In October 1986, 7 to 22 days after a meeting at a farm in Paraíba state, 26 individuals presented with a febrile illness associated with bilateral eyelid and lower limb edema, mild hepatosplenomegaly, lymphadenopathy and, occasionally a skin rash. A 11-year-old boy exhibited atrial premature complexes and a 74-year-old patient developed acute heart failure. In two patients hospitalized in São Paulo city, acute Chagas' disease was diagnosed by the demonstration of circulating Trypanosoma cruzi. At autopsy in a fatal case, acute Chagas' cardiomyopathy was demonstrated. Xenodiagnosis were positive in 9 out of 14 tested patients. A specific IgG immune response was found in all patients and specific IgM antibodies were identified in 20 out of 22 tested patients. A epidemiological survey showed the existence of Triatoma brasiliensis in the outbuildings of this farm, but none in the house where most of the guests stayed. A high rate of infection with Trypanosoma cruzi was found in opossums. These observations together with those related to the food consumed by the patients, lead the authors to suggest that the human infections resulted from oral contamination probably originating from naturally infected marsupials in the area or crushed infected bugs.
This study was made with the objective of reevaluating the colon denervation in chronic Chagas' disease. The diameters of neuron perikaryons of the myenteric plexus were measured on paraffin sections in a ring from the sigmoid in Chagas' disease patients, 17 with and 10 without megacolon and in 10 non-chagasic controls. All neurons were counted in ten en-echelon sections. Neuron hypertrophy only occurred in the group with megacolon, and the average increase in diameter was 69.3%. This could generate an error factor in the neuron count by increasing the probability of neurons being seen in a greater number of histological sections. The original result of the neuron count gave medians of 1264, 1961, and 2665 in the groups of chagasic patients with megacolon, without megacolon, and in the control, respectively. The denervation was greater than 55% in only seven megacolon cases (41.2%). After applying a correction factor, the median in the group with megacolon was 746, and the denervation was greater than 55% in 13 cases (76.5%). This occurrence demonstrates the need to apply a correction factor when the neuron count in chagasic megacolon is being evaluated and in the other pathologies where neuron hypertrophy may be found.
Estudo anatomopatológico sistematizado foi feito em 56 esôfagos de chagásicos crônicos (17 com e 39 sem mega) e em 26 de não chagásicos com as seguintes finalidades: 1) avaliar as variações de calibre e espessura da parede do órgão; 2) analisar qualitativa e quantitativamente o plexo mientérico, na tentativa de avaliar a eventual relação entre suas lesões e o aparecimento de megaesôfago (ME); 3) estudar as lesões das musculares procurando verificar sua contribuição na gênese da visceromegalia; 4) pesquisar formas amastigotas do T. cruzi e sua possível relação com o processo inflamatorio; 5) identificar as principais alterações da mucosa. Confirmou-se que as lesões mais intensas localizavam-se na muscular própria e no plexo de Auerbach. Na primeira, as principais alterações foram miosite e fibrose e nos gânglios mientéricos observou-se inflamação e despopulação neuronal, maior nos esôfagos dos chagásicos com dilatação em relação aos sem dilatação e destes em relação aos controles. Entretanto, foram vistos esôfagos de calibre normal, com intensa denervação. Conclui-se que parecem ser múltiplos os fatores que desencadeiam a esofagopatia, especialmente, o ME. A pesquisa de parasitas em oito esófagos com mega e em oito sem ME foi positiva somente em quatro casos, do primeiro grupo. As lesões da mucosa e submucosa não parecem participar do processo.
INTRODUÇÃOEntre as endemias que constituem sério problema médico-social, destaca-se em nosso meio, por sua gravidade e freqüência, a doença de Chagas. Especialmente grave é sua forma cardíaca que freqüentemente torna o portador da mesma incapaz para o trabalho e, em grande número de casos, leva-o à morte. 0 tip o de morte apresentado pelo chagásico crônico é variável. Pode o ó bito resultar de um quadro de insuficiência cardíaca congestiva (I.C.C.), ou de uma causa não relacionada com a tripanossomíase em si (doenças intercorrentes, morte acidental etc). Em certas ocasiões, ainda, a morte sobrevêm subitamente, atingindo indivíduos, que em vida não manifestaram qualquer distúrbio cardíaco ou que somente tiveram discretas manifestações (p. ex., tontu ras) que poderiam ter decorrido de transitórios e ligeiros distúrbios do ritmo.
