BackgroundLarge loop excision of the transformation zone (LLETZ) has been used for the diagnosis and treatment of precancerous cervical lesions, and it is the first choice of treatment in the majority of cervical pathology services. The aim of this study was to evaluate the presence of thermal artifacts, the need for serial sections, the percentage of clear and involved resection margins and the relationship between endocervical gland involvement and the severity of the lesion in samples resected using LLETZ.MethodsA retrospective study was performed at Santa Casa de Misericordia School of Science (HSCMV), Vitoria, Espirito Santo, Brazil with a sample of 52 histopathology slides from patients submitted to conization because of abnormal cytology findings and a biopsy result of cervical intraepithelial neoplasia (CIN) 2, CIN 3 and adenocarcinoma in situ. Statistical analysis was performed using Student’s t-test.ResultsSerial sections were required to confirm diagnosis in four of 52 cases. Thermal artifacts were present in all cases, with grade I being the most common (94.2% of cases). Clear margins were found in 96.2% of cases. No association was found between glandular involvement and CIN 1 (P > 0.05); however, there was an association with CIN 2 and CIN 3 (P < 0.05).ConclusionThe amount of excised tissue was sufficient, thermal artifacts were slight, resection margins were clear in most of cases, and a possible association was found between glandular involvement and the severity of the lesion.
O propósito deste artigo é apresentar o conhecimento e os sentimentos dos docentes na abordagem de temas relacionados aos cuidados paliativos como: finitude, morte e luto do curso de graduação em medicina da Universidade de São Caetano do Sul. Foi realizado um estudo transversal quantitativo e exploratório, realizado a partir dos preceitos e etapas do método científico, com aplicação de questionário semi-estruturado, por meio do googleforms aos professores do curso de graduação de medicina. O instrumento metodológico utilizado foi um questionário de 17 questões investigando informações pessoais, profissionais e sobre CP. A amostra foi composta por 106 participantes. A partir dos resultados, observamos que cerca de um terço dos docentes não têm formação na área de cuidados paliativos. Em consideração aos assuntos de morte e luto, menos de 50% dos docentes tiveram esses conhecimentos na graduação. A pesquisa também revelou que aproximadamente 88% dos docentes conversam com os discentes sobre morte com segurança, em contraste com a literatura em que muitos médicos se sentem receosos ao tratar sobre morte e paliativismo, pelo fato de serem mal interpretados. Espera-se com a presente pesquisa que os resultados possam ser efetivamente conhecidos pelos docentes das faculdades de medicina, que atuem com mais sensibilidade às necessidades da população e indivíduo, e consequentemente compartilharem com os discentes uma abordagem mais humanizada.
O propósito deste artigo é apresentar o processo de elaboração e os recursos metodológicos de um curso de atualização docente em cuidados paliativos. Como a maioria dos docentes de faculdades de medicina foram graduados antes das diretrizes curriculares nacionais de 2014, a formação foi predominantemente técnica, que visa à cura em detrimento ao cuidado. O objetivo do curso é capacitar docentes sobre conhecimentos e habilidades da temática cuidados paliativos, sobre a importância de lidar com a finitude, morte e luto; e desenvolver noções básicas de cuidados paliativos, comunicação e luto. Para tal, foi utilizada a sequência didática: atividade pré-sala de aula (texto para leitura prévia e aula gravada com conhecimentos técnicos de comunicação); primeiro encontro presencial (acolhida dos participantes; questionário sobre comunicação; discussão sobre percepções sobre o texto; exibição de excertos de filmes com cenas de comunicação; discussão dos sentimentos mobilizados; role-play seguido de feedback e; feedback do primeiro encontro); segundo encontro presencial: leitura e processamento da situação problema – “Mas cuidado paliativo não é deixar morrer?” seguido de feedback do segundo encontro; terceiro encontro presencial com processamento da nova síntese, seguida de feedback do terceiro encontro e; feedback geral do curso. A sistematização do curso de atualização docente em cuidados paliativos pode favorecer a reprodutibilidade deste produto educacional, contribuindo para melhorar a qualidade da formação docente e discente na área da saúde. O uso de metodologias ativas na construção do curso torna-o mais atrativo e facilita o processo de aquisição de conhecimento e o protagonismo discente.
