BackgroundNitazoxanide exerts antiviral activity in vitro and in vivo and anti-inflammatory effects, but its impact on patients hospitalized with COVID-19 pneumonia is uncertain.MethodsA multicentre, randomized, double-blind, placebo-controlled trial was conducted in 19 hospitals in Brazil. Hospitalized adult patients requiring supplemental oxygen, with COVID-19 symptoms and a chest computed tomography scan suggestive of viral pneumonia or positive RT-PCR test for COVID-19 were enrolled. Patients were randomized 1:1 to receive nitazoxanide (500 mg) or placebo, 3 times daily, for 5 days, and were followed for 14 days. The primary outcome was intensive care unit admission due to the need for invasive mechanical ventilation. Secondary outcomes included clinical improvement, hospital discharge, oxygen requirements, death, and adverse events within 14 days.ResultsOf the 498 patients, 405 (202 in the nitazoxanide group and 203 in the placebo group) were included in the analyses. Admission to the intensive care unit did not differ between the groups (hazard ratio [95% confidence interval], 0.68 [0.38–1.20], p = 0.179); death rates also did not differ. Nitazoxanide improved the clinical outcome (2.75 [2.21–3.43], p < 0.0001), time to hospital discharge (1.37 [1.11–1.71], p = 0.005), and reduced oxygen requirements (0.77 [0.64–0.94], p = 0.011). C-reactive protein, D-dimer, and ferritin levels were lower in the nitazoxanide group than the placebo group on day 7. No serious adverse events were observed.ConclusionsNitazoxanide, compared with placebo, did not prevent admission to the intensive care unit for patients hospitalized with COVID-19 pneumonia.Clinical Trial RegistrationBrazilian Registry of Clinical Trials (REBEC) RBR88bs9x; ClinicalTrials.gov, NCT04561219.
O exercício da medicina, desde seus primórdios, deve contar com uma boa relação médico-paciente (RMP), com fundamentos éticos centrados em princípios como atenção e compaixão, além da solidariedade humana, tendo em sua missão tarefas como confortar, escutar, olhar e tocar seu paciente1. Um dos componentes da RMP é a empatia, que apesar da dificuldade de sua definição conceitual, para o paciente ela é capaz de oferecer segurança e torná-lo mais colaborativo durante o processo de tratamento, aspecto fundamental para que se atinja a efetividade do processo terapêutico2. O objetivo do presente estudo foi avaliar a humanização na relação médico-paciente em alunos do primeiro período de curso de medicina (APPCM). Projeto de pesquisa e extensão da Universidade de Vassouras, sob parecer do Conselho de Ética em Pesquisa (CEP) n° 1.963.944, com início em 2017. Período de avaliação foi entre 2017 e 2019. A aproximação do APPM ao paciente foi realizada pelo auxílio do aluno mentor matriculado a partir do segundo período de medicina. Um total de 2386 pacientes foram entrevistados através de questionários. A aproximação proporcionada pelo APPM aos pacientes, foi interpretada como positiva tanto pelos alunos como pelos pacientes. Foi possível observar que os estudantes de medicina foram capazes de participar das necessidades dos pacientes, auxiliando, cuidando, fornecendo informações durante a internação, trazendo mais conforto e tranquilidade aos pacientes durante a internação, procurando suprir problemas relatados pelos pacientes em várias áreas relatadas, trabalhando no estímulo da habilidade de comunicação. A introdução dos alunos de medicina precocemente ao paciente incentivou o fortalecimento do processo de humanização na relação médico-paciente.
Objective: To analyze bronchoalveolar lavage (BAL) specimens of burn victims who inhaled smoke, in order to identify alterations associated with mortality or survival. Methods: Eighteen victims of facial burns were submitted to BAL up to 24 h after the event. We investigated cell and protein content, including TNF-α, HLA-DR, CD14, CD68 and iNOS. Results: Of the 18 patients submitted to bronchoscopy, 8 (44.4%) died during the follow-up period. The mean age of patients who died was significantly higher (44.7 vs. 31.5 years). On average, the patients who died had burns covering 60.1% of the total body surface area, compared with 26.1% in the survivors (p < 0.0001). Of the 18 patients submitted to bronchoscopy, 11 (61.1%) showed endoscopic signs of smoke inhalation injury, and 4 (36.4%) of those 11 died. Of the 7 patients with no signs of smoke inhalation injury, 4 (57.1%) died. The mean number of ciliated epithelial cells in the BAL fluid was significantly higher in the patients who died than in the survivors (6.6% vs. 1.4%; p = 0.03). There were no significant differences between the groups in terms of any of the other parameters evaluated. Conclusions: The total body surface area burned was a predictive factor for mortality. Increased numbers of ciliated epithelial cells in the BAL fluid, denoting bronchial epithelial desquamation, were associated with higher mortality in patients with facial burns. (44,7 vs. 31,5 anos). Entre os 18 pacientes submetidos à broncoscopia, 11 (61,1%) apresentaram sinais endoscópicos de lesão por inalação de fumaça, e 4 (36,4%) destes faleceram. Dos 7 pacientes sem sinais de lesão por inalação de fumaça, 4 (57,1%) faleceram. A média do número de células epiteliais ciliadas no LBA dos pacientes que morreram foi significativamente maior daquela dos sobreviventes (6,6% vs. 1,4%; p = 0,03). Os demais parâmetros analisados não mostraram diferença entre os grupos. Conclusões: A superfície corporal queimada mostrou ser um fator preditivo de mortalidade. A Prática Da Humanização Da Relação Médico-Paciente Nos Alunos De Primeiro Período De Medicina Da Universidade Severino Sombra. Projeto Calouro Humano é um projeto tanto da pró-Retoria de Pesquisa e Pós-Graduação, como da Pró-Reitoria de Extensão Universitária da Universidade de Vassouras voltado para os alunos do primeiro período do curso de medicina de cada semestre letivo – o calouro humano. Possui duração de 14 semanas envolvendo também, monitores, que são alunos de diversos períodos, exceto o primeiro, além dos professores da disciplina de Programa de Aproximação a Prática Médica 1. Ele consiste em trabalhar com o aprendizado e a utilização dos princípios da humanização da relação médico-paciente e inserir o aluno desde o princípio da faculdade no ambiente hospitalar visando o paciente como pessoa portadora de uma doença. Quando perguntados a respeito da experiência ao executarem as suas funções, cerca de 80% dos alunos responderam positivamente em relação a vivência nessa atividade. Trata-se de um projeto de importância para a construção gradual do desenvolvimento da habilidade de empatia e comunicação dos alunos envolvidos em ambas as funções.