This research characterizes the acute and chronic phases o f Chagas ' disease in hamsterMany species of animals have been used as models to study the infection of T. cruzi, being the mouse, rat, guinea pig, rabbit, dog and the monkey the most frequently employed9. Although the susceptibility of hamster to T. cruzi is well known, this animal has been not used yet as an experimental model for Chagas' disease. Only a few papers describe the infection in the acute phase (AP) and no one in the chronic phase (CP).Cariola et al10 infected hamsters with the Tulahuen strain of T. cruzi and observed nests of parasites in all organs, great inflammatory reaction, degenerative phenomena and a high hyperplasia of the macrophagic system. They also observed high mortality rates of the infected animals. Osimani and Gurri25 made compared between the infection of many different Uruguayan strains of T. cruzi and observed a higher susceptibility of mice (100%) in relation to hamster (65%). Schoemaker and Hoffman32 infected mice, rats, hamsters and guinea pigs with trypomastigotes of Tulahuen strain of T. cruzi and observed that mice were more severely infected than the other three species and that fat tissue and the adrenal cortex were not infected.In view of the few papers about the infection by T. cruzi in hamster and in order to search for anew experimental model for Chagas'disease, as recommended by WHO38, we decided to study this animal both in the AP and in the CP o f the infection, analysing the parasitologic and the histopathologic aspects, which preliminary results were related in previous communications29 30. 163
Com finalidade de melhor conhecimento da forma indeterminada da doença de Chagas os autores realizaram estudo anatomopatólogico sistematizado de trinta corações de chagásicos assintomáticos falecidos de modo violento. Demonstram que nos portadores da forma em questão da tripanossomiase cruzi o coração e acometido por lesões da mesma natureza, porém de intensidade muito menor do que as observadas em chagásicos crônicos que falecem subitamente ou após periodo variável de insuficiência cardíaca. Baseados em seus achados e em outros dados da literatura, concluem que a infecção chagásica, sem inflamação do coração, se ocorre, é rara. Tecem ainda considerações a respeito do significado das lesões observadas no sistema nervoso autônomo intracardíaco no sistema de condução e sobre a gênese formal da cardite chagásica crônica humana.
Trinta e cinco indivíduos falecidos subitamente e de modo inesperado foram necropsiados em Serviços Médico-Legais em Uberaba e Uberlândia. A sorologia do líquido pericárdico e os achados anatomopatológicos estabeleceram o diagnóstico de doença de Chagas. Todos, exceto dois, eram do sexo masculino e tinham idade média de 38 anos. Resolvemos fazer uma investigação retrospectiva procurando obter de familiares e amigos e nos registros médicos informações sobre as circunstâncias do óbito e manifestações que pudessem constituir possíveis indícios de doença cardíaca e de comprometimento de outros órgãos. Em 31,2% não havia quaisquer sintomas ou suspeita de cardiopatia. Contudo, em 57,1% dos casos conseguimos detectar história de tonturas (42,8%), dispnéia de esforço (48%), perda de consciência (34,2%) e palpitações (31,4%). A disfagia e ou a obstipação estavam presentes em 38%. Não houve correlação entre a sintomatologia e a intensidade da miocardite. Em 56% o êxito letal surgiu enquanto desenvolviam esforço físico ou tinham emoção. Pelo modo como ocorreu o óbito pode-se concluir que a iminência deste não foi pressentida pelo paciente ou circunstantes em 43,8% dos casos; no restante, o óbito súbito foi precedido de indícios, tais como gestos, pedidos de ajuda, etc. Somente em 5 dos 35 casos o eletrocardiograma havia sido realizado meses ou anos antes do óbito: todos apresentavam alterações.
