OBJETIVO: avaliar a correção da cifose, da altura do corpo vertebral e da redução dos fragmentos retropulsados nas fraturas toracolombares tipo explosão por ligamentotaxia. MÉTODOS: estudo retrospectivo avaliando um total de 238 pacientes com fraturas toracolombares tipo explosão do grupo A3 pela classificação de Magerl et al., dos quais 63 deles elegíveis para a pesquisa. Todos foram tratados com fixador interno pedicular e tinham imagens radiográficas e tomográficas pré e pós-operatórias. RESULTADOS: No estudo, foi constatada, no pós-operatório imediato, uma correção da cifose vertebral no local da fratura de 87%, com correção de 51% na altura do corpo vertebral e redução de 40% dos fragmentos retropulsados intracanal. CONCLUSÕES: o uso de fixador interno por via posterior permite, ao realizar ligamentotaxia, a restauração da altura do corpo vertebral fraturado, do alinhamento sagital da coluna e descompressão do canal espinhal, minimizando as comorbidades em relação à cirurgia pela via anterior e à laminectomia para descompressão do canal.
RESUMOObjetivo: Comparar a rigidez biomecânica entre a coluna toracolombar intacta, a coluna com fratura explosão e a coluna com fratura explosão associada à fixação pedicular curta em suínos. Métodos: 30 amostras de coluna toracolombar (T11-L3) de suínos foram divididas em três grupos com 10 amostras cada. O Grupo 1 representava a coluna intacta, o Grupo 2 representava a coluna com fratura explosão e o Grupo 3 a fratura explosão associada à fixação pedicular curta. Foi realizado o corte ósseo em "V" do terço médio do corpo vertebral comprometendo a coluna anterior e média de L1 para simular a fratura explosão. No Grupo 3 foi realizada a fixação pedicular com Pinos de Schanz. Os grupos foram submetidos ao teste biomecânico em compressão axial controlada. Os parâmetros de carga (N) e deslocamento (mm) eram gerados em um gráfico instantâneo e a rigidez (N/mm) foi determinada. O teste era interrompido quando ocorria uma queda súbita na curva no gráfico indicando falência da amostra. Resultados: A rigidez das colunas fraturadas foi 53% menor do que a rigidez das colunas intactas, sendo essa diferença estatisticamente significativa (p < 0,05). A fixação pedicular curta apresentou uma rigidez 50% maior do que a coluna fraturada. Esse aumento foi estatisticamente significativo (p < 0,05). A rigidez da fixação pedicular curta foi 30% menor do que a rigidez das colunas intactas. Essas diferenças foram estatisticamente significativas (p < 0,05). Conclusão: A fixação pedicular curta não é suficiente para restabelecer a rigidez da coluna intacta nos testes biomecânicos in vitro de compressão axial pura em modelos de fratura toracolombar de suínos.
OBJETIVO: Avaliar os resultados clínicos e radiográficos de pacientes submetidos à corpectomia e fixação com placa cervical, com seguimento de dois anos. MÉTODOS: Análise retrospectiva de 2003 a 2009. Avaliaram-se o tipo de fratura (classificação AO), o grau de déficit neurológico (inicial e após dois anos - escala de Frankel), a taxa de complicações e a taxa de incorporação do enxerto ósseo (de acordo com radiografias dois anos depois da cirurgia). RESULTADOS: Vinte e um pacientes foram avaliados. De acordo com a classificação AO, 14 eram grupo A, 3 B e 4 C. Ao todo, sete pacientes apresentaram déficit neurológico inicial completo (Frankel A) e permaneceram com o déficit neurológico completo após dois anos. Dos seis pacientes que apresentaram déficit neurológico inicial incompleto (Frankel B, C e D), 33% (2 de 6) apresentaram melhora de um nível na escala de Frankel e 50% (3 de 6) deles evoluíram para recuperação completa (Frankel E). Os oito pacientes que não apresentaram lesão neurológica inicial (Frankel E) permaneceram sem déficit neurológico após dois anos. Três complicações clínicas foram verificadas: uma fístula esofágica, uma soltura asséptica do implante e uma infecção no sítio doador de enxerto. Todos os pacientes obtiveram consolidação do enxerto ósseo. CONCLUSÃO: A corpectomia cervical no tratamento da fratura-explosão permite a recuperação neurológica nos pacientes com lesão neurológica incompleta e apresenta baixos índices de complicações.
Objective: Report safety and clinical outcomes of percutaneous intradiscal decompression procedures in patients with discogenic pain that failed conservative treatment. Minimally invasive surgical treatments are a trend. Methods: A retrospective study describing the clinical outcomes of patients treated with Nucleoplasty TM and DeKompressor TM , with follow up of 5 years. Results: Thirty-four (n=34) patients underwent intradiscal decompression with a follow-up period of 5 years, 21 had resolution of pain, eight underwent lumbar interbody arthrodesis, one underwent surgical discectomy and 4 had residual back pain. There were no adverse events. Conclusion: It was demonstrated the same safety obtained in other studies that also evaluated the use of triangular safety zone for percutaneous procedures. The success rate with the percutaneous procedure is similar to other studies published in a recent meta-analysis. The comparative studies, prospective and controlled will be useful to better define the effectiveness of percutaneous procedures.Keywords: Intervertebral disc displacement; Decompression; Intervertebral disc; Low back pain. Objective:Radiographic evaluation of patients with thoracolumbar burst fractures treated with unconvencional transpedicular fixation, which includes additional fixation of the fractured vertebra associated with transverse connector - Crosslink clamp.Methods:Retrospective study evaluating a total of 68 patients operated at the Hospital do Trabalhador de Curitiba, Orthopedics Service, of which 15 were eligible for the study. All patients were treated with posterior pedicle fixation and intermediate screw. The assessment by the Cobb angle method was performed on preoperative, immediate postoperative and one year after surgery radiographs.Results:It was observed an average reduction of kyphosis of 8.3o (77%), with a loss of 1.34o in late postoperative compared to the immediate postoperative period.Conclusion:The method of fixation of burst-type fractures of the thoracolumbar spine by the posterior approach with intermediate screw was effective in maintaining the reduction achieved in the immediate postoperative period and after one year of evolution.
