La parálisis de Bell o parálisis facial periférica es una enfermedad neuromuscular con afectación directa en el nervio facial. Es una patología común con un pico de presentación de los 20 a 29 años y los 50 a 59 años. Es de carácter idiopático, siendo la etiología provocada por el virus herpes simple la mayor sospecha causal. Se muestra como una parálisis facial unilateral súbita con empeoramiento posterior. Presenta sintomatología de afectación nerviosa muscular y sensitiva como dificultad para elevar la ceja, cerrar los ojos, sonreír, fruncir el ceño, parestesias faciales, hiperacusia, entre otros. Su diagnóstico principalmente es clínico, pero también se utilizan otros métodos diagnósticos complementarios para descartar diversas patologías. Es necesario un manejo pronto y oportuno para lograr la remisión completa y evitar complicaciones, este incluye corticoesteroides orales, valorar la utilización de antivirales y una adecuada protección ocular. La mayoría de los pacientes manifiestan remisión espontánea con una recuperación completa, los que muestran una recuperación incompleta pueden verse beneficiados de terapias físicas para mejorar la función facial y la prevención de secuelas como hipertonicidad, asimetría facial y sincinesias.
La tromboembolia pulmonar (TEP) consiste en la obstrucción del flujo sanguíneo arterial pulmonar por embolismo de un coágulo desde sitios distantes de la vasculatura, principalmente en miembros inferiores. La verdadera incidencia, prevalencia y tasa de mortalidad se desconocen. El diagnóstico se basa en una serie de signos y síntomas, inespecíficos y comunes a otras patologías cardiorrespiratorias, hasta el choque cardiogénico con muerte súbita debido a falla ventricular derecha aguda, su diagnóstico es tanto clínico, como por imágenes, también se emplea métodos de laboratorio como el dímero D el cual es útil , ya que da un gran valor predictivo negativo en el diagnóstico de TEP. La angiotomografía de tórax es actualmente el estándar de oro para su diagnóstico ya que presenta una alta sensibilidad y especificidad.
El Síndrome Urémico Hemolítico constituye una microangiopatía trombótica, es una de las principales causas de lesión renal aguda en niños, además tiene un alto riesgo de morbimortalidad. Su etiología es variable, con factores infecciosos y/o genéticos e incluso puede llegar a ser secundario a una patología. Dentro de los hallazgos clínicos se presenta lesión renal aguda, trombocitopenia y anemia hemolítica microangiopática, más de la mitad de los pacientes presentan diarrea acuosa con disentería, acompañada de otros síntomas gastrointestinales como dolor abdominal, náuseas y vómitos. Otras manifestaciones que se presentan en la minoría de pacientes son a nivel cardiaco, neurológico, oftalmológico y dermatológico. Para el diagnóstico se requiere primero identificar clínicamente el Síndrome Urémico Hemolítico y posterior a esto es necesario indagar sobre la causa del mismo. Los exámenes de laboratorio que se requieren para el diagnóstico incluyen hemograma, pruebas de función renal, pruebas de función hepática, examen general de orina y frotis de heces, además de exámenes complementarios para identificar su causa. El tratamiento se debe instaurar lo antes posible con el fin de evitar o reducir las complicaciones agudas y las secuelas a largo plazo, siendo el tratamiento de soporte el manejo esencial.
