Although CNS paracoccidioidomycosis might represent a suspicious lesion pattern on CT scan examination, correlation of the CT scan findings, clinical data, plain chest films, and laboratory results is necessary to define the diagnosis of this uncommon entity.
-bilateral and symmetric globus-pallidus hyperintensities are observed on t1-weighted MrI in most of the patients with chronic liver failure, due to manganese accumulation. We report a 53-year-old man, with rapid onset parkinsonism-dementia complex associated with accumulation of manganese in the brain, secondary to liver failure. A brain MrI was performed and a high signal on t1-weighted images was seen on globus-pallidus, as well as on t2-weighted images on the hemispheric white-matter. He was referred to a liver-transplantation. the patient passed away on the seventh postoperative day. Our findings support the concept of the toxic effects of manganese on the globus-pallidus. the treatment of this form of parkinsonism is controversial and liver-transplantation should not be considered as first line treatment but as an alternative one.Key WOrds: manganese, liver transplantation, parkinsonism-dementia, globus pallidus, high-signal.Transplante hepático em um paciente com complexo parkinsonismo-demência rapidamente progressivo induzido por manganismo devido a insuficiência hepática resuMO -Hiperintesidades simétricas e bilaterais dos gânglios da base são observadas em imagens de ressonância magnética encefálica (rM) ponderadas em t1 na maioria dos pacientes com insuficiência hepáti-ca crônica devidas ao acúmulo de manganês. Nós relatamos o caso de um homem, com 53 anos de idade, com um complexo parkinsonismo-demência rapidamente progressivo associado com o acúmulo de manganês no cérebro, secundariamente a insuficiência hepática. uma rM encefálica foi realizada e foram observadas imagens hiperintensas/hipersinal nas imagens ponderadas em t1 no globo pálido e, também, na substância branca dos hemisférios cerebrais ponderadas em t2. devido à falta de resposta ao tratamento clinico optamos pelo transplante hepático. O paciente faleceu no 7º dia de PO. Nossos achados corroboram o conceito dos efeitos tóxicos do manganês nos gânglios da base/globo pálido. O tratamento desta forma de parkinsonismo é controverso e o transplante hepático não deverá ser considerada uma opção terapêutica de primeira linha, porém como um tratamento alternativo considerando-se os riscos-benefícios dessa escolha.PAlAvrAs-cHAve: manganês, transplante hepático, complexo parkinsonismo-demência, globo pálido.
-Objective: To determine the influence of the location of the irritative zone, and the number and the distribution of the intracranial calcifications in the severity of epilepsy associated with intracranial calcifications. Method: We studied 47 patients with epilepsy and intracranial calcifications, 24 with normal (Group A) and 23 with abnormal interictal EEGs (Group B), a control group (n=21) with abnormal interictal EEGs and normal CT-scans (Group C). Clinical, electroencephalographic and neuro r a d i o l o g i c a l f e a t u reswere compared among gro u p s . Results: Temporal lobe interictal EEG abnormalities were found in 23/24 Group B patients, and in all Group C patients. Most Group B and Group C patients presented temporal lobe seizure symptomatology, whereas in most Group A patients symptomatology was ro l a n d i c (p=0.0001). Epilepsy was more severe in Group B and Group C patients than in Group A patients (p=0.0001 and p=0.0054). No relationship was found between the number of calcifications and epilepsy severity. Método: Foram selecionados 47 pacientes com epilepsia e calcificações intracerebrais, 22 com EEGs i n t e rcríticos normais (Grupo A) e anormais em 23 (Grupo B) e um grupo controle (n=21) com EEGs intercríticos anormais sem calcificações intracerebrais (Grupo C). Características clínicas, eletrencefalográficas e n e u roradiológicas foram comparadas entre os gru p o s . Resultados: Encontramos anormalidades eletre n c efalográficas temporais em 23/24 dos pacientes do Grupo B e em todos do Grupo C. Na maioria dos pacientes do Grupo B e Grupo C a sintomatologia ictal foi interpretada como temporal, porém no Grupo A como rolândica (p=0,0001) A epilepsia foi mais grave nos grupos Grupo B e Grupo C que no Grupo A (p=0,0001 e 0,0054). Não houve relação direta entre o número de calcificações e a gravidade da epilepsia. The purpose of this study is to test the sensitivity of the volumetric analysis compared to the visual analysis of the hippocampal formations of a group of 153 patients with clinical diagnosis of temporal lobe epilepsy and candidates to temporal lobectomy, evaluated by magnetic resonance (MR), using a 0.5 Tesla machine and inversion-recovery T1-weighted images and 5 mm coronal slices. There was agreement between the prospective visual analysis and another retrospective visual analysis carried out by two independent observers (C = 0.748 and C = 0.720). There was also agreement between the retrospective analysis of the two investigators (C = 0.733). There was genuine agreement (C = 0.788) between the results of the qualitative and quantitative analyses carried out prospectively. Volume measurements showed a nonsignificant tendency to lateralize more cases of clinically presumed hippocampal atrophy. Our results confirm the reliability of a qualitative visual analysis and indicate the utility of hippocampal volumetry as a supplementary, objective and quantitative measure of hippocampal sclerosis.
