The study allowed a thorough evaluation of the NI distribution profile in Belo Horizonte, Minas Gerais, Brazil, and showed it to be a serious public health problem that requires interinstitutional efforts so that effective action can be taken.
Entre as variáveis usuais em estudos de mortalidade, a causa de morte vem se destacando como uma das mais importantes, especialmente, quando analisada sob as óticas do sexo e idade. Por outro lado, uma das grandes limitações das estatísticas oficiais de mortalidade é a sua restrição à tabulação de uma só causa -a causa básica de morte -como é então denominada. Esse procedimento implica em grande perda de informações especialmente quando muitos diagnósticos são mencionados no atestado de óbito. Chamblee & Evans 6 ressaltam que, embora a seleção da causa básica de morte seja feita através de regras uniformes e padronizadas, são muitas vezes arbitrárias e nem sempre levam a uma seleção satisfatória. Assim, muitas doenças e lesões mencionadas na declaração de óbito não podem ser selecionadas como causa básica por determinismo das regras de seleção e, por isso, não aparecem nas
Objetivos: analisar os óbitos de menores de um ano residentes em Belo Horizonte, segundo critérios de evitabilidadepropostos na Atualização da Lista de Causas de Mortes Evitáveis por Intervenção do Sistema Único de Saúde. Métodos: estudo descritivo que utilizou dados dos bancos dos Sistemas de Informação sobre Mortalidade e sobre Nascidos Vivos do Ministério da Saúde (DATASUS), no período de 2006 a 2011. As causas básicas de morte foram classificadas segundo critérios de evitabilidade. Foram calculados os coeficientes de mortalidade, a mortalidade proporcional por componentes e por principais grupos de causas. Resultados: o coeficiente de mortalidade infantil apresentou decréscimo de (18,7%) no período. Dentre as causas evitáveis, observou-se que as taxas declinaram nos subgrupos: reduzíveis por ações adequadas de diagnóstico e tratamento (38,8%) e reduzíveis por adequada atenção ao feto e ao recém nascido (30,6%). Os subgrupos reduzíveis por adequadas atenção à mulher na gestação e ações de promoção a saúde, vinculadas às ações de atenção cresceram (17,1% e 22,7%), respectivamente. Conclusões: a utilização da referida lista, permitiu identificar quais óbitos são mais passíveis de prevenção e avaliar qual categoria requer maior investimento para redução da mortalidade. Mostrou boa aplicabilidade para evitabilidade de óbitos nos recém-nascidos menores de 1000 gramas.
Acinetobacter baumannii colonization and infection, frequent in Intensive Care Unit (ICU) patients, is commonly associated with high morbimortality. Several outbreaks due to multidrug-resistant (MDR) A. baumanii have been reported but few of them in Brazil. This study aimed to identify risk factors associated with colonization and infection by MDR and carbapenem-resistant A. baumannii strains isolated from patients admitted to the adult ICU at HC/UFMG. A case-control study was performed from January 2007 to June 2008. Cases were defined as patients colonized or infected by MDR/carbapenem-resistant A. baumannii, and controls were patients without MDR/carbapenem-resistant A. baumannii isolation, in a 1:2 proportion. For statistical analysis, due to changes in infection control guidelines, infection criteria and the notification process, this study was divided into two periods. During the first period analyzed, from January to December 2007, colonization or infection by MDR/carbapenem-resistant A. baumannii was associated with prior infection, invasive device utilization, prior carbapenem use and clinical severity. In the multivariate analysis, prior infection and mechanical ventilation proved to be statistically significant risk factors. Carbapenem use showed a tendency towards a statistical association. During the second study period, from January to June 2008, variables with a significant association with MDR/carbapenem-resistant A. baumannii colonization/infection were catheter utilization, carbapenem and third-generation cephalosporin use, hepatic transplantation, and clinical severity. In the multivariate analysis, only CVC use showed a statistical difference. Carbapenem and third-generation cephalosporin use displayed a tendency to be risk factors. Risk factors must be focused on infection control and prevention measures considering A. baumanni dissemination.
ObjectiveTo analyze infections of the surgical site among patients undergoing clean-wound surgery for correction of femoral fractures.MethodsThis was a historical cohort study developed in a large-sized hospital in Belo Horizonte. Data covering the period from July 2007 to July 2009 were gathered from the records in electronic medical files, relating to the characteristics of the patients, surgical procedures and surgical infections. The risk factors for infection were identified by means of statistical tests on bilateral hypotheses, taking the significance level to be 5%. Continuous variables were evaluated using Student's t test. Categorical variables were evaluated using the chi-square test, or Fisher's exact test, when necessary. For each factor under analysis, a point estimate and the 95% confidence interval for the relative risk were obtained. In the final stage of the study, multivariate logistic regression analysis was performed.Results432 patients who underwent clean-wound surgery for correcting femoral fractures were included in this study. The rate of incidence of surgical site infections was 4.9% and the risk factors identified were the presence of stroke (odds ratio, OR = 5.0) and length of preoperative hospital stay greater than four days (OR = 3.3).ConclusionTo prevent surgical site infections in operations for treating femoral fractures, measures involving assessment of patients’ clinical conditions by a multiprofessional team, reduction of the length of preoperative hospital stay and prevention of complications resulting from infections will be necessary.

RESUMOObjetivo: analisar os fatores relacionados à não investigação dos óbitos fetal e infantil ocorridos no Vale do Jequitinhonha, Minas Gerais. Métodos: trata-se de estudo ecológico que teve, como unidades de análise, todos os 33 municípios da Região Ampliada de Saúde Jequitinhonha, Minas Gerais. Nesses municípios foi aplicado um questionário a um membro do Comitê de Prevenção do Óbito Infantil e Fetal com questões sobre a composição dos comitês, mecanismos de investigação do óbito infantil, fatores dificultadores para a investigação dos óbitos e critérios de evitabilidade entre os anos 2007 e 2012. Foi feita análise descritiva com frequências simples, com o intuito de descrever o perfil dos municípios estudados. Resultados: dos 598 óbitos infantis e 477 fetais registrados, apenas 22,2 e 18,4% foram investigados, respectivamente. Constatou-se a existência de problemas de infraestrutura, técnico-operacionais e políticos que interferiram na prática de investigação. Conclusão: a operacionalização da investigação do óbito infantil e fetal, apesar de exigido por lei, apresenta deficiências, o que acarreta elevado número de óbitos não investigados no período do estudo, o que poderá comprometer as ações para a redução da mortalidade infantil e fetal e a qualidade da assistência infantil. ABSTR ACTThis study aims to analyse aspects related to the lack of investigation of foetal and infant deaths in the Jequitinhonha Valley, State of Minas Gerais. This is an ecological study whose unit of analysis were the 33 municipalities in the Extended Jequitinhonha Health Region. A questionnaire was applied to a member of the Infant and Foetal Death Prevention Committee of each municipality. The questions were related to the committees' structure, mechanisms of infant death investigation, difficulties related to the investigation of deaths, and avoidable criteria between 2007 and 2012. A descriptive analysis with simple frequencies aiming at profiling the studied municipalities was performed. Of the 598 infant and 477 foetal deaths registered, only 22.2% and 18.4%, respectively, were investigated. The researchers detected that infrastructure, technical, operational and policy issues interfered with the investigation. Although required by law, the investigation over infant and foetal deaths is deficient. This led to a large number of deaths being left uninvestigated. Such situation may hinder programmes for the reduction of infant and foetal mortality and the quality of childcare.
