RESUMOÉ aceito que a deficiência de ferro acarreta conseqüências deletérias para o desenvolvimento cognitivo, no entanto, é difícil quantificar esse papel uma vez que ele é determinado conjuntamente com fatores sócio-econômico-ambientais. Mesmo assim, o controle da anemia diminui uma fração do risco do subdesenvolvimento cognitivo e isso significa um avanço positivo na qualidade de vida. Tendo em mente a ampliação do conceito de saúde para o de preservação da vida com qualidade, as ações dirigidas à atenção à saúde devem ser incorporadas às ações governamentais programáticas mais abrangentes e intersetoriais. Background: Improvements in the iron status of a population as result of food fortification are expected at the long term. In Brazil, the effectiveness of mandatory flour fortification with iron has been evidenced mostly from surveys on gestational anemia after 1 or 2 years from its implementation, in 2004. Our aim was to assess hemoglobin (Hb) concentrations and the prevalences of anemia and linked erythrocyte morphology patterns among pregnant women in 2006 and 2008. Methods: The study design was retrospective and cross-sectional. The analysis was based on secondary data in 546 medical records from women at the times of their first prenatal attendance in 13 public primary healthcare units of the Butantan Administrative Region from São Paulo (SP), Brazil. Anemia was evaluated from Hb concentration (cut-off <11.0 g/dL) and erythrocyte morphology patterns from mean corpuscular volume (MCV), mean corpuscular Hb concentration (MHC), and red cell distribution widths (RDW). Recorded sociodemographic and obstetric data included maternal age, gestational age, ethnoracial self-classification, and residence type. Student's t tests, analysis of variance, Chi-squared tests, and multiple linear and logistic regressions were employed in the statistical analysis using a significance level of 5 %. Results: The prevalence of anemia was 9.7 % in 2006 and 9.4 % in 2008 (P = 0.922), with no significant difference in mean Hb concentrations (P = 0.159). Normocytosis (normal MVC), normochromia (normal MHC), and anisocytosis (high RDW) were found in most anemia cases, suggesting that the low Hb concentrations resulted from mixed causes. In multiple regression analysis, gestational age at the first prenatal attendance was an independent predictor of low Hb and of having anemia. Moreover, black ethnoracial self-classification was associated with lower Hb.
Desde 1977, sanitaristas brasileiros estão preocupados com a anemia e suas conseqüências, especialmente entre gestantes. As perdas de capacidades físicas e mentais, dificilmente mensuráveis resultantes dessa má nutrição, implicam altos custos de tratamentos médicos e, principalmente, significativas perdas de capital humano. A fim de controlar esta deficiência, programas de intervenção foram introduzidos no país em 2004 e 2005: a fortificação com ferro das farinhas de trigo e milho e a suplementação profilática para crianças com sulfato ferroso. No entanto, a maioria das poucas avaliações disponíveis desses programas teve resultados inesperados: não houve redução da prevalência de anemia de gestantes atendidas à primeira consulta nos serviços públicos brasileiros, depois de dois anos de fortificação das farinhas com ferro. Este resultado está associado a dois fatores: 1) a baixa biodisponibilidade dos compostos de ferro usados 2) a baixa ingestão de produtos à base de farinha de trigo e de milho devido ao alto preço e/ou hábito alimentar. Os preços do trigo mostram uma tendência de aumento, o que conseqüentemente, levará a diminuição no seu consumo, principalmente entre a população de baixa renda. Uma das metas do Ministério da Saúde é avaliar constantemente as estratégias adotadas visando aperfeiçoar sua implementação, identificar possíveis causas de vieses e buscar alternativas para o controle da anemia e suas conseqüências que afetam tanto a produtividade individual como o desenvolvimento da nação como um todo.