RESUMO -A ossificação do ligamento longitudinal posterior (OLLP) é causa incomum de mielopatia compressiva na população caucasiana. É relatado o caso de um paciente do sexo masculino com um quadro de paraparesia espástica, cuja investigação radiológica mostrou OLLP. O raio-X de coluna cervical mostrou imagem laminar, vertical, com densidade óssea, posterior aos corpos vertebrais, que se estendia de C2 a T1. A tomografia computadorizada (TC) e a mielotomografia mostravam OLLP causando compressão medular ântero-posterior no segmento descrito. Na ressonância magnética, observou-se área de hiperintensidade em T2 no segmento C7-T1, compatível com mielomalácia. O paciente foi submetido a laminoplastia tipo "open-door", com melhora do quadro parético dos membros inferiores. A OLLP deve entrar no diagnóstico diferencial das mielopatias cervicais, sendo facilmente diagnosticada através de radiografias e TC da coluna cervical. São revisados os aspectos clínicos e radiológicos e o tratamento da OLLP. PALAVRAS-CHAVE: ossificação do ligamento longitudinal posterior, mielopatia cervical, tomografia computadorizada, ressonância magnética.Ossification of the posterior longitudinal ligament in the cervical spine: case report ABSTRACT -Ossification of the posterior longitudinal ligament (OPLL) is an uncommon cause of compressive myelopathy in the Caucasian population. A case of spastic paraparesis in a Caucasian man whose radiological investigation showed OPLL is presented. The radiographs of the cervical spine showed a strip of bony density posterior to the vertebral bodies, extending from C2 to T1. Computerized tomography (CT) and CT myelography showed OPLL at the same level. Magnetic resonance showed an area of increased signal on T2-weighted sequences at C7-T1 level suggestive of myelomalacia. The patient underwent an open-door laminoplasty (C2 to C7) with improvement of the paraparesis. OPLL should be included in the differential diagnosis of cervical myelopathy. It can be easily detected by plain radiographs and CT of the cervical spine. RESUMO -Métodos especiais de MRI foram utilizados em 5 pacientes com epilepsia do lobo temporal retrátanos ao tratamento clínico, com a finalidade de localizar a área epileptogênica. Imagens coronáis com espessura de 5 mm foram obtidas da borda posterior do tronco cerebral até a base do lobo frontal, utilizando-se as seqüências "Inversion Recovery T I " (TR=2500 msec, TE=26 msec, TI=600 msec) para avaliar perda de estrutura interna e atrofia do hipocampo e morfología do lobo temporal; Spin-echo T2w (TR=2500 msec, TE=120 msec) visando a avaliar sinais de alta intensidade anormais. Em 2 casos com focos esquerdos no EEG, a MRI mostrou atrofia, perda de estrutura interna e aumento de sinal no hipocampo, e atrofia do lobo temporal anterior, ipsilateralmente. Um caso com focos bilaterais independentes no EEG teve atrofia, perda da estrutura interna e aumento de sinal no hipocampo direito. Um caso com EEG focai direito mostrou atrofia em ambos os lobos temporais anteriores. Outro caso não mostrou anormalidades. Concluimos que esta metodologia pode facilitar a indicação cirúrgica em epilepsia do lobo temporal. Coronal sequences were obtained with 5 mm sections from the posterior margin of the brainstem to the base of the frontal lobe using: "Inversion recovery T l " (TR=2500 msec, TE=26 msec, T1=600 msec) to analyse loss of internal structure and atrophy of the hippocampus, and morphology of the temporal lobe; and Spin-echo T2w (TR=250 msec, TE=120 msec) looking for abnormal high intensity signal. In two cases with left focal EEG the MRI showed atrophy, loss of internal structure and abnormal high intensity signal in the hippocampus, and atrophy of the anterior temporal lobe, ipsilateraly. One case with independent bilateral foci at the EEG showed atrophy, loss of internal structure and abnormal high intensity signal in the right hippocampus. Other case with right focal EEG showed bilateral atrophy of the anterior temporal lobes. Another case was normal. Os autores apresentam o caso de paciente do sexo masculino, 62 anos de idade, com emagrecimento há quatro meses e diminuição da força muscular associada a parestesias em membros inferiores há dois dias. Foi submetido a mielotomografia, que demonstrou massa no mediastino posterior com destruição dos corpos vertebrais e invasão do canal medular, além de espessamento irregular das paredes do esôfago. Na evolução, foi submetido a estudo contrastado do esôfago, que demonstrou falha de enchimento irregular. A biópsia confirmou a presença de carcinoma de células escamosas. Este é o primeiro relato na literatura latino-americana (Lilacs) de carcinoma de esôfago com invasão de canal medular e manifestação inicial de síndrome de compressão medular.
