PurposeThe aim of this article is to report a case of uveal tuberculoma simulating an intraocular tumor in which the diagnosis was only possible by polymerase chain reaction (PCR) analysis.MethodThis is a case report.ResultsA36-year-old male presented with a progressive growing intraocular tumor and no history or positive test for other systemic disease. The eye eventually turned blind and painful and was enucleated. Histopathologic analysis revealed a granulomatous reaction and caseation necrosis but failed to identify any causative microorganisms. Final tuberculosis diagnosis was only possible by quantitative PCR.Conclusion Isolated uveal tuberculoma can present in an otherwise healthy patient with negative systemic tuberculosis evaluation. PCR can be used to confirm tuberculosis when other methods have failed.
A rinossinusite aguda é uma das afecções mais prevalentes das vias aéreas superiores. Fatores anatômicos presentes em crianças e jovens propiciam o aparecimento de complicações orbitárias. Embora mais raras, as complicações intracranianas das rinossinusites perfazem um grau alto de letalidade, são mais comuns em pacientes acima de sete anos, e devem ser tratadas por uma equipe multidisciplinar.
RESUMOMulher de 59 anos apresentava proptose e dor em olho esquerdo com trinta dias de duração. A tomografia computadorizada (TC) de órbitas mostrava massa em órbita esquerda com destruição óssea e invasão intracraniana. A biópsia da lesão revelou hepatocarcinoma. Na investigação sistêmica, a TC de abdômen evidenciou presença de tumor hepático. Iniciou tratamento quimioterápico, evoluindo com redução significativa da massa orbital, porém cerca de quatro meses após o fim da quimioterapia voltou a apresentar aumento da lesão orbital vindo a óbito dezenove meses após o diagnóstico.
RESUMOSão relatados dois casos da associação de micose fungóide e HTLV-I. Os achados oftalmológicos observados foram, no caso 1: ceratopatia ponteada e ceratoconjuntivite seca e; no caso 2: ceratoconjuntivite seca, espessamento e nódulos difusos em bordas palpebrais. A associação de micose fungóide e HTLV-I nos dois casos sugere envolvimento deste vírus na etiopatogenia da doença. Canaliculitis is an uncommon condition of the lacrimal canaliculi. The presentation is usually unilateral and usually runs with epiphora, hyperemia, edema, and pain, with discharge of secretion by the lacrimal point. The diagnosis is clinical since its findings are quite typical. Surgical treatment with canaliculotomy has been described as the most effective. Canaliculitis should be considered in recurrent cases of conjunctivitis and the ophthalmologist should be aware of the signs for earlier management of cases.
