníase na região.
Hansen's disease has a slow evolution, which mostly affects the adult population. Detecting cases in children shows that the endemic continues and there is early exposure of the population to the bacillus. This is an ecological study with the objective of analyzing the occurrence of Hansen's disease in children under fifteen in the Jequitinhonha Valley through epidemiological and operational indicators. The results show that the endemic continues in the region, demonstrating that the health services are not proactive and must implement specific prevention and control measures for this age group in order to reduce the transmission of the disease and the possible damage it can bring to the life of individuals, especially when it affects them in their childhood.
A hanseníase representa um problema de saúde pública no Brasil pelos altos índices de prevalência e incidência. O objetivo deste estudo é analisar a situação epidemiológica da hanseníase e sua relação com o desenvolvimento das ações de controle na microrregião de Araçuaí. Trata-se de estudo epidemiológico, descritivo, cujos dados foram coletados das fichas de notificação de casos de hanseníase de municípios da microrregião, período 1998-2007, disponibilizados no Sistema de Informação de Agravos de Notificação - SINAN. Foram construídos e analisados indicadores epidemiológicos e operacionais preconizados pelo Ministério da Saúde. Foram notificados 343 casos, resultando em uma detecção geral média de 28,5 casos/100.000 habitantes/ano. A proporção de casos detectados com grau II de incapacidade e o predomínio de formas passivas de detecção sugerem diagnóstico tardio e corroboram a importância da integração das ações de controle da hanseníase na atenção básica.
O estudo teve como objetivo analisar a organização tecnológica do processo de trabalho em hanseníase, empreendido pelos profissionais de saúde que atuam nos serviços da microrregião de Almenara, Minas Gerais. Pesquisa qualitativa, realizada em nove municípios. Como técnicas para a coleta de dados foram utilizadas a entrevista semiestruturada e a pesquisa documental. Foram realizadas 45 entrevistas com gestores e profissionais de saúde. A coleta ocorreu entre novembro 2007 e fevereiro 2008. Para tratamento e análise dos dados foi utilizado a Análise de Conteúdo. Verificamos que os médicos ainda mantêm uma assistência curativa e individual; os enfermeiros dominam todo o processo de trabalho em hanseníase; e os agentes comunitários de saúde realizam as ações educativas, a busca de suspeitos dermatológicos, de contatos e de faltosos. Concluímos que a atual organização dos serviços mostra que ainda existe uma fragilidade nas práticas de saúde voltadas para a abordagem coletiva do problema
Conflicting findings about the association between leprosy and TLR1 variants N248S and I602S have been reported. Here, we performed case-control and family based studies, followed by replication in 2 case-control populations from Brazil, involving 3162 individuals. Results indicated an association between TLR1 248S and leprosy in the case-control study (SS genotype odds ratio [OR], 1.81; P = .004) and the family based study (z = 2.02; P = .05). This association was consistently replicated in other populations (combined OR, 1.51; P < .001), corroborating the finding that 248S is a susceptibility factor for leprosy. Additionally, we demonstrated that peripheral blood mononuclear cells (PBMCs) carrying 248S produce a lower tumor necrosis factor/interleukin-10 ratio when stimulated with Mycobacterium leprae but not with lipopolysaccharide or PAM3cysK4. The same effect was observed after infection of PBMCs with the Moreau strain of bacillus Calmette-Guerin but not after infection with other strains. Finally, molecular dynamics simulations indicated that the Toll-like receptor 1 structure containing 248S amino acid is different from the structure containing 248N. Our results suggest that TLR1 248S is associated with an increased risk for leprosy, consistent with its hypoimmune regulatory function.
Leprosy is a complex disease with phenotypes strongly influenced by genetic variation. A Chinese genome-wide association study (GWAS) depicted novel genes and pathways associated with leprosy susceptibility, only partially replicated by independent studies in different ethnicities. Here, we describe the results of a validation and replication study of the Chinese GWAS in Brazilians, using a stepwise strategy that involved two family-based and three independent case-control samples, resulting in 3,614 individuals enrolled. First, we genotyped a family-based sample for 36 tag single-nucleotide polymorphisms (SNPs) of five genes located in four different candidate loci: CCDC122-LACC1, NOD2, TNFSF15 and RIPK2. Association between leprosy and tag SNPs at NOD2 (rs8057431) and CCDC122-LACC1 (rs4942254) was then replicated in three additional, independent samples (combined OR(AA) = 0.49, P = 1.39e-06; OR(CC) = 0.72, P = 0.003, respectively). These results clearly implicate the NOD2 pathway in the regulation of leprosy susceptibility across diverse populations.
