Introdução: dentre os cânceres dermatológicos, o melanoma é o tipo mais agressivo, possuindo um alto potencial de induzir metástases e se constituindo na principal causa de morte por neoplasias de pele. O tratamento para o melanoma metastático consiste em terapia sistêmica, radioterapia e cirurgia. Porém, pelo fato de não ter sido constatado um aumento da sobrevida dos pacientes, novas terapêuticas têm sido aprovadas para combater essa condição oncológica, dentre as quais, a imunoterapia. Nesse sentido, o bacilo intralesional (IL) Calmette-Guérin (BCG), cepa viva atenuada de Mycobacterium bovis, demonstrou atividade antitumoral e está sendo utilizado para tratamento de melanoma metastático. Objetivos: revisar a literatura atual sobre a relevância da imunoterapia BCG no tratamento de pacientes com melanoma e avaliar os avanços mais recentes na área. Metodologia: este trabalho se trata de uma revisão integrativa da literatura realizada na base de dados Pubmed por meio dos descritores em português e inglês: “BCG”, “imunoterapia”, “tratamento” e “melanoma”. Os termos foram organizados em uma chave de busca relacionados com seus Medical Subject Headings (MeSH). Foram incluídos artigos completos, nos idiomas inglês e português, publicados entre 2017-2022. Aqueles trabalhos que não contemplavam os objetivos aqui pretendidos, que não estavam disponíveis na íntegra de forma gratuita e que estavam repetidos foram excluídos. Assim, a amostra inicial resultou em 15 artigos, dos quais 7 foram excluídos por não serem gratuitos e 4 por não abordarem o tema melanoma. Por fim, o corpus analítico foi constituído de 4 artigos científicos. Revisão de Literatura: o BCG é um agente que estimula a imunidade mediada por células, produzindo uma infecção localizada, autolimitada e uma potente resposta inflamatória celular intratumoral que reduz o tamanho do tumor. Os componentes imunogênicos desse agente conseguem induzir tais respostas devido à ativação de receptores de reconhecimento de padrões (PRRs) em células imunes inatas, mas que, provavelmente, também envolvem a imunidade adquirida, favorecendo uma resposta das células T, alterando o microambiente do melanoma. A imunização ativa adjuvante com o BCG pode favorecer o aumento de linfócitos reativos ao tumor, elevando a probabilidade de indução de destruição das células cancerígenas. Já a terapia intralesional (IL) pode ser eficaz na indução da regressão do melanoma metastático cutâneo. Além disso, novas abordagens da droga têm sido investigadas, como hidrogéis, que se mostraram menos tóxicas e mais eficazes que a terapia IL. Os possíveis efeitos colaterais do tratamento são: gripe, febre, mialgia, calafrios, abscessos nos locais de injeção e indução de artrite. Por fim, os resultados com a imunoterapia BCG demonstraram regressão tumoral local e regional, supressão de metástases viscerais (principalmente o hidrogel) e aumento significativo da sobrevida dos pacientes. Conclusão: a exemplo dos recentes avanços das tecnologias de cuidado empregadas no setor Saúde, a literatura consultada evidencia que a imunoterapia BCG vem impactando positivamente o tratamento dos melanomas, especialmente o metastático. Sendo assim, é importante que novos estudos sobre essa alternativa terapêutica sejam realizados para melhor elucidação dos seus mecanismos de ação e consequente desenvolvimento de tratamentos, ainda mais eficazes, direcionados ao manejo do melanoma e sua agressividade neoplásica.
