ZusammenfassungDie positive Entwicklung der Überlebensrate extrem unreifer Frühgeborener sowie bei schweren Fehlbildungen ist das Ergebnis medizinischer Fortschritte und Neuerungen. Diese geht aber oftmals mit einer Zunahme von therapiebedürftigen Folgeerkrankungen und einer ausgeprägten psychosozialen Belastung mit späteren Verhaltensauffälligkeiten einher (1). Die Notwendigkeit begleitender psychosozialer Hilfs- und Unterstützungsangebote für diese Kinder und Eltern findet bisher nur an wenigen Perinatalzentren Beachtung. Das Defizit besteht einerseits in einer noch unzureichenden Verknüpfung der pflegerischen, medizinischen und psychosozialen Behandlungen, andererseits stellt sich durch das Fehlen eines funktionierenden Bindeglieds zwischen Kinderklinik und nachstationärer Betreuung eine große Versorgungslücke dar (2). In der vorliegenden Arbeit sind im Rahmen der psychosozialen Begleitung von Eltern Frühgeborener und kranker Neugeborener an unserem universitären Perinatalzentrum im Sinne eines Case-Management-Ansatzes von 2005–2007 Daten erhoben worden. Diese werden in der vorliegenden Arbeit analysiert und diskutiert. 718 Patienten von insgesamt 606 betreuten Familien wurden während des Studienzeitraums erfasst. Die psychosoziale Begleitarbeit umfasst die individuell auf die Familien abgestimmte Betreuung auf Station, sozialrechtliche Beratungen und Fallkonferenzen. Durch die Häufigkeit der Inanspruchnahme kann ihre Effizienz für die Familien belegt werden. Demzufolge sollte an allen Perinatalzentren die psychosoziale Betreuung von Eltern nach dem Case-Management-Ansatz gesetzlich vorgeschrieben werden.
ZusammenfassungTransiente neonatale Myasthenia gravis wird durch Antikörper ausgelöst, die von der Mutter durch die Plazenta auf den Feten passiv übertragen werden. Die Symptome der Erkrankung entwickeln sich erst Stunden bis Tage nach der Geburt. Die betroffenen Kinder zeigen meist Muskelschwäche in Form von Schluck- und Saugproblemen, leises Schreien, ausdrucksschwache Mimik, in 30 % treten respiratorische Komplikationen auf. Bei schweren Formen tritt bereits pränatal eine Arthrogryposis multiplex congenita auf, die auch zum Tode führen kann. Wir berichten über einen Fall der transienten neonatalen Myasthenie, in dem das Neugeborene am 2. Lebenstag durch die typischen Symptome der Erkrankung auffiel.
