Alveolar echinococcosis (AE) is an infectious zoonotic disease that is caused by Echinococcus multilocularis. The disease is generally identified accidentally because of the long asymptomatic period, has a malignant behaviour, and mainly occurs in the liver. Usually it is diagnosed in adults and is very rare in pediatric patients. We report two cases of AE and 1 differential case between AE and cystic echinococcosis (CE) in children: two of them had lesions in the liver and one had rare extrahepatic presentation of a cyst in the spleen. All our patients received chemotherapy with albendazole because surgical treatment was not recommended. The children were followed-up from 10 to 30 months and no significant improvement was seen. In this report we discuss the difficulties we faced in the treatment and follow-up of these patients. We also review the main clinical manifestations, general diagnostic methods, and treatment options of AE according to the current literature.
Cistinė fibrozė yra pavojingas gyvybei monogeninis sutrikimas, charakterizuojamas kaip lėtinis kvėpavimo takų ir virškinamojo trakto pažeidimas. Cistinė fibrozė dažniausiai pasireiškia naujagimiams. Daugėja duomenų, įrodančių organizmo pokyčių pradžią gimdoje. Progresuojantis plaučių pažeidimas yra daugumos sergančiųjų mirties priežastis. Pasiektas didelis proveržis, aiškinantis cistinės fibrozės patogenezę molekuliniame lygmenyje, tačiau dar nepakankamai suvokiamos priežastys, sukeliančios kvėpavimo ir kitų sistemų pažeidimus. Tyrimo tikslas – gauti naujos informacijos apie cistinės fibrozės aktualumą, sergančiųjų sunkumus ir gydymo perspektyvas. Atlikta sisteminė mokslinės literatūros apžvalga. Peržiūrėta daugiau kaip 150 publikacijų, analizei atrinktos 33 publikacijos, paskelbtos ne vėliau nei prieš 10 metų ir išanalizuotas jų turinys. Straipsnyje pateikiama šiuolaikinių mokslo žinių apie cistinę fibrozę ir jos kilmę, ligai būdingą kliniką, diagnostikos galimybes bei sunkumus, šiuo metu prieinamą gydymą ir ateities perspektyvas. Tyrimo rezultatai atskleidė, kad nauji cistinės fibrozės gydymo pasiekimai įgalino pristabdyti ligos progresavimą ir padidino pacientų išgyvenamumą iki 40 metų, tad toliau tobulinamos esamos bei ieškoma naujų terapinių šios retos ligos pirminės prevencijos priemonių ir gydymo galimybių.
