Total of 110 isolates belonging to 8 fungal species collected from intensive care units (ICUs) and operation rooms (ORs) at Assiut University hospitals were examined for their ability to produce some extracellular enzymes and mycotoxins which are considered as important factors involved in for fungal pathogenicity. The results revealed that 73, 92 and 78 out of the 110 tested isolates produced protease, lipase and urease respectively; meanwhile, 77 of the tested isolates exhibited some hemolytic activities. Chromatographic analysis (TLC) of the crude extract of the fungal isolates tested revealed that 79 isolates of them had the ability to produce at least one of these mycotoxic compounds (aflatoxins B1, B2, G1, gliotoxin, fumigillin, T-2, zearalenone, roridin A & E, verrucarin A & J, trichoveroids, satratoxin H & E). These results demonstrate that the opportunistic fungal species isolated from (ICUs) and (ORs) and tested exhibited some enzymatic and mycotoxic ability which are the most effective virulence factors contributing to fungal pathogenicity indicating that the management of infection control unit at Assiut University hospitals must be aware of not only bacterial but also fungal contamination.

Beta glucan (β-glucan) has promising bioactive properties. Consequently, the use of β-glucan as a food additive is favored with the dual-purpose potential of increasing the fiber content of food products and enhancing their health properties. Our aim was to evaluate the biological activity of β-glucan (antimicrobial, antitoxic, immunostimulatory, and anticancer) extracted from Saccharomyces cerevisiae using a modified acid-base extraction method. The results demonstrated that a modified acid-base extraction method gives a higher biological efficacy of β-glucan than in the water extraction method. Using 0.5 mg dry weight of acid-base extracted β-glucan (AB extracted) not only succeeded in removing 100% of aflatoxins, but also had a promising antimicrobial activity against multidrug-resistant bacteria, fungi, and yeast, with minimum inhibitory concentrations (MIC) of 0.39 and 0.19 mg/mL in the case of resistant Staphylococcus aureus (MRSA) and Pseudomonas aeruginosa, respectively. In addition, AB extract exhibited a positive immunomodulatory effect, mediated through the high induction of TNFα, IL-6, IFN-γ, and IL-2. Moreover, AB extract showed a greater anticancer effect against A549, MDA-MB-232, and HepG-2 cells compared to WI-38 cells, at high concentrations. By studying the cell death mechanism using flow-cytometry, AB extract was shown to induce apoptotic cell death at higher concentrations, as in the case of MDA-MB-231 and HePG-2 cells. In conclusion, the use of a modified AB for β-glucan from Saccharomyces cerevisiae exerted a promising antimicrobial, immunomodulatory efficacy, and anti-cancer potential. Future research should focus on evaluating β-glucan in various biological systems and elucidating the underlying mechanism of action.

Aspergillus species are the main causing agents of invasive apergillosis chest disease. Eighty isolates of Aspergillus species, A. flavus (20), A. fumigatus (15), A. niger (30) and A. terreus (15 isolates), previously isolated and identified from aspergillosis suspected patients at our lab in Assiut university hospitals, were assayed for their enzymes and toxins profile. The results revealed that, all of the tested isolates were able to utilize calf lung tissue and produce catalase and peroxidase enzymes. Meanwhile, 82.5-90% of the fungal isolates had the ability to produce protease, lipase, urease and phospholipase, whereas, 70% of isolates exhibited hemolytic activities. Thin layer chromatography (TLC) of the cleaned extracts of the tested isolates exhibited the ability of all A. flavus isolates assayed to produce aflatoxins B1 and G1, 53%, moreover, A. fumigatus isolates produced fumagillin and gliotoxin. On the other side, 43.3% and 23.3% of A. niger isolates produced ochratoxins and gliotoxin respectively. Virulence assay of 10 µl of gliotoxin standard and cleaned extracts of A. fumigatus toxic isolates showed necrotic area on Guinea Pigs lungs compared to the control. Thus, opportunistic fungi isolated from aspergillosis patients possess high enzymatic and toxic profile that might play an important role in their mycopathy.

Mycoflora of atmospheric air and dust samples collected from air conditioning systems in 12 of each I.C.U. (intensive care units) and O.R. (operation rooms) were tested using settle and dilution plate methods on four types of agar media and incubated at 25 °C. Forty-five fungal species representing 23 genera were isolated and identified. The most prevalent genera recorded were Cladosporium, Aspergillus, Penicillium and Fusarium. The total colony forming units of airborne fungi recovered in I.C.U. and O.R. ranged between 31.13-49.61 colonies/m 3 on the four types of media used. The fungal total catch of the dust samples collected from the air conditioning system filters in I.C.U. and O.R. were ranged from 65.5-170 colonies/mg dust. Since, the interest to replace synthetic xenobiotics by natural compounds with low environmental persistence and biodegradable to control such airborne fungal contaminants is needed. In this respect, essential oils showed to possess a broad spectrum of antifungal activity. Fungal static ability of six oils was tested on 30 different fungal isolates. Vapors of common thyme oil exhibited the strongest inhibitory effects on the tested isolates, whereas the headspace vapors of blue gum and ginger had no inhibitory effects on the tested fungal isolates. These data revealed that the air conditioning systems may be an important source of contamination in I.C.U. and O.R. of Assiut university hospitals. Thus, patients may be in risk of being exposed to contaminated atmospheric air by opportunistic fungi and the use of essential oils as an alternative option to control hospital wards from fungal contaminants needs further studies.

Contamination of the air by fungi in the intensive care units (ICU) in Assiut university hospitals was investigated. Air samples were collected using settle plate method using four types of media with Rose Bengal-Streptomycin Agar. The mycobiota was isolated and identified. Cladosporium spp., Aspergillus spp., Penicillium spp., Fusarium were recorded most prevalent genera, whereas; Alternaria and Scopulariopsis were found to be with moderate frequency level. Botryotricum, Circinella, Drechslera, Epicoccum, Ulocladium, and others were recorded as the least fungal contaminants. The Aspergillus genus showed the greatest spectrum by being represented by 17 species followed by Penicillium (five species) .Each of Cladosporium, Fusarium and Scopulariopsis was represented by two species while the remaining 18 genera were represented by only one species for each genus. The gross total fungal counts of air born fungi recovered were 1009, 858, 1236 and 1366 cfu/72 plates on the four types of media used (24 exposures, 5 minutes for each exposure). These data revealed that patients of I.C.U. in Assiut university hospitals may be in high risk due to being exposed to atmospheric air highly contaminated by those opportunistic fungal genera that may be deadly to immuno-comprmised individuals who must remain away from any airborne fungal contaminants.
