Samenvatting De huidige stand van zaken omtrent selectief mutisme wordt beschreven. Uit het grote aantal publicaties blijkt dat er evenveel uiteenlopende verklaringen als behandelvormen zijn. Recente onderzoeken wijzen echter steeds meer op het verband tussen selectief mutisme en angststoornissen bij kinderen. Deze ontwikkeling biedt de mogelijkheid om in de behandeling van selectief mutisme aan te sluiten bij die behandelingsstrategiee¨n die effectief zijn gebleken bij angststoornissen. Dit wordt toegelicht aan de hand van de overwegingen en uitgangspunten die aan een door de auteurs ontwikkeld behandelprotocol ten grondslag liggen.Keywords selective mutism Á childhood anxiety disorders Á treatment protocol Soms worden we in de klinische praktijk geconfronteerd met een kind dat hardnekkig zwijgt in sommige sociale situaties, terwijl het vrijuit spreekt in andere situaties. Sinds de eerste beschrijving van dit klinische fenomeen in 1877 door Kussmaul is een groot aantal publicaties verschenen met evenveel uiteenlopende verklaringen als behandelvormen. In veel van de oudere literatuur ging het om gevalsbeschrijvingen. Hoe boeiend deze ook waren, algemene uitspraken over mogelijke oorzaken van dit hardnekkige zwijgen en welke vorm van behandeling geı¨ndiceerd is konden nauwelijks worden gedaan.Pas in het laatste decennium verschenen meer systematisch opgezette onderzoeken op zoek naar een bevredigend antwoord op de vraag hoe dit zwijgen in bepaalde situaties het beste verklaard kan worden. Het doel van dit artikel is tweeledig. Allereerst willen we een overzicht geven van de stand van zaken ten aanzien van mogelijke verklaringen en behandelvormen. In het tweede deel zullen we de uitgangspunten en overwegingen beschrijven die aan ons behandelprotocol ten grondslag liggen.
