The aim of the study was to compare endosonographic signs of pancreatic lesion in patients with inflammatory bowel diseases (IBD) and in patients with diagnosed chronic pancreatitis (CP). Materials and methods. 62 patients with IBD (39 with ulcerative colitis (UC), 23 with Crohn’s disease (CD)), 33 patients with previously established CP without IBD and 42 patients without CP and IBD were examined. All patients underwent endosonographic examination of the pancreas with an assessment of parenchymal and ductal criteria according to the Rosemont classification. Results. There was no statistically significant difference when comparing changes in the pancreas in UC and BC. Ductal disorders in patients with UC and CD were more common than in the control group. Parenchymal changes of the pancreas were statistically significantly more common in patients with IBD than in CP, and ductal, on the contrary, less common. At the same time, a significant difference was determined between the frequency of ductal changes in IBD and CP when compared with the control group. According to the Rosemont classification, certain CP was more common in patients with previously verified gland damage, probable - in patients with IBD. Conclusion. Based on the results of our study, with different etiologies of CP, endosonographic changes in the pancreas will manifest in different ways. A detailed approach to the etiology of CP will optimize the diagnosis and treatment of pancreatic insufficiency, and as a consequence of the underlying pathology of the intestine. Timely administration of therapy for changes in the pancreas can help to avoid progressive changes in the pancreas and improve the prognosis of the disease.
Rationale: At present, there is no established standard for the differential diagnosis of Crohn's disease (CD) and ulcerative colitis (UC). Five to 15% of the patients have clinical, endoscopic, morphological, and radiological signs both of UC and CD and are therefore diagnosed with indeterminate colitis. However, the timely and correct diagnosis is essential for the choice of treatment strategy. Aim: To evaluate the potential of endoscopic ultrasound examination (EUS) for the differential diagnosis of UC and CD and to identify the most pathognomonic criteria for each of the disorders. Materials and methods: This was a prospective single center controlled study including 50 in-patients who were treated in the Department of Gastroenterology for inflammatory bowel disease (IBD) exacerbation. The inclusion criteria were an established diagnosis of IBD, absence of strictures, colon tumors, and infectious diseases. The control group consisted of 15 patients without IBD. In all patients, colon EUS with a radial ultrasound sensor and measurement of the intestinal wall thickness, assessment of the degree of intestinal wall vascularization by color Doppler mapping and measurement of the wall density by compression elastography were performed. Results: From 50 patients of the study group, 28 (16 men and 12 women aged 18 to 49 years) had CD of the colon and 22 (8 men and 14 women aged 22 to 60 years) had total UC. In CD, the colon wall thickness was 2-fold higher than in the control group (5.66 0.36 vs 2.62 0.11; р 0.001) and 1.5-fold higher than in the UC patients (5.66 0.36 vs 3.96 0.13; p = 0.002). In UC, the intestinal wall was thickened mainly due to its mucosal and submucosal layers (in 82% of the cases, р 0.001 compared to that in the CD patients; diagnostic sensitivity 82%, specificity 93%). In CD, transmural thickening of the intestinal wall was more common (in 68% of the cases, p 0.001 compared to that in UC; sensitivity 68%, specificity 91%), as differentiation of the intestinal wall layers was absent (in 68% of the cases, p 0.001 compared to UC, sensitivity 68%, specificity 100%). The intestinal wall in most cases of UC was less well vascularized that in the control group (54.6% of the cases, p 0.001), whereas in CD, on the contrary, the vascularization was increased (71.4% of the cases, р 0.001); the sensitivity and specificity of this parameter being 54.6 and 82%, for UC vs 71.4 and 77.3% for CD, respectively). Compression elastometry showed that in CD, type 2 staining (E. Ueno classification) was more frequent (45%) compared to UC (22%) and the control group (6%; p = 0.002), which indicates a more dense structure of the intestinal wall in CD patients. Conclusion: The differences in the intestinal wall structure (its thickness, density and degree of vascularization) identified by EUS UC and CD can be the differential diagnostic criteria between these diseases.
РезюмеАктуальность: метод эндоскопической подслизистой диссекции (ESD) позволяет удалять крупные новообразования желудочно-кишечного тракта единым блоком, что способствует значительному снижению риска местного рецидива опухоли. Однако метод ESD технически сложный процесс, сопровождающийся высоким риском операционных осложнений.Цель исследования: модифицировать методику тракции при ESD и оценить ее эффективность. Материалы и методы: экспериментальное сравнительное исследование на биологических препаратах (свиные желудки). Выполнено 144 ESD тремя различными методами: 1 группа -метод стандартной ESD (n-48); 2 группа -ESD с методикой тракции с использованием клипсы и нити (n-48); 3 группа -ESD с модифицированной методикой тракции опухоли (n-48). Результаты: наименьшее время диссекции подслизистого слоя было в 3-й группе 4,89 [2-12] мин, в 1-й и 2-й группах 13,27[4-31] мин и 9,37 [4-26] мин, соответственно, (p = 0,05). Общее время операции в 3-й группе составило 21,10 [12-31] мин, в 1-й -23,85 [12-48] мин и 2-й -25,56 [13-49] мин, p = 0,002. Максимальная скорость диссекции составила в 3-й группе 0,48 [0.20-1.27] (мин/см 2 ), во 2-й и 3-й -0.94[0.47-2.06] (мин/см 2 ) и 1.48 [0.56-4.10] (мин/см 2 ), соответственно, p=0,003. Показатель HM0 в 1-й группе -89,6% [43/48], во 2-й -93,7% [45/48] и в 3-й -100% [48/48], p>0,05. Расслоение мышечных волокон в ходе диссекции подслизистого слоя в 1-й группе -25% [12/48], во 2-й -в 6.25% [3/48] и в 3-й -12.5% [6/48], p=0,389. Заключение: модифицированная методика тракции опухоли при эндоскопической подслизистой диссекции позволяет лучше визуализировать подслизистый слой, увеличить скорость диссекции, тем самым сокращая общее время операции и время диссекции подслизистого слоя. обзор review РезюмеЭндоскопическая резекция слизистой оболочки с диссекцией в подслизистом слое (ЭПД) в настоящее время является передовым методом лечения раннего рака верхних и нижних отделов пищеварительного тракта. Метод ЭПД позволяет удалять опухоли любых размеров единым блоком, в отличие от эндоскопической резекции слизистой оболочки (ЭРС), когда возможно en bloc удалять образования размером менее 20 мм. Несмотря на это преимущество, ЭПД сопровождается большим количеством перфораций и кровотечений, чем EMR. Сложная анатомическая локализация, тонкий мышечный слой стенки органа, фиброз подслизистого слоя и недостаточный опыт оперирующего эндоскопистаосновные причины неполных резекций опухоли, развития осложнений и рецидивов.Разработанные методики тракции опухоли при выполнении подслизистой диссекции позволяют увеличить долю резекции единым блоком и добиться радикальности. Нами представлен широкий арсенал различных методик тракции опухоли при выполнении ЭПД.
