Архангельск, РоссияЦель исследования -выявление прогностической значимости динамических тестов и показателей в оценке восприимчивости к инфузионной нагрузке (ИН) после аортокоронарного шунтирования на работающем сердце В исследование включено 32 пациента, которым последовательно выполняли тест с повышением положительного давления в конце выдоха (ПДКВ-тест), тест с минимальной ИН и стандартный тест с ИН Также с использованием двух систем мониторинга оценивали вариабельность пульсового давления (ВПД PiCCO и ВПД NK ), вариабельность ударного объема (ВУО), индекс сердечно-легочного взаимодействия (HLI) и индекс вариабельности плетизмограммы Пациент считался восприимчивым к ИН, если в ходе стандартного теста с ИН сердечный индекс увеличивался более чем на 15% Снижение среднего артериального давления в ходе ПДКВ-теста более чем на 5 мм рт ст позволяло выявить чувствительных к ИН пациентов (AUC 0,73; p = 0,03) В ходе теста с минимальной ИН в качестве ориентира восприимчивости к инфузии можно использовать снижение ВУО и ВПД PiCCO более чем на 2% (AUC 0,75 и 0,77; p < 0,05) При стандартном тесте с ИН выявить респондеров к инфузионной терапии позволяла изолированная оценка ВУО (AUC 0,77), ВПД PiCCO (AUC 0,84), ВПД NK (AUC 0,71) и HLI (AUC 0,77) (p < 0,05)Ключевые слова: восприимчивость к инфузионной нагрузке, динамические показатели, динамические тесты Для цитирования: Фот Е В, Изотова Н Н, Смёткин А А, Кузьков В В, Киров М Ю Оценка восприимчивости к инфузионной нагрузке после аортокоронарного шунтирования на работающем сердце // Вестник анестезиологии и реаниматологии -2018 -Т 15, № 5 -С 5-13Thirty-two adult patients after off-pump coronary artery bypass grafting were enrolled into a prospective observational study All patients received positive end expiratory pressure test (PEEP-test), mini-fluid challenge test (mFCT) and standard fluid challenge test (sFCT) Pulse pressure variation using two monitoring systems (PPV PiCCO , PPV NK ), Stroke volume variation (SVV), heart-lung index (HLI) and plethysmogram variability index (PVI) were assessed before and after sFCT The patients with an increase in cardiac index by ≥15% after fluid challenge were defined as fluid responders According to receiver operating characteristic analysis, changes in mean arterial pressure induced by PEEP-test identified fluid responsiveness with AUC 073 (p = 003) The reduction in PPV PiCCO and SVV during mFCT predicted positive response to the fluid load with AUC 077 and 075, respectively (p < 005) Response to sFLT can be predicted by the assessment of PPV PiCCO (AUC 084), PPV NK (AUC 071), SVV (AUC 077) and HLI (AUC 077) (p < 005)
Objective This study aimed to assess the predictive value of invasive and non-invasive dynamic parameters for evaluation of fluid responsiveness after off-pump coronary artery bypass grafting. Methods Thirty-two adult patients after off-pump coronary surgery were enrolled into a single-center pilot prospective observational study. After arrival to the intensive care unit, all patients received standard fluid challenge test to assess fluid responsiveness. The patients with an increase in cardiac index ≥ 15% after the test were defined as fluid responders. We measured pulse pressure variation using 2 monitoring systems (PPV PiCCO and PPV NK ), stroke volume variation, heart–lung interaction index, and plethysmogram variability index before and after standard fluid challenge test. Results After intensive care unit admission, the absolute values of stroke volume variation, PPV PiCCO , PPV NK , and heart–lung interaction index were significantly higher among fluid responders ( P < .05). Response to standard fluid challenge test was predicted by dynamic assessment of PPV PiCCO (area under the curve 0.84), PPV NK (area under the curve 0.71), stroke volume variation (area under the curve 0.77), and heart–lung interaction index (area under the curve 0.77) ( P < .05). The plethysmogram variability index value did not demonstrate any predictive ability regarding fluid responsiveness (area under the curve 0.5, P = .1). Conclusions In patients after off-pump coronary surgery, both invasive parameters such as pulse pressure and stroke volume variations and non-invasive parameter such as heart–lung interaction index are able to predict fluid responsiveness. Thus, these dynamic parameters can be used to guide fluid therapy during the early postoperative period after off-pump coronary surgery.
