In cases of fall from a height, the presence of laryngohyoid fracture or bleeding at autopsy constitutes a suspicion of strangulation before trauma in these types of cases. This study has aimed to investigate the incidence of laryngohyoid fractures in nonhomicidal fall from height cases, and their relationship with age, sex, height of fall, body mass index, and other injuries.A total of 170 cases proven to be nonhomicidal falls from height with certain witnesses and evidences in 4683 autopsy cases in 2013 were included in this study.The mean age of the patients included in the study was 39.96 ± 22.25, and 70.6% of the cases were men. It was found that 118 (69.4%) of the deaths were accidental and 52 (30.6%) were suicidal. Laryngohyoid fracture was determined in 9.4% of the cases. In the multivariate logistic regression analyses, it was found that as the height increased, the incidence of hyoid bone fracture increased, and as the age increased, the incidence of thyroid cartilage fracture increased significantly. Furthermore, in cases with cervical spine fractures, the incidence of hyoid bone fracture, and in cases with clavicle or cervical spine fractures, the incidence of thyroid cartilage fracture were observed to be significantly higher.When there is a suspicion in falls from height with laryngohyoid fractures, the height of fall, the age of case, and the injury in the adjacent bones contribute to the determination of the cause and manner of death.
The management of biliary disorders in patients with Roux-en-Y hepaticojejunostomy anastomosis is challenging and remains controversial. Our aim is to share our experiences of endoscopic treatment via a permanent access loop in 5 patients. Endoscopic treatment via a permanent access loop is an invaluable procedure for the management of stenotic hepaticojejunostomy anastomosis, anastomotic leakage, and hepatolithiasis. In the autopsy of choking cases, diagnosis is made through detecting a foreign body in the airways or determining the findings of asphyxia. If any specific reasons of death cannot be determined during the autopsy, the possibility of choking is taken into account. There are a lot of risk factors that cause choking. We found out during the autopsy that meat which was found in oesophagus blocked larynx completely. Antipsychotic medications were detected in the toxicological analyses of blood and bile. Otopsi sırasında özofagus girişinde kalan büyük bir et parçası larinks hava yolu girişini tamamen tıkamış bir şekilde bulunmuştur. Aynı zamanda servikal 5. vertebrada (C5) bulunan hiperosteotik formasyonun özofagus girişini daralttığı izlenmiştir. Kan ve safranın toksikolojik analizlerinde antipsikotikler grubunda bulunan ilaçlar tespit edilmiştir. ÖZET AMAÇ: Genel cerrahi, komplikasyon ile hata karşılaştırmasında karar vermenin zor olduğu dallardan biridir. Çalışmamızda tıbbi uygulamada hata iddiası bulunan genel cerrahi ölüm olgularında, tıbbi uygulama hatasının değerlendirilmesi için göz önüne alınan temel adli tıbbi parametrelerin sunulması ve bu kavramların literatür üzerinden tartışılması amaçlandı. 01.01.2012-31.12.2013 tarihleri arasında, genel cerrahi branşı hekimleri hakkında tıbbi uygulamada hata iddiası nedeniyle Adli Tıp Kurumu Birinci Adli Tıp İhtisas Kurulu'na gönderilmiş ve tıbbi uygulama ile ölümün illiyetli olduğu belirlenmiş olgular geriye dönük olarak incelendi. GEREÇ VE YÖNTEM: BULGULAR:Toplam 105 olgunun %21.9'unda (n=23) tıbbi uygulama hatası olduğu ve en sık primer hastalık tanılarının sırasıyla travma-yaralanma (n=32, %30.5), kolesistit (n=25, %23.8) ve apandisit (n=8, %7.6) olduğu görüldü. Tedavi şekilleri, tıbbi uygulamada hata kararı açısından karşılaştırıldığında, sadece medikal tedavi olan olgularda hata gözlenme oranı, cerrahi+medikal tedavi gören olgulara göre istatistiksel olarak anlamlı düzeyde yüksek bulundu (p=0.003; p<0.01). Ancak, acil ve elektif cerrahi tedaviler, tıbbi uygulamada hata kararı açısından karşılaştırıldığında istatistiksel olarak anlamlı farklılık saptanmadı (p>0.05). Klinik tanı ile otopside belirlenen tanı arasındaki ilişkinin tıbbi uygulamada hata kararı üzerindeki etkisi değerlendirildiğinde; klinik tanı ve -otopsi var olduğunda-otopsi tanısının uyum durumu ile tıbbi uygulamada hata kararı arasında istatistiksel olarak anlamlı farklılık tespit edildi (p=0.031; p<0.05). Klinik tanının otopsi ile doğrulandığı olgularda, hata olduğu yönünde karar verilme oranı istatistiksel olarak anlamlı düzeyde düşük olarak bulundu (p=0.028; p<0.05). SONUÇ:Elde edilen sonuçlar yorumlandığında, Adli Tıp Kurumu'nun tıbbi uygulama hatası değerlendirmesinde otopsi verileri kadar klinik takip verilerini de dikkate aldığı, genel cerrahide medikal tedavinin doğru uygulanmasına önem verilmesi gerektiği ve hekimlerin acil şartlarda elektif şartlar kadar başarılı oldukları unsurları dikkat çekici bulunmuştur.Anahtar sözcükler: Adli tıp; genel cerrahi; otopsi; tıbbi uygulama hatası.
