Severe periodontitis is prevalent in Down syndrome (DS). This study aimed to identify genetic variations associated with periodontitis in individuals with DS. The study group was distributed into DS patients with periodontitis (n = 50) and DS patients with healthy periodontium (n = 36). All samples were genotyped with the “Axiom Spanish Biobank” array, which contains 757,836 markers. An association analysis at the individual marker level using logistic regression, as well as at the gene level applying the sequence kernel association test (SKAT) was performed. The most significant genes were included in a pathway analysis using the free DAVID software. C12orf74 (rs4315121, p = 9.85 × 10−05, OR = 8.84), LOC101930064 (rs4814890, p = 9.61 × 10−05, OR = 0.13), KBTBD12 (rs1549874, p = 8.27 × 10−05, OR = 0.08), PIWIL1 (rs11060842, p = 7.82 × 10−05, OR = 9.05) and C16orf82 (rs62030877, p = 8.92 × 10−05, OR = 0.14) showed a higher probability in the individual analysis. The analysis at the gene level highlighted PIWIL, MIR9-2, LHCGR, TPR and BCR. At the signaling pathway level, PI3K-Akt, long-term depression and FoxO achieved nominal significance (p = 1.3 × 10−02, p = 5.1 × 10−03, p = 1.2 × 10−02, respectively). In summary, various metabolic pathways are involved in the pathogenesis of periodontitis in DS, including PI3K-Akt, which regulates cell proliferation and inflammatory response.
Summary Tooth extraction is the dental treatment most commonly carried out among HIV positive patients. In this paper we propose a simple protocol for dental extractions in patients of this type, based on a review of the literature and on our own experience in this field. The factors to be considered when planning a dental extraction for HIV positive patients are: diagnosis of the lesion justifying the need for tooth extraction and medical assessment of the patient including the infection pathway, the stage of the disease, laboratory tests and drug therapy. The most frequent post extraction complications are a delay in the wound healing process, alveolitis and surgical wound infection. Fortunately, these complications tend to be rather uncommon and not too severe; they can be managed on an out‐patient basis and their prevalence will probably tend to decrease with the introduction of the new anti‐retroviral drugs for HIV‐1. Résumé L'extraction dentaire est la procédure la plus communément pratiquée parmi les patients HIV séropositifs. Dans cet article, nous proposons un protocole simple d'extraction dentaire chez des patients de ce type, basé sur la littérature et notre propre expérience dans ce domaine. Lorsque l'on prévoit une extraction dentaire sur des patients séropositifs, il faut considérer les facteurs suivants: diagnostic de la lésion justifiant le besoin d'extraction et évaluation médicale du patient incluant la pathologie infectieuse, l'état de la maladie, les tests de laboratoire et la thérapie médicamenteuse. Les plus fréquentes complications rencontrées après une extraction portent sur le retard à la cicatrisation, l'alvéolite et l'infection de la plaie. Heureusement, ces complications sont en général peu nombreuses et pas trop graves; elles peuvent être gérées sans hospitalisation et leur prévalence diminuera probablement avec l'introduction des nouveaux médicaments anti‐rétrovirus pour HIV 1. Zusammenfassung Bei der Zahnextraktion handelt es sich um die bei HIV‐positiven Patienten am häufigsten durchgeführte Art der Behandlung. Dem Papier liegt die Absicht zugrunde, eine einfache und bewährte Extraktionsmethode vorzustellen, die sowohl auf der analysierten Fachliteratur als den praktischen Erfahrungen der Autoren fuz̀t. Zu den im Vorfeld abzuklärenden Faktoren zählen u.a.: Diagnose der Läsion und Ermittlung des bestehenden Extraktionsbedarfs, Untersuchung des medizinischen Status des HIV‐positiven Patienten einschliez̀lich Bestimmung des Infektionsweges und des Krankheitsstadiums, Labortests sowie Medikamententherapie. Alles in allem treten postoperative Komplikationen nur sporadisch auf und wenn, handelt es sich dabei zumeist um eine Verzögerung des Wundheilprozesses, Alveolitis oder chirurgische Wundinfektionen, d.h. alles in allem um weniger schwerwiegende Komplikationen, deren Behandlung ambulant erfolgen kann.
