Objective: Developing and validating a scale to assess perioperative thirst discomfort. Method: A methodological research conducted in 2014 and 2015 at the surgical center of a public hospital in the north of Paraná, Brazil. The scale was developed after literature review, followed by face and content validation, and reliability assessment through its internal consistency and inter-observer equivalence. Results: Seven items make up the scale: dry mouth; dry lips; thick tongue; thick saliva; dry throat, bad taste and desire to drink water. The content validity index for attributes and items was 0.98, and the reliability index was 1 for the scale attributes and items. Internal consistency assessed by Cronbach's alpha was 0.91 and inter-rater equivalence was 1, as measured by weighted kappa coefficient. Conclusion: The Perioperative Thirst Discomfort Scale showed high content and reliability indexes.
Objective: To describe breastfeeding prevalence and associated factors, in the municipality of Londrina, PR, Brazil. Methods: This quantitative, descriptive study included interviews of 770 companions of children younger than 12 months of age during an anti-polio vaccination campaign in August 2008. Results: A total of 72.5% of the children were breastfed in the first hour after birth; 33.8% were exclusively breastfed from zero to six months of age, 53.7% until the fourth month and 7.8% until the sixth month; 51.5% were in continued breastfeeding between 9 and 12 months of age. The women who breastfed the most were 35 years old or older, had a bachelor's degree, had more than one child and were on maternity leave. Conclusion: Breastfeeding rates increased in the city, however, analyzing health services actions is needed to improve these rates. Las mujeres que más amamantaron tenían edad igual o superior a 35 años, escolaridad de tercer grado, más de un hijo y estaban con licencia-maternidad. Conclusión: los índices de lactancia materna del municipio presentaron evolución, sin embargo se hace necesario analizar las acciones en los servicios de salud para mejorar esos índices.
In 2012, the Baby-friendly Hospital Initiative for Neonatal Wards (Neo-BFHI) began providing recommendations to improve breastfeeding support for preterm and ill infants. This cross-sectional survey aimed to measure compliance on a global level with the Neo-BFHI's expanded Ten Steps to successful breastfeeding and three Guiding Principles in neonatal wards. In 2017, the Neo-BFHI Self-Assessment questionnaire was used in 15 languages to collect data from neonatal wards of all levels of care. Answers were summarized into compliance scores ranging from 0 to 100 at the ward, country, and international levels. A total of 917 neonatal wards from 36 low-, middle-, and high-income countries from all continents participated. The median international overall score was 77, and median country overall scores ranged from 52 to 91. Guiding Principle 1 (respect for mothers), Step 5 (breastfeeding initiation and support), and Step 6 (human milk use) had the highest scores, 100, 88, and 88, respectively. Step 3 (antenatal information) and Step 7 (rooming-in) had the lowest scores, 63 and 67, respectively. High-income countries had significantly higher scores for Guiding Principles 2 (family-centered care), Step 4 (skin-to-skin contact), and Step 5. Neonatal wards in hospitals ever-designated Baby-friendly had significantly higher scores than those never designated. Sixty percent of managers stated they would like to obtain Neo-BFHI designation. Currently, Neo-BFHI recommendations are partly implemented in many countries. The high number of participating wards indicates international readiness to expand Baby-friendly standards to neonatal settings. Hospitals and governments should increase their efforts to better support breastfeeding in neonatal wards.
