Background Respiratory medicine (RM) and palliative care (PC) physicians’ management of chronic breathlessness in advanced chronic obstructive pulmonary disease (COPD), fibrotic interstitial lung disease (fILD) and lung cancer (LC), and the influence of practice guidelines was explored via an online survey. Methods A voluntary, online survey was distributed to RM and PC physicians via society newsletter mailing lists. Results 450 evaluable questionnaires (348 (77%) RM and 102 (23%) PC) were analysed. Significantly more PC physicians indicated routine use (often/always) of opioids across conditions (COPD: 92% vs. 39%, fILD: 83% vs. 36%, LC: 95% vs. 76%; all p < 0.001) and significantly more PC physicians indicated routine use of benzodiazepines for COPD (33% vs. 10%) and fILD (25% vs. 12%) (both p < 0.001). Significantly more RM physicians reported routine use of a breathlessness score (62% vs. 13%, p < 0.001) and prioritised exercise training/rehabilitation for COPD (49% vs. 7%) and fILD (30% vs. 18%) (both p < 0.001). Overall, 40% of all respondents reported reading non-cancer palliative care guidelines (either carefully or looked at them briefly). Respondents who reported reading these guidelines were more likely to: routinely use a breathlessness score (χ2 = 13.8; p < 0.001), use opioids (χ2 = 12.58, p < 0.001) and refer to pulmonary rehabilitation (χ2 = 6.41, p = 0.011) in COPD; use antidepressants (χ2 = 6.25; p = 0.044) and refer to PC (χ2 = 5.83; p = 0.016) in fILD; and use a handheld fan in COPD (χ2 = 8.75, p = 0.003), fILD (χ2 = 4.85, p = 0.028) and LC (χ2 = 5.63; p = 0.018). Conclusions These findings suggest a need for improved dissemination and uptake of jointly developed breathlessness management guidelines in order to encourage appropriate use of existing, evidence-based therapies. The lack of opioid use by RM, and continued benzodiazepine use in PC, suggest that a wider range of acceptable therapies need to be developed and trialled.
A tanulmány dedikáltan az Információs Társadalom folyóirat jubileumi számába készült, elsődleges célja, hogy áttekintse a folyóirat elmúlt éveinek publikációit és szerzői hálózatát. A tanulmány szerzői azt vizsgálják, hogy az Információs Társadalom folyóirat milyen témákat emelt be az „information society studies” hazai diskurzusába az elmúlt 15 évben. A cikk szerzői természetesnyelv-feldolgozással (Natural Language Processing, NLP), azon belül is topikmodellezéssel elemzik a folyóiratban 2005-től megjelent tanulmányokat, hogy feltárják a folyóirat tematikus szerkezetét. Az elemzés során 10 topikot, vagyis látens témát különítenek el, például a hálózatok/rendszerek, az oktatás, az adatvédelem és az internet és közösségek területét azonosítva a Látens Dirichlet Allokáció (LDA) eljárás segítségével. A tartalmi elemzés mellett a cikk betekintést nyújt a folyóiratba publikálók társ-szerzőségi hálózatába, valamint a szerzők és az egyes topikok kapcsolatába is. A társszerzőség szerinti hálózatban három, viszonylag sok szerzőt tartalmazó algráf azonosítható, de a hálózat alapját jellemzően diádok, triádok és négyes csoportok alkotják. A szerzők cikkeinek topikhovatartozása azt mutatja, hogy a sokat publikáló szerzők cikkei jellemzően 3-4 topikba kerülnek.
Tanulmányunk a természetesnyelv-feldolgozás (Natural Language Processing – NLP) korrupciókutatásban való felhasználását és felhasználhatóságát vizsgálja. Átfogó irodalmi áttekintésünk során a 2000 után született, automatizált szövegelemzésre épülő korrupciókutatások teljeskörű összegyűjtésére és összegzésére törekedtünk az NLP alkalmazás elterjedtségére, illetve lehetőségeire fókuszálva. Lényeges eltéréseket találtunk a felhasznált szöveges adatforrást, a korrupció mérésének módját és az elemzési megközelítést tekintve, ugyanakkor sajnálatosan kevés volt az (adatforrását, módszerét vagy mérési módját tekintve) kevert típusú tanulmány. A klasszikus, a korrupció volumenét vagy a vele kapcsolatos attitűdöt vagy percepciót leíró, illetve észlelésének következményeit vizsgáló munkákon kívül találtunk a korrupció megelőzésére felhasználható eredményeket, sőt intervencióra közvetlenül alkalmasakat is. Az NLP-t csupán néhány tanulmány használta, és ezek egy része sem annyira tartalmi, mint csupán technikai feladatra. Eredményeink szerint az NLP nem nagyon elterjedt még ezen a területen, ugyanakkor az is látható, hogy gyümölcsöző lehet a használata: alternatív eszközként jól támogathatná a tradicionális kvantitatív kutatásokat. Cikkünk célja inspirációt adni az NLP társadalomtudományi felhasználására és felhívni a figyelmet annak beágyazhatóságára a meglevő tudományos diskurzusokba.
Experts state an increasing trend toward vaccination hesitancy in the last decade, which also seems tangible concerning COVID-19 vaccines. The impact of online media on this trend is expanding; social media is one of the top sources for seeking and sharing health-related information. One of the most widely disseminated topics of health-related online misinformation is connected to vaccination and spread by anti-vaccination groups. Since billions of articles, posts, and comments dealt with the Covid-19 pandemic, it has provided an opportunity to obtain detailed knowledge about the pro and anti-vaxxer attitudes in the online sphere. Our study analyses anti-vaccination narratives, focusing on a group of people who express a rather partial anti-vaccination attitude in the Hungarian online sphere. The novelty of our analysis is to follow the birth and development of new and locally embedded narratives concerning vaccine hesitancy due to the country's unique situation, where one of the broadest ranges of vaccine types became available quite soon. Based on our analysis, anti-vaxxer comments are non-political or differentiate national politics from global aspects of COVID-19 vaccination. On the contrary, partially anti-vaxxer discourses are highly contextual and dependent on the continuous changing of the conditions. The east-west political narrative has severely impacted non-anti-vaxxer and somewhat anti-vaxxer groups and contributed to vaccination hesitancy.
ResumenEl nombre de Lajos Kassák (1887-1967, el representante más importante del vanguardismo húngaro, no es muy conocido entre el público hispanohablante debido sobre todo a que sus obras literarias, por falta de traducciones, no llegaron a los lectores españoles e hispanoamericanos. Sin embargo, tanto en la obra propia de Kassák como en las revistas fundadas por él muchas veces aparecen diferentes temas hispanos. AbstractThe name of Lajos Kassák (1887Kassák ( -1967, the greatest figure of the Hungarian avant-garde is not well known to the Spanish-speaking public, mainly because the lack of translations of his literary works, that consequently did not reach Spanish and Hispanic American readers. However, both in Kassák's own work and in the journals he founded, many different Hispanic themes do appear. The present article examines the Spanish and Hispanic American motifs and personalities receiving attention and publicity in the five journals -A Tett, Ma, Munka, Dokumentum y Kortárs− founded by Lajos Kassák.
