De laatste jaren is er veel belangstelling voor de fase voorafgaand aan een eerste psychose. Meestal zijn er weken tot jaren voor een psychose al symptomen aanwezig die kunnen duiden op een zich ontwikkelende psychose. Mogelijk zou het beloop van schizofrenie al in de voorfase kunnen worden beı¨n-vloed. Een mogelijkheid hiertoe is cognitieve gedragstherapie. In dit artikel wordt een casus beschreven van een jong meisje met risicosymptomen voor het ontwikkelen van een psychose. In de nabije toekomst zal een multicentrische randomized controlled trial gestart worden naar de effectiviteit van cognitieve gedragstherapie (CGT) bij jongeren met risicosymptomen. Vroege onderkenning risicosymptomenSchizofrenie is een ernstige ziekte die bij ongeveer 1% van de wereldbevolking voorkomt. Naast wanen, hallucinaties en verwardheid die in de psychotische fase op de voorgrond staan, kunnen negatieve symptomen (anhedonie, initiatiefverlies), cognitieve symptomen (concentratiestoornissen, geheugenproblemen) en affectieve symptomen (depressie, angst) het beloop beı¨nvloeden. Na 100 jaar onderzoek naar schizofrenie zijn de aard en oorzaak van schizofrenie nog steeds onbekend. Het beloop van schizofrenie is bij de grootste groep patie¨nten (ongeveer 75%) ongunstig met meerdere psychotische episoden (recidieven) en negatieve symptomen (Linszen e.a., 2001). Ondanks antipsychotische medicatie krijgt het grootste deel van de patie¨nten met schizofrenie een recidiefpsychose. Veel patie¨nten hebben geen ziekteinzicht en ervaren bijwerkingen van de medicatie, bijvoorbeeld gewichtstoename en parkinsonverschijnselen, waardoor ze tegen doktersadvies in zelf met de medicatie stoppen. Iemand met de diagnose schizofrenie heeft meestal een langdurige ziekte met een grote kans op een ongunstig beloop in het vooruitzicht.Onderzoek naar de aard, oorzaak en behandeling van schizofrenie heeft laten zien dat de behandeling en de prognose van schizofrenie slechts 10% verbeterd zijn (Hogarty & Ulrich, 1998). Door die marginale vooruitgang werd naar nieuwe wegen gezocht om de ziekte beter te begrijpen en beter te kunnen behandelen. Maanden en soms zelfs jaren voorafgaand aan het uitbreken van de eerste psychose, ervaren de meeste patie¨nten in de zogenoemde prodromale fase van schizofrenie beangstigende symptomen zoals vluchtige betrekkingsideee¨n, derealisatie en visuele, akoestische en tactiele waarnemingsstoornissen. Daarnaast zijn er vaak affectieve klachten zoals angst en depressie.Een patie¨nt in de prodromale fase van een psychose heeft vaak het gevoel dat er iets vreemds aan de hand is wat hij niet helemaal onder woorden kan brengen.
