La tromboembolia pulmonar (TEP) consiste en la obstrucción del flujo sanguíneo arterial pulmonar por embolismo de un coágulo desde sitios distantes de la vasculatura, principalmente en miembros inferiores. La verdadera incidencia, prevalencia y tasa de mortalidad se desconocen. El diagnóstico se basa en una serie de signos y síntomas, inespecíficos y comunes a otras patologías cardiorrespiratorias, hasta el choque cardiogénico con muerte súbita debido a falla ventricular derecha aguda, su diagnóstico es tanto clínico, como por imágenes, también se emplea métodos de laboratorio como el dímero D el cual es útil , ya que da un gran valor predictivo negativo en el diagnóstico de TEP. La angiotomografía de tórax es actualmente el estándar de oro para su diagnóstico ya que presenta una alta sensibilidad y especificidad.
Las recomendaciones sobre ayuno preoperatorio actuales son la suma de esfuerzos de diferentes sociedades de anestesiología y nutrición durante más de 20 años, esto con la finalidad de conocer a fondo la fisiología del vaciamiento gástrico y así poder pautar recomendaciones de tiempo previo sobre que se pueden ingerir los distintos tipos de alimentos como líquidos claros, leche materna o no materna, comida ligera y comidas completas; también demostrar las ventajas de ponerlas en práctica, al igual si es recomendado o no el uso de distintos fármacos en el preoperatorio para ayudar o acelerar el vaciamiento gástrico en pacientes con cirugías programadas.
El síndrome de embolia grasa (SEG) es una complicación grave que se presenta por la formación de una embolia grasa (es la obstrucción de flujo de un vaso sanguíneo por material lipídico liberado a partir de fracturas) en pacientes con fracturas de huesos largos de extremidades inferiores, principalmente. Su incidencia real se desconoce, pero por su asociación con los politraumatismos, ocurre con mayor frecuencia en la segunda y tercera década de la vida, siendo más frecuente en hombres que mujeres. Dentro de síntomas más comunes se encuentran la insuficiencia respiratoria, alteraciones neurológicas y petequias. Su diagnóstico es fundamentalmente clínico y su tratamiento se debe centrar en brindar ventilatorio, y mantener volemia adecuada para no empeorar la lesión pulmonar.
