RESUMOObjetivo: apresentar na língua portuguesa a terminologia, conceito e descrição dos estágios da lesão por pressão estabelecidos pelo National Pressure Ulcer Advisory Panel em 2016. Método: estudo descritivo, reflexivo do tipo ensaio teórico. Para nortear este estudo foi utilizado como matriz o resultado do consenso realizado pelo órgão americano em 2016. Resultado: o termo úlcera por pressão passou a ser denominado lesão por pressão, o conceito e a descrição dos estágios da lesão foram alterados. São apresentados detalhamentos das lesões classificadas em Estágio 1: Pele íntegra com eritema não branqueável; Estágio 2: Perda de espessura parcial da pele com exposição da derme; Estágio 3: Perda total da espessura da pele; Estágio 4: Perda total da espessura da pele e perda tissular; Lesão por Pressão não Estadiável: Perda da pele em sua espessura total e perda tissular não visível; e Lesão por Pressão Tissular Profunda: descoloração vermelho escura, marrom ou púrpura, persistente e que não embranquece. Descreve ainda as definições adicionais: Lesão por Pressão relacionada a Dispositivos Médicos e Lesão por Pressão em Membrana Mucosa. Conclusão: o National Pressure Ulcer Advisory Panel ampliou o conceito e acrescentou novas apresentações da lesão por pressão, informações que podem descrever melhor o comportamento de tais lesões. ResumoEste estudo teve como objetivo descrever as características das dimensões de estrutura e de processos dos Serviços de Atenção à Saúde da Pessoa Ostomizada (SASPO) no Estado de Minas Gerais, Brasil. Foi uma pesquisa transversal e descritiva realizada na Rede Estadual de Atenção à Saúde das Pessoas Ostomizadas no Estado de Minas Gerais, em 2011. Foram analisados critérios relacionados à estrutura e aos processos de 28 unidades de SASPO distribuídas em 13 macrorregiões de saúde. Os dados foram obtidos por meio de questionários estruturados com base nas normas governamentais para o serviço de referência ambulatorial para os estomizados. Foi realizada a distribuição de frequências para a análise descritiva dos dados. Os resultados mostraram que 72% dos SASPO têm estrutura para o cadastro dos pacientes e a dispensação de dispositivos e que 40% das unidades correspondem a serviços do tipo II. Quanto aos processos, 96% das unidades contavam com a assistência de enfermeiros, porém 52% deles sem capacitação. Percebeu-se ainda que esses profissionais estão mais envolvidos com as ações relacionadas à operacionalização do programa do que à assistência clínica. Conclui-se que os SASPO de Minas Gerais contam com estrutura e processos relacionados ao cadastro, controle e dispensação de equipamentos coletores e dispositivos.Palavras-chave: estomia; avaliação de serviços de saúde; estrutura dos serviços. AbstractThis study aimed to describe the characteristics of the structure and processes dimensions of ostomized patients' Health Care Services (SASPO) in the State of Minas Gerais, Brazil. It's a cross-sectional and descriptive survey carried out in 28 SASPO of Minas Gerais in 2011. Criteria related to structure and processes were analyzed in these 28 health care units distributed in 13 health macro-regions. The data were obtained from structured questionnaires based on governmental regulations for reference service to ambulatory of the ostomized patients. Frequency distribution was used for the descriptive analysis. The results showed that 72% of the units have a structure able to register the patients and dispense devices, and that 40% of them correspond to the service classified as type II. Regard to the processes, 96% of the units relied on nurses, but 52% of them had no qualification. It was also observed that these professionals are more involved in actions related to the program implementation than in health care. It was concluded that the SASPO of Minas Gerais have structure and processes related to registration, control and dispensing of collector bags and devices.
RESUMOOs objetivos do estudo foram estimar a prevalência de lesões crônicas de um município de Minas Gerais (MG), identificar o perfil clínico, epidemiológico e sociodemográfico dos pacientes com lesão e descrever as características das lesões e o tratamento recebido. Trata-se de estudo epidemiológico, descritivo exploratório, transversal realizado em um município de médio porte da Zona da Mata de MG, com amostra de 104 participantes com lesão crônica provenientes das 25 equipes de Estratégia de Saúde da Família e dos sete Postos de Saúde do referido município que atenderam aos critérios de inclusão. Para a coleta de dados os participantes passaram por avaliação clínica, incluindo a avaliação da lesão e alguns dados foram extraídos do prontuário. A prevalência estimada de lesões crônicas foi de 0, 164% (1,64/1.000 habitantes), os participantes tinham idade média de 66,6 anos; a maioria eram mulheres; com baixa escolaridade e renda mensal. A maioria possuía doenças associadas e fazia uso de um ou mais medicamentos. O tempo médio de existência das lesões foi de dois anos e a maioria estava localizada nas pernas e era de etiologia venosa, sendo tratadas com pomadas. O médico era o principal responsável pelo tratamento e as trocas de curativos eram realizadas pelo paciente ou cuidador. Concluiu-se que a prevalência obtida foi compatível com a literatura e os resultados obtidos subsidiarão os enfermeiros e gestores na organização da rede de assistência ao usuário com lesão crônica do município onde a pesquisa foi realizada. Recomenda-se a realização de estudos semelhantes em outros municípios para comparação de dados.The objectives of the study were to estimate the prevalence of chronic wounds in a city of Minas Gerais, Brazil, to identify the clinical, epidemiological and sociodemographic profile of the patients with wounds and to describe the characteristics of the wounds and the treatment received. This is an exploratory, descriptive, cross-sectional study carried out in a medium-sized city in Zona da Mata/MG, with a sample of 104 participants with chronic wound from the 25 Family Health Strategy and the seven Health Centers city that met the inclusion criteria. For the data collection the participants underwent clinical evaluation, including evaluation of the wound and some data were extracted from the medical record. The estimated prevalence of chronic wounds was 0. 164% (1.64 / 1,000 inhabitants); the participants had a average age of 66.6 years; most were women; with low schooling and monthly income. Most had associated illnesses and used one or more medications. The average duration of the wounds was 2 years, and most were in the legs and were of venous origin, being treated with ointments. The physician was primarily responsible for the treatment and the dressing changes were performed by the patient or caregiver. OBJECTIVE To evaluate the costs of a topical treatment of pressure ulcer (PU) patients in a hospital unit for treatment of chronic patients in 2014. METHOD This is an activity-based costing study. This method encompasses the identification, measurement and pricing of physical and human resources consumed for dressings. RESULTS Procedure costs varied between BRL 16.41 and BRL 260.18. For PUs of the same category, of near areas and with the same type of barrier/adjuvant, the cost varied between 3.5% and 614.6%. For most dressings, the cost increased proportionally to the increase of the area and to the development of PU category. The primary barrier accounted for a high percentage of costs among all items required to the application of dressings (human and material resources). Dressings applied in sacral PUs had longer application times. CONCLUSION This study allowed us to understand the costs involved in the treatment of PUs, and it may support decision-makers and other cost-effectiveness studies.
