An acquired partial pituitary insufficiency of unknown origin with selective ACTH and STH deficiency was demonstrated in a 44-year-old patient. The clinical course over many years corresponds to subclinical Addison's disease with occasional acute crises. Ossification of both auricular cartilages and anhidrosis were outstanding signs. There is possibly a connection between the glucocorticoid deficiency over many years with normal mineralocorticoids and the auricular cartilage ossification.
EinleitungDas Gesetz zur Reform des Risikostrukturausgleichs in der gesetzlichen Krankenversicherung (GKV) ist seit dem 1.1.2002 in Kraft. Es empfiehlt die Entwicklung strukturierter Behandlungsprogramme (Disease Management Programmes, DMPs) bei chronischen Krankheiten und normiert deren Anforderungen und Ausgestaltung. Die ersten beiden Krankheiten, für die DMPs eingeführt werden sollen, sind der Diabetes mellitus und der Brustkrebs der Frau.Durch ihre Bindung an den Risikostrukturausgleich sind die DMPs ganz in den Gravitationsbereich der GKV und der von ihr finanzierten ambulanten und stationären akutmedizinischen Versorgung geraten. Die Versorgungsnotwendigkeiten und -verläufe bei chronisch Kranken zeichnen sich aber gerade dadurch aus, dass sie nach-oder nebeneinander heterogene Leistungen erforderlich machen. Es ist bei chronisch Kranken schwierig, präventive, kurative, palliative, rehabilitative und pflegerische Aufgaben, Ziele, Interventionen und die jeweils zuständigen Leistungsträger trennscharf voneinander abzugrenzen.Rehabilitation ist -und in Deutschland nicht erst seit Einführung des SGB IX -eine Grundfunktion jedes entwickelten Gesundheitswesens. Ihr Ziel ist die Sicherung von Selbstbestimmung und sozialer Teilhabe, gerade auch der Teilhabe am Arbeitsleben. Deutschland verfügt über ein einzigartiges System rehabilitativer Einrichtungen und Leistungen; deren größerer Teil wird immer noch von der gesetzlichen Rentenversicherung (GRV) belegt bzw. finanziert. Es hat sich neben unserem System der akutmedizinischen Versorgung und in einer gewissen -auch räumlichen -Distanz zu ihm entwickelt. In anderen Ländern werden vergleichbare Leistungen unter dem Stichwort "multidisciplinary team care/rehabilitation" zumeist in ambulanten oder teilstationären Settings erbracht und erscheinen generell besser integriert.Es ist das Ziel der folgenden "Indikationsleitlinie", die bei chronisch Kranken unverzichtbare Integration von akutmedizinischer und rehabilitativer Versorgung zu fördern und die Potentiale der Rehabilitation gerade auch solchen Patienten zu eröffnen, die sich in DMPs der GKV eingeschrieben haben.Die Hinweise wurden von einer Arbeitsgruppe der Kommission "Leitlinien" der Deutschen Gesellschaft für Rehabilitationswissenschaften (DGRW) -in enger Zusammenarbeit mit Rehabilitationsklinikern und Vertretern der Allgemeinmedizin sowie der GRV und der GKV -erarbeitet (Liste der Mitglieder s. u. Fn. 1). Die hier veröffentlichte Fassung fand die Zustimmung aller Beteiligten.Die Koordinatoren danken allen Mitgliedern der Arbeitsgruppe für die konzentrierte und kollegiale Zusammenarbeit; sie danken der DGRW für die vielfältige Unterstützung der gemeinsamen Arbeit.Hinweise zur Indikationsstellung 1. Die folgenden Hinweise richten sich zuerst an Praxis-und Krankenhausärzte. Sie wollen diese zu einem aktiven Screening nach Rehabilitationsbedarf bei Patienten mit einem Diabetes mellitus Typ 2 ermutigen. Clinical findings, symptoms and predisposing factors were studied in 43 patients with oesophageal candidiasis, 40 patients with peptic oesophagitis and 40 normal controls. Oesophageal candidiasis was confirmed cytologically. 2.4% of patients who had undergone gastroscopy had oesophageal candidiasis; only three of them had simultaneous candidiasis of the oral cavity. Cardiac failure, oesophageal varices, hiatus hernia and gastric ulcer were common associated disorders. 42% of patients with candidal oesophagitis were symptom-free. Most common symptoms were vomiting, retrosternal and epigastric pain. Peptic oesophagitis was more frequently associated with symptoms. Predisposing factors were present in 88% of cases of oesophageal candidiasis: alcoholism, hepatic cirrhosis, diabetes mellitus, malignant tumours and other wasting diseases. 18 patients had had treatment with cimetidine; they included all 13 patients whose candidiasis was first detected at check endoscopy.
For the following reasons it has, despite plausible different opinions, seldom been possible realize ambulant rehabilitation offers:For historical reasons gut rehabilitation centers lie outside of conurbations. A model of gastrointestinal-oncologic rehabilitation supports developing an entirely new concept for ambulant treatments, instead of merely copying the presently in-patient rehabilitation therapy. In the exemplary thoughts of the GRVS, which were already presented five years ago, quantitative factors have not been taken into consideration satisfactorily. Considering this the following three diagnosis groups become relevant - diabetes m. - obesity - gastrointestinal cancer. The needed ambulant rehabilitation concepts are to be connected to the long-term ambulant treatment, available close to the patient's home city.
