RESUMO:Fitoterápico é uma classe de medicamento largamente utilizada no país e constitui um mercado em potencial expansão. Este artigo busca traçar um panorama sobre os medicamentos fi toterápicos registrados no país e as espécies vegetais que lhe deram origem, enfocando o papel regulador da Anvisa. Realizou-se uma pesquisa nas seguintes bases de dados: Visalegis e site da Anvisa para verifi car os medicamentos fi toterápicos registrados na Anvisa, obtendo-se os seguintes dados: há 432 fi toterápicos simples registrados e 80 compostos, as espécies vegetais com maior número de derivados registrados são o Ginkgo (Ginkgo biloba) e a Castanha da Índia (Aesculus hippocastanum), com 33 e 29 registros, respectivamente. O artigo traz ainda uma tabela com as espécies vegetais que possuem derivados registrados como fi toterápicos simples, sua classifi cação terapêutica, lista as espécies registradas em associação, as principais formas farmacêuticas registradas e os percentuais de registro e empresas por região do país, fornecendo assim um panorama da atuação da Anvisano setor de fi toterápicos.Unitermos: Medicamento, fi toterápico, regulação, registro, Anvisa.ABSTRACT: "Situation of herbal medicines register in Brazil". Herbal medicines are widely used in Brazil and constitute a market in potential expansion. This article explains the situation on herbal medicines registration in Brazil plant species, focusing on the regulatory role of Anvisa. INTRODUÇÃOFitoterápico, de acordo com a legislação sanitária brasileira, é o medicamento obtido empregandose exclusivamente matérias-primas ativas vegetais. É caracterizado pelo conhecimento da efi cácia e dos riscos de seu uso, assim como pela reprodutibilidade e constância de sua qualidade (Brasil, 2004a).Os fi toterápicos sempre apresentaram uma parcela signifi cativa no mercado de medicamentos. RESUMO:Neste artigo são apresentados os resultados da avaliação das notificações de eventos adversos a plantas medicinais e seus derivados (fitoterápicos), efetuadas voluntariamente ao Sistema Nacional de Farmacovigilância, coordenado pela Agência Nacional de Vigilância Sanitária, no período de janeiro de 1999 a março de 2009. A farmacovigilância envolve a detecção de reações adversas que ocorrem durante ou após o uso de um medicamento, a interação medicamentosa, o desvio de qualidade, o uso abusivo e a inefetividade. Como as pesquisas realizadas para a avaliação do uso eficaz e seguro de fitoterápicos são incipientes, as notificações de eventos auxiliam na geração de novas informações, promovendo seu uso racional.Unitermos: Fitoterápicos, plantas medicinais, farmacovigilância, evento adverso, Anvisa.ABSTRACT: "Pharmacovigilance: a step towards the rational use of herbs and herbal medicines". This article presents the evaluation of the notifications of adverse events related to medicinal plants and their derivatives (herbal medicines). Such evaluations were voluntarily run by the Brazilian Pharmacovigilance System, coordinated by the Brazilian Health Surveillance Agency (Anvisa), from January 1999 to March 2009. Pharmacovigilance involves the detection of adverse reactions that occur during or after the use of drugs, drug interactions, problems of quality, improper use and inefficacy. There is scarce scientific confirmation about efficacy and safety of herbal medicines, so the notification of adverse events provides of information, and promotes rational use.
