Résumé -La niaprazine, commercialisée sous la dénomination de Nopron , est largement utilisée en France pour ses effets hypnotiques dans le cas des insomnies occasionnelles chez l'enfant. Ce produit est accessible sans ordonnance médicale. Le premier cas de la littérature impliquant l'administration répétitive de niaprazine comme arme chimique dans le but d'une agression sexuelle est présenté ci-dessous. Trois enfants (2 filles et 1 garçon) ont été sédatés et agressés par leur beau-père de façon répétée entre 2002 et 2006. Selon la demande du juge en charge de cette affaire, les cheveux des trois enfants ont été prélevés afin de réaliser une analyse segmentaire dans le but d'y rechercher des sédatifs (30 molécules) par chromatographie liquide couplée à la spectrométrie de masse en tandem. De la niaprazine a été retrouvé dans les cheveux des trois enfants à des concentrations allant de 21 à 3431 pg/mg. Ces concentrations, démontrant l'absorption répétée de niaprazine, ne peuvent pas être comparées à des résultats antérieurs, par manque de référence sur le sujet. This compound is available without medical prescription. The first case involving repetitive administration of niaprazine as a drug-facilitated sexual assault is reported. Three children (2 girls and 1 boy) were repetitively sedated and assaulted by their father-in-law between 2002 and 2006. Niaprazine's liquid formulation represents a good potential access to surreptitiously administer it in beverages. According to the request of the judge in charge of this case, hairs of victims were collected, segmented and screened for sedatives by LC-MS/MS. Niaprazine was detected in the range 21 to 3431 pg/mg for the three children. These concentrations could not be compared with previous results, due to a lack of literature.
e14086 Background: PD-1 and PD-L1 inhibitors have now demonstrated their efficacy. Finding predictive or pre-emptive response factors is crucial and the role of immune-related adverse events (irAEs) is widely debated. The aim of our study was to evaluate the correlation between the occurrence of irAEs and overall survival, according to the time of onset. Methods: We retrospectively collected efficacy and safety data from patients treated with nivolumab, pembrolizumab and atezolizumab for metastatic cancer between July 2015 and January 2019 at the Pitié-Salpêtrière Hospital. Progression-free survival (PFS) and overall survival (OS) were analyzed for the global population, for patients who had an irAES, according to the time of onset (before or after 12 weeks). Results: 158 patients were treated with anti-PD1/PD-L1: 125 patients for NSCLC, 12 for melanoma, 11 for clear cell renal cancer, 5 for bladder cancer, 3 for digestive adenocarcinoma and 2 for mesothelioma. At the cut-off analysis, with a median follow-up of 8.6 months, 63 (40%) patients died, 30(19%) had a progressive disease, 31 (20%) were still receiving an immunotherapy. 25(18%) patients developed irAEs, 18(72%) before 12 weeks of treatment and 7(28%) after. 6 patients had to stop the treatment because of irAEs and 3 of them were still on immunotherapy at the cut-off analysis. Progression disease occurred in 7 patients and 9 died under treatment. Only a trend of efficacy was found between patients with irAEs and those without, with a median PFS of 13.2 vs 9.8 months (HR 1.3; p = 0,4) and a median OS of 28 vs 20 months (HR 1.4; p = 0,4). This statistically non-significant trend was found for OS between patients with an early irAEs and those without any toxicity with a median OS of 28 vs 20 months (HR 1.4; p = 0,8). No statistically difference was found between patients with early irAEs and late irAEs. Conclusions: In our study, 18% of the patients had irAEs. Patients with irAEs seemed to have better OS and PFS but no statistically difference was found. This trend is probably related to patients with late toxicity, which reflects the time of treatment and the increasing probability to develop an irAEs. In our study, early irAEs could not be considered as a reliable preemptive factor of response to immunotherapy.
Résumé -Objectifs : Afin de pallier la très courte demi-vie du GHB dans le sang, l'analyse de cheveux a connu, ces dernières années, un intérêt croissant pour la mise en évidence d'une exposition unique au GHB. Cet article fait le point sur les concentrations en GHB rapportées dans les cheveux chez des sujets témoins, des consommateurs réguliers de GHB et des victimes d'agression dans un contexte de soumission chimique. Méthodes : Pour son dosage, la mèche de cheveux est décontaminée par du dichlorométhane et segmentée. Après un broyage fin à l'aide de ciseaux, 5 à 30 mg sont incubés dans de la soude en présence de GHB-d 6 utilisé en tant qu'étalon interne. Le milieu est ensuite acidifié et extrait avec de l'acétate d'éthyle. La phase organique est évaporée et l'extrait sec dérivé par silylation. Les analyses sont réalisées sur un couplage chromatographie gazeuse -spectrométrie de masse en tandem à l'aide d'une source utilisée en mode ionisation par impact électronique. La détection est réalisée en mode MRM après collision induite. Résultats : Dans le cas de consommateurs réguliers de GHB (n = 4), les concentrations en GHB variaient de 9,2 à 229,1 ng/mg, alors que les concentrations physiologiques variaient de 0,2 et 12 ng/mg (n = 423) avec une prévalence de celles comprises entre 0,5 et 5 ng/mg. Dans les cas d'exposition unique (n = 4), la solution réside en l'utilisation des cheveux de l'intéressé comme son propre témoin avec une augmentation significative de la concentration en GHB dans le segment couvrant la période des faits. Dans ces situations (n = 4), une élévation de la concentration en GHB d'un facteur 1,7 à 4,3 a été observée par rapport aux concentrations physiologiques mesurées chez ces mêmes sujets. Conclusion : Les concentrations physiologiques de GHB varient de 0,2 et 12 ng/mg (avec une prévalence de celles comprises entre 0,5 et 5 ng/mg) et entre 9,2 et 229 ng/mg chez des consommateurs réguliers. Dans le cadre d'une exposition unique (soumission chimique), il est observé une élévation de la concentration basale de GHB de l'ordre de 170 à 430 %.Mots clés : GHB, cheveux, CPG-SM/SM, usage détourné, soumission chimique Abstract -Objective: Due to the fact that the half-life of GHB is very short in blood, the potential use of hair to document GHB exposure has been increasingly investigated for many years. The goal of this work is to determine GHB concentrations in controlled subjects, chronic GHB abusers and victims of GHB-facilitated crimes. Methods: For GHB testing, hair strands are decontaminated in methylene and segmented. After grinding the specimen thinly and weighing it (5 to 30 mg), the specimen is incubated in sodium hydroxide in the presence of GHB-d 6 used as an internal standard. After cooling, the homogenate is acidified and extracted with ethylacetate. The organic phase is evaporated to dryness before silylation.
