С 2015 по 2016 г� в Республике Марий Эл было выделено 85 культур нетуберкулезных микобактерий от 65 пациентов� Из них 15 (23,1%) пациентов были с установленным диагнозом туберкулеза и проходили лечение от туберкулеза� В остальных случаях (76,9%) нетуберкулезные микобактерии выявляли у пациентов с неспецифическими заболеваниями органов дыхания и жалобами респираторного характера� Анализ возрастно-половой структуры выделителей нетуберкулезных микобактерий показал, что большинство из них являлись лицами пожилого возраста (от 55 до 64 лет)� Преобладающим видом нетуберкулезных микобактерий в случае их неоднократного выделения у пациентов и подозрения на диагноз микобактериоза оказались культуры вида M. intracellulare� В случаях однократного выделения преобладающим видом был M. gordonae, что могло быть расценено как носительство либо контаминация�
Проведен сравнительный анализ алгоритма микробиологической диагностики, разработанного и используемого в Центральном НИИ туберкулеза, и алгоритма лабораторной диагностики и мониторинга лечения туберкулеза легких и туберкулеза с лекарственной устойчивостью возбудителя на основе применения современных быстрых молекулярных методов, разработанного экспертами Европейской лабораторной инициативы совместно с секретариатом Европейского регионального бюро Всемирной организации здравоохранения (ВОЗ), для стран Европейского региона ВОЗ� Показано, что, несмотря на некоторые имеющиеся в этих алгоритмах различия, основные принципы, на которых они базируются, и подход к диагностическому процессу одинаковы� В обоих случаях для первичной диагностики всех предполагаемых случаев туберкулеза предпочтение отдается молекулярным диагностическим тестам и посевам на жидких средах� Это позволяет обеспечить быструю, точную диагностику и раннее выявление туберкулеза с лекарственной устойчивостью возбудителя, что в итоге повышает эффективность лечения и снижает трансмиссию туберкулеза� Ключевые слова: алгоритм микробиологической диагностики туберкулеза, быстрые молекулярные тесты Для цитирования: Севастьянова Э� В�, Черноусова Л� Н� Современные алгоритмы микробиологической диагностики туберкулеза // Туберкулёз и болезни лёгких�-2018�-Т� 96, № 7�-С� 11-17�
