Çalışmamızda doğum analjezisi yöntemlerinden, hasta kontrollü epidural analjezi (HKEA) ile hasta kontrollü intravenöz remifentanil analjezisi (HKİVA), hasta memnuniyeti, vizüel analog skala (VAS), anne ve yenidoğan yan etkileri açısından karşılaştırıldı. Gereç ve Yöntem: Tek fetüse sahip, doğum analjezisi isteyen 37 gebe HKİVA (Grup 2) ve HKEA (Grup 1) gruplarına ayrıldı. Grup 1 için hazırlanan 100 mL epidural mayi içinde 1.25 mg/mL bupivakain, 2 mcg/mL fentanil bulunmaktaydı. Başlangıç dozu, 5 mL'lik bölünmüş dozlar halinde 15 mL uygulandı. Devamında 5 mL/saat bazal infüzyon, 5 mL hasta kontrollü bolus ve 20 dk kilit olarak ayarlandı. Grup 2 için hazırlanan solüsyona, 100 mL mayi içine 2 mg remifentanil konuldu. 20 mcg/h infüzyon dozu, 0.05 mcg/kg hasta kontrollü bolus ve 5 dk kilit olarak ayarlandı. Gebelerin, doğum eylemi bitene kadar VAS, maternal kalp hızı, kan basıncı, oksijen satürasyonu, bulantı, kusma, kaşıntı, sedasyon, fetal kalp hızı ve doğum eylemi sonunda yenidoğan Apgar skoru ve anne memnuniyetleri kaydedildi. Bulgular: İki grubun da yeterli düzeyde analjezi sağlayabildiği görülmüştür. Ancak VAS değerleri grup HKİVA'da daha yüksek bulunmuştur. Anne memnuniyetleri ve yenidoğan Apgar skorları benzerdi. Her iki grupta da oksijen desteği gerektiren satürasyon düşüklüğü saptanmamasına rağmen, oksijen satürasyonları Grup 2'de daha düşük seyretti. Yan etki ve hasta memnuniyetleri her iki grupta da benzer bulundu. Sonuç: Remifentanil ile intravenöz HKA'da, anne memnuniyeti yeterli görülse de, HKEA ile karşılaştırıldığında 2. saatten sonraki VAS skorları daha yüksekti. Biz HKEA'nın doğum analjezisinde altın standart olduğunu düşünmekle birlikte, epidural kateter uygulamasının kontrendike olduğu veya gebenin epidural girişimi istemediği durumlarda, HKİVA'nın epidural analjeziye iyi bir alternatif olduğunu düşünmekteyiz.
ÖZET Amaç: Yoğun bakım hastalarında nörolojik durumun değerlendirilmesinde en yaygın olarak kullanılan ölçek Glasgow Koma Skalası'dır (GKS). GKS'deki sınırlamalar entübe veya afazik hastalardaki verbal yanıt zorluğunu içermektedir. Full Outline of UnResponsiveness (FOUR) skoru son zamanlarda önerilen sözlü yanıta bağımlı olmayan yeni bir koma skalasıdır. Yeni skalalar, ileri nörolojik ayrıntıyı kolayca sağlayabilen bir skalayı kuvvetle önermektedir. Bizim çalışmamızın amacı FOUR skorun GKS ile yoğun bakımda yatan tüm hastalarda karşılaştırılması ve gözlemciler arası güvenirliğinin saptanmasıdır.Gereç ve Yöntemler: Çalışmamıza 105 hasta dahil edildi. Hastalar üç farklı uygulayıcı (yoğun bakım hemşiresi, 2.yıl anestezi asistanı, anestezi uzmanı) tarafından FOUR ve GKS skorları açısından değerlendirildi. Sedasyon veya kas gevşetici alan hastalar çalışma dışı bırakıldı. Her hasta, üç uygulayıcı tarafından, birbirinin değerlendirmelerinden habersiz ve en fazla bir saat aralıklı olacak şekilde değerlendirildi.Bulgular: Çalışmamızda GKS ve FOUR Skorları, uygulayıcılar arasındaki uyumluluk bakımından karşılaştırılmaktadır. FOUR skorunun entübe ve afazik hastalarda üstün olabileceği düşünülse de istatistiksel olarak ne GKS ve FOUR Skorları ne de uygulayıcılar arasında anlamlı fark saptanmadı.Sonuç: Sonuç olarak, yoğun bakımlarda kullanılan skorlar basit, güvenilir ve öngörülebilir olmalıdır. Bizim çalışmamız göstermiştir ki, FOUR skor en az GKS kadar güvenilirdir. Ayrıca GKS ve FOUR skorlarının hem doktorlar hem de hemşireler için kullanımı kolaydır.Anahtar Kelimeler: Full Outline of Unresponsiveness Score, Glasgow Koma Skalası, yoğun bakım ünitesi ABSTRACT Objective: The Glasgow Coma Scale (GCS) is the most widely used scoring system to evaluation of neurological status for patients in intensive care unit. Limitations of the GCS include severe to assess the verbal score in intubated or aphasic patients. The Full Outline of UnResponsiveness score (FOUR), a new coma scale not reliant on verbal response, was recently proposed. New scales strongly suggest a scale is needed that could provide further nerological detail that is easy to use. We aimed to compare FOUR score and GCS among unselected patients in intensive care units and comparerealibility betweenobservers.Material-Methods: In our study 105 patients was admitted. Three different types of examiners tested FOUR score and GCS: one intensive care unit nurse, one anaesthesiology resident (2. year), and one anaesthesiology fellow. Patients receiving sedative agents or neuromuscular function blockers were excluded. The raters performed their examination within 1 hour of each other without knowledge of the others scores.Results: In our study compared the interrater agreement of GCS and FOUR score. BACKGROUND: This prospective randomized study aims to compare outcomes between immediate laparoscopic cholecystectomy (LC) and same admission delayed LC in patients with acute cholecystitis and also to investigate the relation between oxidative stress markers and complication rates in the patients with AC. METHODS: This study included 64 patients with AC who were randomly divided into two groups. Patients in Group 1 (n=32) were immediately administered LC, while in Group 2 (n=32) patients underwent transient LC following medical treatment. All patients were operated on their first hospitalization. RESULTS: No statistically significant differences were observed between the groups for the comparison of complications, conversion rates, or operation durations (p>0.05). The length of postoperative hospital stay was found to be significantly shorter in group 1 compared to group 2 (1.75 vs 2.93 days; p=0.024). Only the total antioxidant status result was significantly higher in group 1 (p=0.017), but the finding was not correlated with complications. CONCLUSION: LC for AC was performed during the first admission was found to be safe, even beyond 72 hours following symptom onset. Pre-operative oxidative stress markers did not correlate with the complication rates.