RESUMOEste trabalho relata a experiência de um projeto de atendimento integral ao prematuro e sua família desde sua implantação. O referencial teórico utilizado foi o Cuidado Centrado na Família, proposto pelo Instituto para o Cuidado Centrado na Família. O projeto inclui atendimento hospitalar, ambulatorial e domiciliar, nas seguintes etapas: contato e apoio no dia do nascimento; atendimento individual semanal durante a hospitalização do prematuro; reuniões com grupo de pais; treinamento da família nos cuidados do recém nascido para a alta; visita domiciliar antes e após a alta hospitalar; referência para a unidade de Saúde da Família e consultas de enfermagem no ambulatório. Desde sua implantação em 2007, o projeto tem cadastradas 309 crianças atendidas e suas famílias. As experiências vividas no projeto confirmam a necessidade de um novo olhar sobre o acompanhamento desses bebês e suas famílias que enfrentam muitas situações conflitantes. Por meio da atuação no projeto, acreditamos poder contribuir para a redução da morbimortalidade pela melhora da qualidade de vida dessas crianças e famílias. INTRODUÇÃOO parto prematuro desperta ansiedades e tensões em toda a família. Desta forma, trabalhar com o bebê prematuro implica trabalhar com seus pais, especialmente em relação a seus sentimentos e atitudes. É preciso entender o funcionamento emocional, abolir a postura crítica, hostilidade e juízos de valores, e auxiliar na resolução da ansiedade, medo, fantasias e rejeições dos pais em relação ao filho. Neste contexto, a melhor abordagem seria uma assistência centrada na criança e sua família, de modo que suas demandas servam como o principal determinante das intervenções fornecidas, pois geralmente essas crianças prematuras permanecem internadas por períodos prolongados e a hospitalização é considerada um trauma tanto para a criança como para sua família. Nesta perspectiva, a continuidade da assistência em nível domiciliar é essencial (1) . O grande progresso técnico-científico ocorrido nas últimas décadas culminou em uma considerável melhoria na assistência perinatal. A partir dos anos de 1970, observou-se uma diminuição progressiva da mortalidade neonatal influenciada, pelo advento das unidades de terapia intensiva neonatais (UTIN), por uma melhor compreensão das principais fisiopatologias que acometem os prematuros, pela reposição do surfactante exógeno pelo aprimoramento dos equipamentos utilizados no tratamento e recuperação dos prematuros, entre outros avanços. Entretanto, a morbidade dessas crianças a curto e longo prazo não tem acompanhado a redução da mortalidade. Ao contrário, houve um aumento de doenças associadas à morbidade, decorrente do aumento da sobrevida dos prematuros extremos, expondo esta população a um maior risco de sequelas e essa família a uma enormidade de situações de crise e sofrimento.Muito tem sido proposto para o aprimoramento da assistência à saúde do recém-
Objective: To evaluate the effectiveness of the menthol package (lip hydration and ice popsicles) compared to a package of non-menthol measures (lip hydration and ice popsicles) as a way to relieve thirst in patients in the Anesthetic Recovery Room. Method: Randomized and parallel trial study, with 120 patients randomized patients in an experimental group - menthol measurements (n=59) and control group - measures without menthol (n = 61). Results: There was a significant (p<0.05) decrease in intensity, hydration, dryness and taste in the oral cavity between the three moments of assessment/intervention in the two groups. The difference was significant in the experimental group for thirst intensity at the second assessment/intervention point (p<0.05) after a single administration of the menthol package. Conclusion: There was a reduction in thirst intensity in both groups. Patients who received menthol packages showed a significant decrease in intensity after a single evaluation/intervention time. Considerando os efeitos deletérios da dor pós-operatória na criança e seu direito a receber alívio, sentiu-se a necessidade de estudar esse tema. Pesquisa qualitativa desenvolvida na unidade pediátrica de um hospital público de Londrina - Paraná, visando investigar a percepção dos pais quanto ao manejo da dor pós-operatória pela equipe de enfermagem e seu envolvimento neste processo. Foram entrevistados 10 familiares que acompanhavam crianças submetidas a cirurgias. Os dados foram agrupados em dois temas: caracterização do manejo da dor pós-operatória e estratégias adotadas para o alívio da dor pós-operatória. A terapia farmacológica foi mencionada como principal método utilizado pela equipe para analgesia. Quanto à atuação dos pais, referiram o uso de estratégias não farmacológicas como: distração, brinquedos, entre outros. O tratamento farmacológico é supervalorizado pelos profissionais e pelos pais. Técnicas não farmacológicas são extremamente úteis, porém, é necessário incentivar os pais a participar ativamente no manejo da dor pós-operatória de seus filhos.
Objective: To identify and analyze the evidence on the effect of chewing gum on thirst in adults. Method: Integrative review, guided by the question: “What is the scientific evidence about the effects of chewing gum on adult’s thirst?” LILACS, PubMed, Scopus and Web of Science were the databases consulted. Results: Of a total of 2,414 articles found, 12 papers were selected whose publications varied between 1991 and 2016. The use of chewing gum resulted in increased salivary volume (five studies), xerostomia relief (seven studies), and thirst reduction (four studies). It was not possible to establish the number of chewing gums per day, being frequent the recommendation to use as desired. There was a predominance of studies with patients under dialysis and with cancer. Conclusion: Chewing gum is an effective strategy to reduce thirst discomfort in adults.
