RESUMO A leishmaniose visceral no Brasil estava inicialmente associada a áreas rurais, mas devido às diversas alterações no ambiente como, desmatamentos, urbanização e intenso processo migratório, ocorreu a expansão das áreas endêmicas, levando à urbanização da doença, principalmente nas regiõesAs leishmanioses são doenças enzoóticas e zoonóticas causadas por protozoários parasitas, morfologicamente similares, do gênero Leishmania (Kinetoplastida: Trypanosomatidae), podendo acometer o homem 6 .A leishmaniose visceral (LV) vem se tornando um importante problema de Saúde Pública, devido à sua incidência e alta letalidade, não só nas Américas mas na Europa, África, Ásia e Oriente Médio 17 . Nas Américas, a LV ocorre desde o México até a Argentina, sendo que cerca de 90% dos casos humanos descritos são procedentes do Brasil 16 . A LV apresenta amplo espectro epidemiológico no mundo, ocorrendo em vastas áreas tropicais e subtropicais do globo, podendo apresentar-se como zoonose, antroponose ou antropozoonose, estas duas últimas, quando o homem atua como reservatório no ciclo de transmissão do parasito 21 .
A study of the phlebotomine sand fly fauna was carried out in an endemic area of American visceral leishmaniasis (AVL) in the municipality of Porteirinha, in the Brazilian state of Minas Gerais. Captures were performed with CDC light traps in 7 districts, 5 days per month, during 2 consecutive years (January 2000 to December 2001). A total of 3240 sand flies were captured and identified. Sixteen species were found, among which 15 belonged to the genus Lutzomyia and one to the genus Brumptomyia. Lutzomyia longipalpis, a proven vector of AVL, was the predominant species (71.85%) throughout the time period. The interference of climatic factors (temperature, humidity, and rainfall) over the populational dynamics of the sand flies was determined. Statistical analysis of the data showed a significant correlation among the number of phlebotomine sand flies collected, rainfall, and humidity, whereas the effect of temperature was negligible, in that particular region. The present open pilot study was conducted to assess the efficacy of azithromycin for the treatment of patients with cutaneous leishmaniasis in Araçuaí and Varzelândia, MG. Twenty-four patients with less of six months of disease evolution were treated after clinical examination, Montenegro test and a biopsy. The treatment schemes consisted of oral doses of 500 mg per day for 3, 5 and 10 days and of 1000 mg for two days. A clinical control was performed monthly and treatment cycles were repeated when necessary until full reepithelialization of the lesions. On the occasion of the final evaluation, 20 patients had completed the study and 17 of them (85%) were cured. The time to obtain a cure was 60 days ifor 6 (30%) patients, 90 days for 7 (35%), and 120 for 4 (20%). The three patients with treatment failure received a pentavalent antimonial for 20 days. No adverse reactions to the medication were observed and a 14 month follow-up did not show recurrence in any patient. These results suggest that azithromycin can be a good therapeutic option for the treatment of cutaneous leishmaniasis caused by Leishmania Viannia brasiliensis. Key words: Cutaneous leishmaniasis. Azithromycin. Leishmania Viannia brasiliensis.Resumo O presente estudo piloto aberto avaliou a eficácia da Azitromicina em pacientes com leishmaniose cutânea, em Araçuaí e Varzelândia, MG. Foram tratados 24 pacientes após exame clínico, teste de Montenegro, e biópsia, e que tivessem menos de seis meses de evolução da doença. Os esquemas de tratamento empregaram doses, pela via oral, de 500 mg por dia durante 3,5 e 10 dias e de 1000 mg durante dois dias. Mensalmente foi realizado controle clínico e quando necessário, repetidos ciclos do medicamento, até reepitelização completa das lesões. Na avaliação final 20 pacientes terminaram o estudo, dos quais 17 (85%) foram curados. O tempo de cura foi de 60 dias em 6 (30%) pacientes, de 90 dias em 7 (35%) e de 120 dias em 4 (20%). Os três pacientes que tiveram falha terapêutica receberam antimonial pentavalente durante 20 dias. Não foram observadas reações adversas ao medicamento e o seguimento até 14 meses pós-tratamento não mostrou recidiva em nenhum dos pacientes. Estes resultados sugerem que a Azitromicina pode ser uma boa opção terapêutica para o tratamento de leishmaniose cutânea causada por Leishmania Viannia brasiliensis.
To characterize an area of endemic leishmaniasis, aiming to test a candidate leishmania vaccine, a prospective epidemiological survey was implemented in 1999 in a rural area of Varzelândia, Minas Gerais, Brazil. From a total of 1,253 persons in 246 households,1,170 were included, of whom 593 (50.6%) were males and 662 (56.5%) were under 21 years of age. A Montenegro intradermal test performed in 1,120 individuals and evaluated in 1,020 was reactive in 282 (27.6%). Serological testing through indirect immunofluorescence and ELISA was performed in 970 individuals (82.9%). Antibodies to Leishmania sp. were detected in 117 (13.1%) and 170 (17.5%) IntroduçãoAs leishmanioses estão em expansão devido às graves modificações nos ecossistemas, sobretudo o desflorestamento para assentamentos populacionais, abertura de estradas, projetos de irrigação, construção de usinas hidrelétri-cas e urbanização desmedida, entre outros. Ademais, desnutrição, susceptibilidade gené-tica, síndrome da imunodeficiência adquirida e resistência do parasita aos antimoniais pentavalentes em vários países, agravam a situação 1,2 .O Brasil figura entre os cinco países que relatam mais de 90% dos casos de leishmaniose visceral e entre os sete com 90% dos casos de leishmaniose tegumentar no mundo 1,3 . A urbanização das leishmanioses no país é um problema de saúde pública e, nas últimas décadas, ocorreram surtos em várias capitais 4,5 . A incidência da doença aumentou substancialmente e o sub-registro de casos impede conhecer a magnitude do problema 1,6,7 .O controle das leishmanioses é prioridade da Organização Mundial da Saúde (OMS) e medidas como eliminação de vetores com inseticidas no domicilio, peridomicílio e colares impregnados para prevenir a infecção canina, além do sacrifício de cães infectados, não causaram o impacto esperado 8,9 . Uma vacina eficaz é outra alternativa, e diversos candidatos estão em fase de pesquisa clínica 10,11 . ComentáriosO inquérito foi realizado numa área com características de colonização antiga, onde há pouca mata a ser derrubada e em seu lugar as diversas atividades agropecuárias estão associadas à transmissão permanente dessa infecção 2,15 . Isso justifica a presença de cicatrizes cutâneas sugestivas de LTA em 25% dos habi- 16 . Neste estudo, foram vistos 297 (25,4%) casos com lesões cicatriciais. Destes, 168 (59,6%) com IDRM positiva, em contraste com 27,6% observados na população geral da área. Essas lesões, quando bem caracterizadas, podem servir como marcador clínico-epidemiológico de LTA mais sensível e concordante com os testes sorológicos do que a IDRM, embora outras condições como traumatismos, queimaduras e piodermites etc. possam deixar cicatrizes semelhantes.A alta prevalência de reatividade ao antíge-no de Montenegro (27,6%) dos casos tem sido assinalada em outras áreas onde LTA é endêmi-ca e não está necessariamente associada a lesões ativas ou cicatriciais.
