Атаксия Фридрейха (АФ) является одним из наиболее распространенных вариантов наследственных атаксий. Это заболевание характеризуется слож-ным фенотипом, включающим неврологические проявления (атаксия, сенсор-ная полиневропатия, дизартрия, нарушение глубокой чувствительности, арефлексия, пирамидные симптомы с вовлечением нижних конечностей, вегетативные нарушения, иногда -психопатологические симптомы), кардио-логические нарушения (кардиомиопатия, сердечная недостаточность, арит-мии), а также нарушения углеводного обмена и скелетные деформации. если неврологические нарушения приводят к значительному ухудшению качества жизни и инвалидизации пациентов, то осложнения со стороны сердечно-сосу-дистой системы являются основными причинами смерти больных с АФ. стоит отметить, что вовлеченность в патологический процесс сердца может оста-ваться не диагностированной. В настоящее время подавляющее большинство зарубежных и отечественных публикаций по АФ посвящены неврологическим проявлениям этого заболева-ния. Несмотря на то, что основной причиной смерти пациентов с АФ являются тяжелая сердечная недостаточность и внезапная сердечная смерть вслед-ствие кардиопатии (выраженной симметричной гипертрофии миокарда с участками интрамиокардиального фиброза), в кардиологическом научно-практическом обществе данной проблеме уделяется мало внимания. Нами представлено клинико-генетическое описание пациента 27 лет с АФ с умеренными неврологическими нарушениями и выраженной гипертрофией миокарда. В статье рассмотрены современные клинико-диагностические аспекты кардиомиопатии, ассоциированной с АФ. Friedreich's ataxia (FA) is one of the most prevalent variants of the inherited ataxias. the disease is characterized by complicated phenotype that includes neurological signs (ataxia, sensory polyneuropathy, dysarthria, disorders of deep sensitivity, areflexia, pyramid symptoms with lower extremities involvement, vegetative disorders, sometimes -psychopathological symptoms), cardiological disorders (cardiomyopathy, heart failure, arrhythmias), and disorders of carbohydrate metabolism, and skeletal deformities. If neurological disorders lead to significant decrease of life quality and disability, cardiovascular complications are the main cause of fatal outcome in FA. It is worthy to mention that involvement of the heart into pathological process might long remain undiagnosed. Recently, most of abroad and local publications on FA are related to neurological presentation. Cardiovascular side of the problem in the scientific society remains underestimated, regardless the risk of sudden death and of heart failure (due to symmetric myocardial hypertrophy with the areas of intramyocardial fibrosis) being high.We present clinical and genetic observation of a 27-year old patient with FA, moderate neurological disorders and severe myocardial hypertrophy. the article is focused on the contemporary clinical and diagnostic aspects of FA associated cardiomyopathy.

Задолго до эры доказательной медицины эти вещества нашли широкое применение в китайской медицине, где желчь медведей и панд использовалась в качестве жаропо-нижающего, противовоспалительного и литолитического средства.Длительное время желчные кислоты рассматрива-лись лишь как вещества, которые регулируют процесс пищеварения, участвуя в переваривании и абсорбции пищевых жиров и жирорастворимых витаминов. Од-нако более углубленное изучение физиологических ме-ханизмов действия позволило им выйти за рамки просто поверхностно-активных веществ.Структурное и функциональное разнообразие со-держащихся в животном организме ЖК позволяет им участвовать не только в процессах переваривания и вса-сывания пищевых жиров, но и проявлять гормонопо-добные эффекты, связанные с активацией мембранных и ядерных рецепторов. Дальнейшее изучение роли желчных кислот позволило получить более чем не-ожиданные результаты. Так, было установлено что желчные кислоты, связываясь с FXR, играют роль «ме-таболических интеграторов» в контроле уровня жиров и глюкозы, а также регулируют энергетический мета-болизм, модулируя генную экспрессию. Через три года после открытия FXR, Maruyama и др. описали G-белковый рецептор клеточной мембраны (GPCR), ко-торый также активировался с помощью ЖК. Namely tissue localization of TGR5 determines biological effects of activation of these receptors. This review focuses on the role of TGR5 as a new pharmacological target for the treatment of patients with metabolic syndrome, diabetes, obesity, atherosclerosis, liver disease and cancer processes.
The article presents the case report of a rare hereditary form of systemic ATTR-amyloidosis in Russian patient with a discussion of approaches to the diagnosis and treatment of this form, also based on the own experience in the management of such patients. Modern ideas about the pathogenesis of the disease as well as detailed information about the clinical manifestations of amyloid cardiopathy and of other organs are presented. The nature of structural and hemodynamic changes in the heart is discussed on the basis of experience, including own, ultrasound examination of the heart in patients with amyloidosis, especially the article focuses the reader's attention on the true infiltrative nature of transtiretin amyloid cardiopathy in contrast to AL-amyloidosis, in which there is a significant inflammatory component that determines a more unfavorable natural course of AL-amyloidosis of the heart. The article discusses the differential diagnosis of different types of amyloidosis, the diagnostic difficulties associated with weak congophilia of transtiretin amyloidosis and at the same time substantiates the need for morphological verification of the diagnosis. Modern methods of treatment of ATTR-amyloidosis are discussed.
Friedreich's disease is a hereditary neurodegenerative multiple organ disease, primarily affecting the most energy-dependent tissues (cells of the nervous system, myocardium, pancreas), the lesion of which is characterized by progressive ataxia, dysarthria, dysphagia, oculomotor disorders, loss of deep tendon reflexes, pyramid signs, diabetes mellitus, visual impairment. Friedreich's ataxia is the most common of all hereditary ataxias; nevertheless, this disease is considered orphan. By its pathogenesis, Friedreich's disease is mitochondrial ataxia, caused by a deficiency in the transcription of the FXN gene, leading to a decrease in the synthesis of the frataxin protein. Frataxin is a protein associated with the inner mitochondrial membrane, which in turn is involved in the formation of iron-sulfur clusters, the lack of which leads to a decrease in the production of mitochondrial ATP, an increase in the level of mitochondrial iron and oxidative stress. The basis of the clinical picture of Friedreich's disease is ataxia of a mixed (sensitive and cerebellar) nature. The steady and gradual progression of neurological symptoms significantly affects the quality of life of patients and is most often the leading reason for seeking medical attention. However, the prognosis is primarily due to the involvement of cardiac tissue in the pathological process. The main causes of death in patients with Friedreich's ataxia are severe heart failure and sudden cardiac death due to cardiomyopathy. The overwhelming majority of foreign and domestic publications on Friedreich's ataxia are devoted to the neurological manifestations of this disease, and little attention is paid to this problem in the cardiological scientific and practical society. The purpose of this review is to provide up-to-date information on modern methods of diagnosing myocardial damage at various stages of Friedreich's disease.