BackgroundEnd-stage renal disease (ESRD) is the last stage of chronic kidney disease, mainly caused by type 2 diabetes mellitus and characterized by an increased mortality risk related to cardiovascular disease. Low-dose aspirin (acetylsalicylic acid or ASA) seems to effectively prevent cardiovascular events in patients with ESRD. However, the number of interventional studies in this population remains limited and the mechanisms of aspirin-related bleeding remain poorly understood. Aspirin’s efficacy and safety may be modified by the presence of type 2 diabetes mellitus or platelet hyperreactivity.ObjectiveThe overall objective of this protocol is to (1) evaluate aspirin’s safety and efficacy in reducing the risk of thrombotic events in patients with ESRD on hemodialysis and (2) examine whether aspirin’s efficacy is modified by the presence of type 2 diabetes mellitus or platelet hyperreactivity. Specifically, the primary objective is to compare the 12-month rate of any thrombotic event (cardiac death, nonfatal myocardial infarction, nonfatal stroke, arteriovenous fistula thrombosis) and Thrombolysis in Myocardial Infarction (TIMI) major bleeding in patients treated with aspirin compared to those on placebo. Secondary objectives are to test for effect modification of treatment by the presence of type 2 diabetes mellitus or platelet hyperreactivity and compare the rate of TIMI minor bleeding between treatment groups.MethodsWe developed a protocol for a phase 2 randomized, single-center, placebo-controlled, triple-blind, superiority clinical trial to assess the prophylactic efficacy and safety of aspirin in patients with ESRD and on hemodialysis. It follows the ethical principles of the Declaration of Helsinki of the World Medical Association. A total of 342 participants would be enrolled over 12 months at a large dialysis center. Patients will be randomized in a 1:1 ratio and stratified by presence of type 2 diabetes mellitus and platelet hyperreactivity to receive either oral aspirin (100 mg/d) or placebo for a treatment period of 12 months. An intention-to-treat statistical analysis will be performed.ResultsThe randomized clinical trial will be performed after approval by the ethical committee of the participating center and registration at ClinicalTrials.gov.ConclusionsWe provide a protocol for a randomized controlled trial to evaluate the safety and efficacy of treatment with aspirin to reduce the risk of thrombotic events. In addition, such a study would further our understanding of the mechanism of aspirin-related bleeding and help identify subgroups of best-responders and patients with a higher risk of adverse events.Registered Report IdentifierRR1-10.2196/10516
Fitoterápico é o medicamento obtido empregando-se exclusivamente derivados de droga vegetal como constituintes ativos, caracterizado pelo conhecimento da eficácia e dos riscos de seu uso, assim como pela constância de sua qualidade. A eficácia e segurança podem ser validadas através de levantamentos etnofarmacológicos de utilização, publicações tecnocientíficas ou estudos farmacológicos e toxicológicos pré-clínicos e clínicos. A segurança também é determinada pelos ensaios que comprovam a identidade da planta e a ausência de contaminantes. A qualidade deve ser alcançada mediante o controle das matérias-primas, do produto acabado, materiais de embalagem, formulação farmacêutica e estudos de estabilidade. Este artigo descreve alguns tópicos do registro de fitoterápicos junto à Agência Nacional de Vigilância Sanitária (ANVISA), órgão brasileiro que regula e registra medicamentos.
Results: we developed a protocol for a phase II randomized, single-center, placebocontrolled, triple-blind, superiority clinical trial in order to assess aspirin prophylactic efficacy and safety in ESRD patients on hemodialysis. It follows the ethical principles of the Declaration of Helsinki of the World Medical Association. Participants will be randomized (1:1 ratio) in 2 arms and orally receive acetylsalicylic acid 100mg/day or placebo, for 12 months. An intent-to-treat (ITT) statistical analysis will be performed.The primary composite outcome (12-month incidence of thrombotic events [cardiac death, nonfatal myocardial infarction, nonfatal stroke, arteriovenous fistula thrombosis] and TIMI major bleeding) will be analyzed with Kaplan Meier curves and a log-rank test to compare treatment arms. Cox proportional hazards regression will be used to adjust for covariates, if appropriate. As secondary outcomes, the same components of the primary outcome will be assessed in a subgroup analysis after patients' stratification for the presence versus absence of type-2 Diabetes Mellitus and platelet hyper-reactivity. The presence of TIMI minor bleeding will be compared between groups and tested with a Chi-square test. Conclusions:We provide a protocol for a randomized controlled trial to evaluate aspirin's prophylactic efficacy for thrombotic events and safety in ESRD patients.When conducted, such a study would further our understanding of the mechanism of aspirin-related bleeding, and would help identify best-responders and patients with a higher risk of adverse events.
As sementes de Aesculus hippocastanum L. (Hippocastanaceae), conhecida como castanha-da-Índia, são utilizadas em várias partes do mundo no tratamento da insuficiência venosa crônica. O objetivo do presente estudo foi avaliar a qualidade de amostras comerciais de medicamentos fitoterápicos contendo extrato de sementes de A. hippocastanum. O total de 16 amostras comercializadas no Brasil com registro válido em março de 2013 foram avaliadas quanto à correspondência com as especificações do dossiê de registro aprovado pela ANVISA. As amostras foram analisadas em Laboratório Oficial, quanto ao perfil cromatográfico, teor de escina, características macroscópicas e microscópicas e peso médio. A bula e a rotulagem foram avaliadas por especialista da ANVISA.Os resultados apontaram que 13 dos 16 medicamentos avaliados apresentavam ao menos uma não-conformidade. Portanto, é imprescindível a implementação de ações de vigilância sanitária efetivas, com foco no monitoramento e avaliação de medicamentos fitoterápicos ao longo de sua comercialização.
