Низкодифференцированные глиальные опухоли являются наиболее часто встречаемыми злокачественными опухолями головного мозга и составляют около 65% от злокачественных опухолей ЦНС. Диагностика и лечение данной патологии остается острой проблемой в нейроонкологии. Цель исследования: изучение изменений цитокинового состава в ткани различных гистологических и молекулярно-генетических подтипов глиальных опухолей. В исследовании приняли участие 49 пациентов. В основную группу вошли 34 пациента с впервые выявленной супратенториальной глиальной опухолью. В контрольную группу вошли 15 пациентов с впервые выявленной супратенториальной менингиомой. Во время операции у пациентов основной группы проводился забор опухолевой ткани, а у пациентов контрольной группы -прилежащего к менингиоме неизмененного мозгового вещества в функционально незначимой зоне. В основной группе диффузная астроцитома верифицирована у 4 пациентов, у 4 -анапластическая астроцитома и у 26 -глиобластома. Среди пациентов с HGG мутация гена IDH1 обнаружена у 3 пациентов. У 30 пациентов с HGG определен уровень метилирования промотора MGMT. Изучение локального цитокинового состава неизмененного мозгового вещества и опухолевой ткани осуществлялось методом ИФА. При сравнении цитокинового состава неизмененного мозгового вещества с тканью HGG имеются статистически достоверные отличия (p<0,05) в содержании ИЛ-2, ИЛ-4, ИЛ-6, ИЛ-8, ИЛ-10, ФНО-α. У больных с HGG и наличием мутации в генах IDH1 тканевой уровень ИЛ-4 был в 33 раза выше, чем у больных без мутации, а уровни ИЛ-6 и ИЛ-8, напротив, были ниже в 33 и 249 раз соответственно. При уровне метилирования промотора MGMT >10% отмечено статистически достоверное повышение локального уровня ИЛ-1β. Изучение локального цитокинового состава HGG наряду с гистологическими и молекулярно-генетическими аберрациями может служить предиктором течения заболевания и чувствительности к химиотерапии препаратами алкилирующей группы.

By the end of the experiment, subcutaneous xenograft volumes and the tumor growth indices in the groups receiving temozolomide or bortezomib monotherapy, as well as in the temozolomide+bortezomib group, were statistically significantly lower than the corresponding values in the control group. The highest ITG% value was registered in the temozolomide+bortezomib group (85.38%); the values in groups with temozolomide and bortezomib monotherapy were 57.32% and 63.11%, respectively. Studies of the antitumor efficacy of the bortezomib and temozolomide combination in human subcutaneous PDX-glioblastomas demonstrate a synergistic effect of these drugs.
Глиомы -наиболее распространенные опухоли головного мозга. Частота встречаемости глиальных опухолей составляет около 5 случаев в год на 100 000 человек. Максимально возможная циторедукция улучшает выживаемость пациентов, однако вопрос дальнейшей терапии глиом остается актуальным. Одним из перспективных подходов может стать разработка эффективных терапевтических схем на основании молекулярно-ориентированных воздействий на опухоль. Целью настоящей работы было исследование относительной экспрессии 15 генов сигнальных путей, ассоциированных с онкотрансформацией клеток, в 54 образцах опухолей в сравнении со здоровой тканью мозга, генотипированных на наличие соматических мутаций в генах IDH1/2 и ATRX. Случаев мутаций в генах IDH2 и ATRX не выявлено. Мутация R132H гена IDH1 была идентифицирована в 18,5% опухолей, достоверно отличавшихся от опухолей IDH1 дикого типа по транскрипционной активности гена SMAD7 (p=0,012). Аберрантный паттерн относительной экспрессии был характерен преимущественно для глиобластом G IV. По результатам исследования можно выделить группу молекулярных маркеров (SMAD7, SMO, EGLN1/3, HIF1A, KDM1A), с высокой частотой изменяющих транскрипционную активность в глиальных опухолях и перспективных для использования в таргетной терапии и дифференциальной диагностике. Существенная роль в инициации и прогрессии злокачественных процессов отводится микроРНК. МикроРНК представляют собой небольшие некодирующие РНК, которые могут действовать как опухолевые супрессоры или онкогены. Целью работы стало исследование дифференциальной экспрессии микроРНК и возможности их применения в качестве предикторов выживания пациентов с глиомами высокой степени злокачественности. Методом RT-qPCR была произведена оценка уровня относительной экспрессии микроРНК miR-215-5p, miR-22-3p, miR-122-5p, miR-107, miR-324-5p, miR-34a-5p, miR-155-5p, miR-21-5p, miR-497-5p, miR-330-3p, miR-146a-5p, miR-92a-1-5p, miR-326 в 30 парных образцах тканей глиом высокой степени злокачественности и условно нормальных тканей мозга по данным МРТ. В опухолевой ткани были идентифицированы микроРНК с достоверно возросшим (miR-155-5p) и, напротив, снизившимся уровнем экспрессии (miR-215-5p, miR-22-3p, miR-107, miR-324-5p, miR-330-3p, miR-326, miR-122-5p). Обнаружено изменение общей выживаемости пациентов в зависимости от уровня экспрессии микроРНК miR-22-3p, miR-107 и miR-330-3p. Уровень экспрессии микроРНК miR-22-3p, miR-107 и miR-330-3p может быть использован как предиктор в оценке выживания пациентов с глиомами высокой степени злокачественности.
