Background: many researchers report numerous predictors of severe COVID-19 and poor prognosis. However, to make a quick decision, the doctor needs to have a certain set of data that he can use in routine practice to predict the outcome in patients with this disease. Aims: develop and describe a predictive model for determining an unfavorable outcome in COVID-19 patients based on age, objective, laboratory and instrumental data and comorbid pathology. Materials and methods: the study included 447 patients with a laboratory-confirmed diagnosis of COVID-19 who underwent inpatient treatment in the period from March to August 2021. Discriminant analysis was used with cross-validation to build a predictive model. Results: Based on discriminant analysis, a predictive model was developed to predict outcome in patients with COVID-19. Evaluation of clinical findings such as respiratory rate, heart rate, SpO2, laboratory data and computed tomography results on admission to the hospital showed their significance as predictors of poor outcome. The discrimination constant was 0.4435. The sensitivity of the model is 96.4%, the specificity is 90.4%. Conclusion: the developed model will help medical institutions predict the outcome of the disease when a patient is admitted to the hospital and, on this basis, optimize and prioritize the provision of necessary medical care.
Êëèíè÷åñêàÿ áîëüíèöà ¹ 1 ÔÃÓ «Þaeíûé îêðóaeíîé ìåäèöèíñêèé öåíòð» ÔÌÁÀ Ðîññèè, Ðîñòîâ-íà-Äîíó Цель исследования. Разработать схемы применения иммуномодуляторов имудона, виферона, суперлимфа в комплексном лечении часто болеющих детей (ЧБД) с частотой ОРЗ 6 раз и более в год, очагами хронической инфекции рото и носоглотки и интермиттирующим (сезонным) аллергическим ринитом (САР) легкой и средней степени тяжести.Материалы и методы. Обследованы 120 ЧБД в возрасте от 5 до 7 лет (56 мальчиков и 64 девочки). Длительность наблюдения ЧБД составила 2 года.Результаты. Установлено, что в спектре причинно значимых аллергенов преобладают пыльцевые и бы товые аллергены. Микробный пейзаж из очагов хронической инфекции рото и носоглотки характеризу ется преобладанием Staphylococcus aureus -у 71,2%, Сandida albicans -у 28,8%, Streptococcus hаemolуticus -у 50% пациентов. У 67,4% больных преобладают микробные ассоциации из 2 и 3 возбудителей. Наруше ния иммунной системы характеризуются снижением Т клеток, уровня сывороточного IgA у 62,5%, гамма интерферона (ИФН) -у 80%, альфа ИФН -у 60%, повышением сывороточного IgG -у 62,5%, фактора некроза опухоли (ФНО) альфа -у 80,0% и IgE -у 100% детей. Введение иммуномодуляторов имудона, суперлимфа и виферона в комплексное лечение у детей данной категории приводит к уменьшению обсе мененности слизистых активными формами грибов Сandida albicans в 2 4 раза, бактериальной флорой (Staphylococcus aureus, Streptococcus spp.) в 2 3,4 раза, уменьшает воспаление слизистых рото и носоглот ки, гипертрофию аденоидов, снижает частоту ОРЗ в 1,9; 2,5 и 3,1 раза соответственно. У 16,6% ЧБД с САР, не получавших иммуномодуляторы в комплексном лечении, за год наблюдения диагностировано развитие персистирующего аллергического ринита, что не отмечено у групп детей, получавших суперлимф или ви ферон.Ключевые слова: часто болеющие дети, иммунологические показатели, интермиттирующий аллергический ринит легкой и средней степени тяжести, иммунотропные препараты, виферон, имудон, суперлимф. IMMUNOTROPIC AGENTS IN COMPLEX TREATMENT OF CHILDREN WITH INTERMITTENT ALLERGIC RHINITIS AND RECURRENT RESPIRATORY INFECTIONSThe purpose of the research was to develop a prescribing of immunomodulators Imudon, Viferon, Superlymph in the complex treatment of frequently ill children (FIC) with a frequency of ARI 6 or more times a year, foci of chronic infection in oro and nasopharynx, and intermittent (seasonal) AR mild or moderate severity (SAR).Methods: 120 FIC from 5 to 7 years old (56 boys and 64 girls) were followed up during 2 years.
