GirişEl-ayak-ağız hastalığı enterovirus ailesine ait viruslara bağlı olarak gelişen, deri ve mukoza lezyonlarına yol açan, çoğunlukla komplikasyonsuz seyreden sistemik bir infeksiyondur. En sık "Coxsackievirus" A16 ile ortaya çıkan hastalığa başka koksaki viruslar da (A5, A7, A9, B1, B3) neden olabilmektedir. İkinci sıklıkla görülen etken ise Enterovirus 71 olup salgınlara yol açabilme özelliği ile öne çıkmaktadır. Hastalık 5 yaş altı çocuklarda daha sık olmakla birlikte her yaşta görülebilir 1,2 . Solunum yoluyla bulaşır. Lenf nodlarında çoğalan virus viremi sırasında deri ve mukozaya gelmektedir. Kuluçka süresi 5-7 gün kadardır. Sıklıkla ateş, halsizlik, iştahsızlık ve boğaz ağrısı ile başlar. Birkaç günlük prodrom dönemi sonrası önce az sayıda, küçük veziküller veya aft benzeri ağız lezyonları ortaya çıkar 3 . El ve ayak lezyonları, oral veziküller ile birlikte veya kısa bir süre sonra ortaya çıkar. Sayıları değişkendir, bazen yüzlerce olabilir. Özellikle el-ayak parmaklarının ventral ve lateral bölgeleri ile avuç içi ve ayak tabanında 2-10 mm'lik, eritemli halka ile çevrili, oval, uzun eksenleri deri çizgilerine paralel olan vezikülöpüstüller en tipik bulgusudur. El-ayakağız hastalığı olarak adlandırılmış olmakla birlikte diz, dirsek, gluteal bölge ile gövde ve nadiren yüz de tutulabilir. Eritemli makülopapüler lezyonlar da eşlik edebilir. Lezyonlar gerilerken görülen deskuamasyon ön plana çıktığında klinik tanı güçleşir. Lenfadenopati bulunabilir 1-3 . Sum maryWe have reviwed that there is a dramatic increase in the frequency of hand-foot-mouth disease at Dermatovenerology Department of İstanbul Medical Faculty in June 2013 compared to previous years. When we compared these data with those of June 2011, we have determined a significant difference statistically. We have learned from the authorities of City Health Administrative that they had not been reported about this issue. Although it is difficult to comment based on the data of a single clinic, we want to draw attention that there is a possibility for epidemic disease and a need for national data about this disease. ÖzetTrikoblastom nadir görülen bir kıl folikülü tümörü olmakla birlikte, nevus sebaseus zemininde gelişen tümörler içinde en sık görülen tümördür. Sağlam deride yerleştiğinde genellikle saçlı deri ve boyunda ortaya çıkan, soliter, asemptomatik, deri renginde, ülsere olmayan papül veya nodül şeklinde görülür. Gerek klinik, gerekse histopatolojik açıdan bazal hücreli karsinom ile karışması nedeniyle önem taşır. Birçok histopatolojik alt tipi tanımlanmış olup, bunlar içinde pigmente trikoblastom nadir rastlananlar arasındadır. Genç erişkin bir kadın hastada saçlı deride öncesinde başka bir lezyon bulunmaksızın, 4 yıl içinde büyüyerek yaklaşık 2 cm'ye ulaşmış, mavimsi siyah renkli nodüler lezyona histopatolojik inceleme ile pigmente trikoblastom tanısı kondu. Trikoblastomun öncü bir lezyon olmaksızın da ortaya çıkabileceğine ve bazen pigmente olabileceğine dikkat çekilmektedir. ( AbstractWhile trichoblastoma is a rare tumor with hair follicles differentiation, it is the most frequent benign tumor developing on nevus sebaceous. It generally appears as a solitary asymptomatic skin-coloured papule or nodule on the scalp and neck. It should be differentiated clinically and histopathologically from basal cell carcinoma. There are various histopathologic subtypes of this adnexal tumour and pigmented trichoblastoma is one of the rarest forms. A young adult female patient presented with a pigmented smooth-surfaced nodule on the scalp, which had enlarged within four years without a preexisting lesion. The histopathological diagnosis was pigmented trichoblastoma. Trichoblastoma may also present without a preexisting nevus sebaceous and may sometimes be of the pigmented type. Miçetoma veya 'Madura Ayağı', gerçek mantarlar (ömiçetoma) veya Aktinomiçes (aktinomiçetoma) türü bakteriler tarafından oluşan kronik granülamatöz yumuşak doku enfeksiyonudur. Bu zor gelişen mikroorganizmaları, biyopsi veya mikrobiyolojik kültürde göstermek zor olabilir, bu yüzden tanı ve tedavi gecikmeleri nedeniyle artmış morbidite gelişebilir. Objective: Community-acquired methicillin-resistant Staphylococcus aureus (CA-MRSA) infections have been increasingly reported in recent years, and they commonly cause skin and skin-related structure infections. The aim of this study was to investigate CA-MRSA frequency and nasal carriage in patients with pyoderma. Methods: Between November 2011 and August 2013, we aimed to investigate the frequency of MRSA isolated from nasal swab and wound specimens of 38 patients with pyoderma according to the "community-acquired infection" criteria of Centers for Disease Control and Prevention. Also, the criteria investigated included age, gender, number of people residing in the same household and their hospitalization frequency, chronic illnesses, and antibiotic use in the previous six months. The control group of 103 patients who were evaluated with the same criteria and with various skin diseases excluding pyoderma were examined by culturing their nasal swabs. Results: Totally, 38 specimens obtained from patients with folliculitis (n=18), furuncle (n=7), cellulitis (n=4), abscess (n=2), hidradenitis suppurativa (n=2), carbuncle (n=1), and impetigo (n=4) were investigated, and 2 (5.3%) CA-MRSA were isolated. One of these strains were isolated from a patient with folliculitis, and the other strain was isolated from nasal swab of the another patient of whose sample from folliculitis was negative for CA-MRSA.
