Purpose: Detection of the relationship between the activity of SLE and the histopathology of renal biopsy. Patients and methods: This is a cross-sectional study. It included one hundred Egyptian patients with SLE according to the 2015 ACR/SLICC Revised criteria. SLE Disease Activity Index (SLEDAI) and renal biopsy were done on all patients in addition to the routine investigations.Results: Of the one hundred patients; Nine are males and 91 are females with a mean age ± standard deviation of 32.28 ± 9.59 years. The mean value of SLEDAI was 11.92 ± 3.92. Lupus nephritis (LN) was found in 85 patients (85%) included 5 patients (5%) having class 1, 73 patients (73%) having class 2, and 7 patients (7%) having class3 LN. SLEDAI mean value is directly proportional to the class of LN increasing from 10.6 ± 3.91 in normal cases to 16 ± 3.92 in class III LN. Conclusion:The relationship between the SLEDAI score and the histopathological study of renal biopsy in patients with SLE is strong and significant with a mean value of SLEDAI directly proportional to the class of LN.
Objectives:To study changes in lipid profile in patients with rheumatoid arthritis (RA) and to evaluate the relation between lipid profile and disease severity of RA. Design: Cross-sectional study. Patients:Patients with rheumatoid arthritis (RA) according to 2010 ACR/EULAR classification criteria for RA (n = 150). Methods: Demographic data, arthritis history, medical and therapeutic history were evaluated. Disease activity was evaluated using DAS28 (ESR). Immunological investigations included RF, ESR, CRP, and anti-CCP.Lipid profile levels were measured. Results:Raised TG was found in 38 (25.3 %) of patients, raised TC was found in 69 (46 %) of patients and decreased HDL-C was detected in 85 (56.7 %) of the patients.
Behçet's disease (BD) is a chronic inflammatory disease with multisystem involvement. It presents with remission and exacerbation. The aim of our study was to study the pattern of Behcet's disease among patients attending the Rheumatology and Rehabilitation department, Sohag University. Conclusion: Behcet's disease starts frequently around the beginning of the third decade and has a male predominance (70%). The age at diagnosis of Behcet's disease ranged from 19-59 years, with a mean of around 28.8 years. Most of the cases in our study were married (73.3%). It was found that the most common first manifestation in our study group was oral ulcers, followed by ocular manifestations, then CNS manifestations, articular or vascular manifestations and lastly genital ulcer or skin manifestations.
Systemic lupus erythematosus (SLE) is an autoimmune disorder affecting many systems in the body. Because of the disease's tremendous variety, some researchers believe that it is a syndrome rather than a single disease. One of the most prevalent manifestations of SLE is lupus nephritis (LN). Nearly 40% to 70% of patients with SLE have LN. The manifestations of LN range from asymptomatic urinary findings to nephrotic syndrome and progressive renal impairment. The International Society of Nephrology and the Renal Pathology Society (ISN/ RPS) have indicated that the various LN classes exhibit different natural history and clinical patterns. Subclinical lupus nephritis (SLN) is nephritis diagnosed via renal biopsy despite normal urinalysis. Renal biopsy is the most important procedure to detect renal damage and SLN.
Objectives:To determine the prevalence of metabolic syndrome (MetS) in patients with rheumatoid arthritis (RA) and to evaluate the relation between parameters of MetS and clinical aspects of RA. Design: Cross-sectional study. Patients:Patients with rheumatoid arthritis (RA) according to 2010 ACR/EULAR classification criteria for RA (n = 150). Methods: Demographic data, arthritis history, medical and therapeutic history were evaluated. Height, weight, BMI, waist circumference (WC) and blood pressure (BP) were recorded. Disease activity was evaluated using DAS28 (ESR). Immunological investigations included RF, ESR, CRP, and anti-CCP. Fasting blood glucose and lipid profile were measured.The patient was diagnosed as having MetS according to 3 definitions; ATPIII (2004), IDF (2005), and JIS (2009). Results:Prevalence of MetS in rheumatoid patients was 48% (ATPIII definition), 52 % (IDF definition), and 46 % (JIS definition with Egyptian cut-off of WC), respectively. The most prevalent component of MetS was central obesity (75.3-92 %according to the used definition).DAS28 was significantly higher in patients with MetS with higher number of tender joints and VAS (0-100) and most rheumatoid patients with MetS (68 %) had high disease activity. Conclusions:The increased prevalence of MetS components in RA patients, suggests greater attention be given to modifiable risk factors, including improvement of dietary habits, physical activity and blood pressure control.
