Цель-оценить распространенность и факторы риска артериальной гипертензии (АГ) у больных ревматоидным артритом (РА) молодого и среднего возраста. Материал и методы. Обследованы 222 больных РА (34% с ранним РА) без сопутствующих кардиоваскулярных заболеваний. Средний возраст 47±9,7 года; мужчин было 15%; 60% больных были серопозитивны по ревматоидному фактору (РФ), среднее значение DAS 28 составило 5,77±1,1, HAQ-DI-1,58±0,7. Ассоциации между факторами риска и АГ анализировали с помощью моделей логистической регрессии с поправкой на пол и возраст. Результаты. АГ обнаружена у 153 (69,4%) больных (при раннем РА-у 55%). У 56,3% больных повышение артериального давления было впервые зафиксировано после дебюта РА. С АГ при РА возраст (отношение шансов [ОШ] 2,06; 95% доверительный интервал [ДИ] 1,51-2,81), серопозитивность по РФ (ОШ=3,05; 95%

Лидирующую позицию среди не-посредственных причин преждевременной смерти при РА занимают сердечно-сосудистые заболевания (ССЗ), обу-словленные ускоренным развитием атеросклероза: ин-фаркт миокарда, острая сердечно-сосудистая недостаточ-ность, декомпенсация кровообращения, ишемический или геморрагический инсульт [2, 3]. У пациентов с РА риск развития ССЗ в 1,5-2 раза выше, чем в общей популяции [4, 5], и сопоставим с таковым у больных сахарным диабе-том (СД) II типа (2,16 и 2,04 соответственно) [6]. Факторы риска (ФР) ССЗ не обеспечивают адекватного учета степе-ни риска развития ССЗ у больных РА [7, 8].Все большее количество фактов свидетельствует о ключевой роли воспаления в возникновении и прогресси-ровании атеросклероза [9]. Это подчеркивает необходи-мость поиска более точных маркеров для уточнения сер-дечно-сосудистого риска у больных РА. В последнее время в качестве интегрального показателя сердечно-сосудисто-го риска у больных РА рассматривается жесткость артери-альной стенки [10], одним из методов оценки которой яв-ляется определение скорости распространения пульсовой волны (СРПВ). Результаты ряда работ свидетельствуют о снижении эластических свойств сосудистой стенки при РА в отсутствие факторов риска ССЗ [11][12][13]. Увеличение же-сткости артериальной стенки наряду с повышенным арте-риальным давлением (АД) способствует росту смертности при РА [14]. В свою очередь, артериальная гипертензия (АГ) является ведущим фактором риска развития CCЗ у больных РА и встречается в 16-63% случаев [15, 16].В настоящее время вопрос о вкладе хронического воспаления при РА в увеличение жесткости сосудов оста-ется дискуссионным; кроме того, требуется дополнитель-ное изучение динамики показателей СРПВ при развитии АГ у больных РА.Цель. Оценить у больных РА эластические свойства сосудистой стенки в сопоставлении с течением основного заболевания и динамикой АД по результатам наблюдения в течение 1 года.Материал и методы. В период с 2007 по 2009 г. на базе Ивановского городского ревматологического центра про-ведено динамическое наблюдение (в течение 1 года) за 52 пациентами с достоверным диагнозом РА, установленным в соответствии с критериями Американской коллегии рев-матологов (ACR) [17]. Подавляющее большинство пациен-тов составляли женщины среднего возраста со средней длительностью РА 72 (24; 125) мес, с умеренной и высокой степенью активности процесса, преимущественно со II-IV рентгенологической стадией заболевания, II-III функ-циональным классом (ФК), без сопутствующих ССЗ (табл. 1). ...
