Els van Es Carolien GeversDe Spinozapremie van NWO, ook wel 'de Nederlandse Nobelprijs' genoemd, is dit jaar voor de tiende keer uitgereikt. Rien van IJzendoorn, hoogleraar gezinspedagogiek aan de Universiteit van Leiden, is een van de laureaten. Hij kreeg deze premie vanwege zijn onderzoek naar gehechtheidsrelaties. Wij spraken met hem over het ontstaan van ernstige hechtingsproblemen, over kindermishandeling, de rol van de opvoedingsomgeving en over hoopgevende interventiemogelijkheden.
Els van EsWilliam Vule, klinisch psycholoog en hoogleraar toegepaste kinderpsychologie in Londen, is gespecialiseerd in de behandeling van kinderen met een posttraumatische stress-stoornis (PTSS). Acht jaar lang onderzocht hij de stoornis en ontdekte dat PTSS veel vaker bij kinderen en adolescenten voorkomt en vaker chronisch is dan tot dan toe werd aangenomen.Vule adviseerde UNICEF in exJoegoslavie¨, leidde een groot project voor de opvang van jeugdige oorlogsslachtoffers in Bosnie¨, en is bestuurslid van de Children and War foundation, Tijdens dit gesprek praat Vule over zijn werk en over wat de PTSS nu versterkt of juist verzwakt. Hij vertelt over de schuldgevoelens die kinderen kunnen hebben nadat zij een ramp of een oorlog hebben overleefd, maar ook over kinderen uit oorlogsgebieden en over de toekomst van Irak. U werkte al vele jaren als klinisch psycholoog, toen u zich plotseling ging verdiepen in PTSS. Wat was daarvoor de aanleiding?Op 6 maart 1987 zou mijn loopbaan dramatisch veranderen, maar dat had ik op dat moment nog niet in de gaten. Mijn vrouw en ik hadden onze trouwdag gevierd, en ik herinner me het hartverscheurende nieuws over het zinken van de Herald of Free Enterprise Wij reizen ook vaak met zon boot, en ik leefde heel erg met die mensen mee. Enkele dagen later vroeg een advocaat of ik de gezinnen wilde helpen van een paar kinderen die aan boord waren geweest. Ik zei ja, niet beseffend waar ik aan begon. Tot die dag had ik nog nooit van PTSS gehoord. Ik was al 25 jaar klinisch psycholoog en had al heel wat getraumatiseerde kinderen meegemaakt. maar dit? Hier had ik geen kader voor. Ik was z6** onder de indruk van het eerste gezin Herald-slachtoffers, van de mate waarin zij door de ramp overweldigd waren! Zij wisten gewoon niet waar ze moesten beginnen, en ik wist het ook niet. Gelukkig had een collega van mij zich al verdiept in PTSS bij volwassenen. Samen gingen we te rade bij andere collega's, we lazen erover, maar het ging nauwelijks over kinderen. We wisten natuurlijk wel wat we konden met angst, met depressie, met rouw..-maar dit was alles tegelijl! In Amerika was men eind jaren '80 al begonnen met behandeling met exposure bij volwassenen met PTSS. In die materie heb ik me toen verdiept, zodat ik er vertrouwd mee raakte en er zelf, met deze kinderen, ook iets mee kon. De behandeling bleek toegankelijk, was niet te theoretisch en duurde ook niet te lang.Waarin verschillen kinderen die goed met hun trauma kunnen leven van kinderen die dat niet kunnen? Een minderheid van de kinderen vertoont alle tekenen die duidelijk maken dat het echt slecht leven is niet een trauma. Er is in het Verenigd Koninkrijk vijf of zes keer onderzoek gedaan naar kinderen die betrokken zijn geweest bij ernstige auto-ongelukken, waaruit blijkt dat 25 tot 30% van die kinderen PTSS ontwikkelt. Andere kinderen vertonen weer individuele symptomen die niet in de PTSS diagnose voorkomen, maar die wel behandeling behoeven. Toch kun je in het algemeen een paar dingen wel zeggen.
